    Abstract Background: The aim of the present systematic review and meta-analysis was to evaluate the therapeutic efficacy of bosentan, a dual endothelin receptor antagonist, for systemic sclerosis (SSc) patients with digital ulcers (DUs).
    Materials and methods: A systematic search of MEDLINE, Embase, Web of Science, and Scopus was done using appropriate keywords till September 2021. Weighted mean difference (WMD) as the effect of therapeutic efficacy of bosentan on continuous outcomes was an estimate. Furthermore, the pooled prevalence of diffuse SSc and limited SSc was computed. Fixed or random effects models when appropriate were used for data synthesis.
    Results: Totally, 469 patients, with a mean age ranging from 48.1 to 63.7 years, from 8 studies were included in the systematic review and meta-analysis. The pooled frequency of diffuse SSc and limited SSc was 56% (95% confidence interval [CI]: 39%, 73%) and 44% (95% CI: 27%, 61%). The pooled prevalence of new DUs following bosentan treatment was 21% (95% CI: 10%, 33%). The results of the meta-analysis showed a pooled mean decrease of WMD: -0.09 (95% CI: -0.020, 0.02,
    Conclusion: Bosentan possibly is an efficacious treatment option for SSc-related DUs. Although further large-scale randomized clinical trials are required to confirm the preliminary finding and underlying mechanisms of action.

    Abstract Background: Sjogren's syndrome, as a chronic autoimmune disease, involves in lymphocytic infiltration in the exocrine glands. As the result of exocrine glands disruption, the clinical hallmark of this disease including dryness of mouth and eyes along with fatigue and joint pain occur. However, heterogeneity of clinical presentations among newly diagnosed adult patients with Sjogren's syndrome leads to difficulty in its diagnosis. One of the diagnostic criteria for Sjogren's syndrome is the presence of autoantibodies in patient serum. One of the novel biomarkers suggested for diagnosis of Sjogren is alpha-fodrin antibody. In this study, we aimed to evaluate the diagnostic power of anti-α-fodrin antibody among the Iranian population for the first time.
    Materials and methods: We recruited 82 individuals in this study. Alpha-fodrin were measured in case and control with Elisa kit as 16.71 (9.84) and 18.44 (11.54).
    Results: There was no any significant difference between two groups regarding alpha-fodrin level (
    Conclusions: Alpha-fodrin test did not have acceptable predictive power for predicting Sjogren's disease; however, it could be associated with disease progression.

    Abstract Numerous clinical studies have shown bisphoshonates (BPs) to be useful and cost-effective options for the fractures prevention and postmenopausal bone loss. The use of oral bisphoshonates is an established option for managment of osteoporosis in postmenopausal women, but many of them complaint from gastrointestinal side effect or frequently dosed oral regimens. To improve upon the suboptimal therapeutic compliance in postmenopausal women, newer, longer-acting intravenous formulations of BPs has been approved for intermittent administration in postmenopausal women. These preparations would become an option for patients who can not tolerate oral BPs or it was ineffective in increasing their bone density.This article proposed to review effectiveness and tolerability of intravenous BPs in postmenopausal women with osteoporosis.

    Abstract Introduction: Lecture and computer-based teaching are two important methods for the teaching of medical students. However, comparison of the effectiveness and efficacy of both of these methods shows substantial deficiencies. The aim of this study was to compare the effects of lecture and computer-based teaching methods on the learning of medical students taking introduction to clinical medicine (ICM) courses at Isfahan University of Medical Sciences. Methods: This study was a quasi-experimental research which was conducted in Isfahan University of Medical Sciences. Research population included students taking ICM courses in two consecutive semesters. The first group of students were assigned to the control group (n=67) and were taught rheumatology through in-class lecture method while the second group of students (the intervention group) received computer-based teaching. For evaluation and comparison of the effects of the intervention on learning, two multiple-choice question tests were administered at the end of the training course and 8 weeks after the intervention. Results: No significant difference was found between the mean scores of the two groups in the first test (p=0.77), but the difference in the second test was significant (p<0.001). The computer-based teaching group performed better in recalling of the learning materials. Conclusion: Although short-term results were similar in both groups, long-term learning and recalling were greater with computer-based teaching than with lecture.

As the result of exocrine glands disruption, the clinical hallmark of this disease including dryness of mouth and eyes along with fatigue and joint pain occur. However, heterogeneity of clinical presentations among newly diagnosed adult patients with Sjogren's syndrome leads to difficulty in its diagnosis. Then we applied the receiver operating characteristic (ROC) curve analysis to determine the predictive value of alpha-fodrin for diagnosing Sjogren's disease. The area under curve of the ROC curve was calculated as 0.5453. Also, there were significant association between age and alpha-fodrin antibody. Conclusions: Alpha-fodrin test did not have acceptable predictive power for predicting Sjogren's disease; however, it could be associated with disease progression.

    Abstract Background: Musculoskeletal pain is one of the most common pains in patients referred to medical centers which may have a psychological basis. The aim of this study was to develop a structural model to explain the mediating role of catastrophizing pain, depression and anxiety between psychological flexibility and pain self-efficacy in patients with musculoskeletal pain.Methods: In a cross-sectional study, 200 patients with chronic musculoskeletal pain were selected by convenience sampling method from those referred to medical centers affiliated to Isfahan University of Medical Sciences in 2020-2021. Eligible participants filled out: Chronic Pain Acceptance Questionnaire (CPAQ), Cognitive Fusion Questionnaire (CFQ), Self-Experience Questionnaire (SEQ), Value-based Life Questionnaire (VLQ), Committed Action Questionnaire (CAQ-8), Pain Self-Efficacy Questionnaire (PSEQ) and the Mindful Attention Awareness Scale (MAAS), Pain Catastrophizing Scale (PCS), and Hospital Anxiety and Depression Scale (HADS). Then the data were analyzed using SPSS-24 and AMOS software and statistical methods of regression analysis and structural equations.Findings: The results showed that the direct effect coefficient of pain catastrophizing, anxiety, depression and psychological flexibility on pain self-efficacy is (0.357), (0.255), (0.127) and (0.849) respectively. The total relationship between psychological flexibility and pain self-efficacy is (0.346). To test the suitability of the models, NFI, TLI, CFI and IFI indices were used, which were within acceptable limits.Conclusion: The psychological flexibility variable predicts pain self-efficacy mediated by catastrophic pain, depression and anxiety (P < 0.01, r = 0.85) and it appears that predicting pain catastrophe, depression, anxiety and psychological resilience can increase pain self-efficacy. Ultimately, the improvement of these psychological factors may affect the process of better disease management.
