    Abstract Objectives: Chronic kidney disease of undetermined or non-traditional aetiology (CKDu or CKDnT) has been reported in Mesoamerica among farmers under heat stress. Epidemiological evidence was lacking in Asian countries with similar climatic conditions. The objective of this study was to investigate the prevalence of CKDu and possible risk factors.
    Methods: We used the data from the Changhua Community-based Integrated Screening programme from 2005 to 2014, which is the annual screening for chronic diseases in Taiwan's largest rice-farming county since 2005. Our study population included farmers and non-farmers aged 15-60 years. CKDu was defined as an estimated glomerular filtration rate <60 mL/min/1.73 m
    Results: 5555 farmers and 35 761 non-farmers were included in this study. CKDu accounted for 48.9% of all CKD cases. The prevalence of CKDu was 2.3% in the farmers and 0.9% in the non-farmers. The crude POR of CKDu in farmers compared with non-farmers was 2.73 (2.13-3.50), and the adjusted POR was 1.45 (1.10-1.90). Dehydration (blood urea nitrogen-to-creatinine ratio >20) was found in 22% of the farmers and 14% of the non-farmers.
    Conclusions: Farmers in subtropical Asian countries are at increased risk of CKDu. Governments should take the CKDu epidemics seriously and provide farmers with occupational health education programmes on thermal hazards.

    Abstract This study surveyed the behavior, attitudes, and knowledge about antibiotic usage among residents of Changhua County, Taiwan. A questionnaire designed to evaluate general knowledge and attitudes towards antibiotic usage was administered to a total of 1024 adults. The male-to-female ratio was 0.75. All of the 1024 adults were aware of the term "antibiotics". Only 39.2% of subjects had knowledge about basic antibiotic terminology, 69.7% expected that physicians should educate people about antibiotics. There were 7 main findings of this study: (1) 52.7% of subjects considered that physicians advice about the need for compliance was poor; (2) 15.3% of subjects always requested an antibiotics prescription when they suffered from flu-like symptoms; (3) 49.8% failed to comply with prescribed antibiotic regimens; (4) 53.1% of subjects were not aware that antibiotic syrup should be stored in the refrigerator; (5) 27.1% of subjects lacked knowledge about how to deal with the misuse of antibiotics; (6) 9.3% of pregnant and breast-feeding subjects did not have adequate knowledge of the safety of using antibiotics during pregnancy; and (7) 30% of subjects did not know how to obtain information about antibiotic usage. Residents of Changhua County had inadequate or incorrect knowledge about antibiotic usage. The findings of this study imply the need for programs to promote greater education about antibiotics usage in the general population of Taiwan.

    Abstract Hospice palliative care for terminal patients is necessary, yet challenges are on the way worldwide. This study demonstrated that hospice palliative care has been quickly developed in Taiwan due to the support of the National Health Insurance system, the promotion by civil societies and religious groups, patient's legal right for DNR, easier access to pain killers through medical prescription, and well-planned hospice staff training programs. This paper introduces how hospice consultation is provided by a comprehensive hospice palliative team at Changhua Christian Hospital to establish trust and cooperation with the medical team, and to improve hospice-palliative care referral and utilization rates.

    Abstract Purpose: Incidence rate of oral cancer in Changhua County is the highest among the 23 counties of Taiwan during 2001. However, in health data analysis, crude or adjusted incidence rates of a rare event (e.g., cancer) for small populations often exhibit high variances and are, thus, less reliable.
    Methods: We proposed a generalized Bayesian Maximum Entropy (GBME) analysis of spatiotemporal disease mapping under conditions of considerable data uncertainty. GBME was used to study the oral cancer population incidence in Changhua County (Taiwan). Methodologically, GBME is based on an epistematics principles framework and generates spatiotemporal estimates of oral cancer incidence rates. In a way, it accounts for the multi-sourced uncertainty of rates, including small population effects, and the composite space-time dependence of rare events in terms of an extended Poisson-based semivariogram.
    Results: The results showed that GBME analysis alleviates the noises of oral cancer data from population size effect. Comparing to the raw incidence data, the maps of GBME-estimated results can identify high risk oral cancer regions in Changhua County, where the prevalence of betel quid chewing and cigarette smoking is relatively higher than the rest of the areas.
    Conclusions: GBME method is a valuable tool for spatiotemporal disease mapping under conditions of uncertainty.

    Abstract This study investigates PM2.5-bound PAHs for rural sites (Dacheng and Fangyuan) positioned close to heavy air-polluting industries in Changhua County, central Taiwan. A total of 113 PM2.5 samples with 22 PAHs collected from 2014 to 2015 were analyzed, and Positive Matrix Factorization (PMF) and diagnostic ratios of PAHs were applied to quantify potential PAH sources. The influences of local and regional sources were also explored using the conditional probability function (CPF) and potential source contribution function (PSCF) with PMF-modeled results, respectively. Annual mean concentrations of total PAHs were 2.91 ± 1.34 and 3.04 ± 1.40 ng/m3 for Dacheng and Fangyuan, respectively, and their corresponding BaPeq were measured at 0.534 ± 0.255 and 0.563 ± 0.273 ng/m3 in concentration. Seasonal variations with higher PAHs found for the winter than for the spring and summer were observed for both sites. The lifetime excess cancer risk (ECR) from inhalation exposure to PAHs was recorded as 4.7 × 10−5 overall. Potential sources of PM2.5-bound PAHs include unburned petroleum and traffic emissions (42%), steel industry and coal combustion (31%), and petroleum and oil burning (27%), and unburned petroleum and traffic emission could contribute the highest ECR (2.4 × 10−5). The CPF results show that directional apportionment patterns were consistent with the actual locations of local PAH sources. The PSCF results indicate that mainly northeastern regions of China have contributed elevated PM2.5-bound PAHs from long-range transports.

    Abstract To investigate the prevalence of Helicobacter pylori (H. pylori) infection in adolescents and children in Changhua and to compare the differences between urban and rural areas, we used enzyme immunoassay to detect H. pylori stool antigen for feco-prevalence study. In addition, a questionnaire was designed to obtain demographic and clinical information. Children and adolescents under 18 years of age living in Changhua city and Yung-Chin village were divided into 6 groups respectively, ie. 0-3, 3-6, 6-9, 9-12, 12-15, and 15-18 years. A minimum of 30 persons were investigated in each group. A total of 567 subjects (M/F 295/272) were included in this study. There was no significant difference in H. pylori infection with regard to residential area, gender, education of the mother, occupation of parents, family history of peptic ulcer, and number of family members, except for epigastralgia of the subjects (p=0.001) and education of the father (p=0.039). The overall prevalence was 13.75%. A low prevalence was noted in the group 0-3 years (2.0%). The prevalence increased with age and reached its maximum of 23.8% in the age group of 9-12 years and then declined. The multiple logistic regression analysis revealed that only age (odds ratio (OR)=1.3, 95% confidence interval (C.I.)=1.1-1.5) and epigastralgia (OR=1.71, 95% C.I.=1.0-3.0) were the most important risk factors for H. pylori infection after adjusting for participating gender, place of residence and father's age. The difference in age pattern of H. pylori infection between our results and other studies needs further investigation.

    Abstract Foam is a medium-stable system composed of gas and liquid phases, which has the advantages of low density at the gas phase and high viscosity at the liquid phase, and has a wide application in oil and gas field development and mineral flotation, but its special medium-stable system also brings many problems in industry applications. Scientists have carried out extensive analyses and research on the foam stability and bubble-bursting mechanism, which initially clarified the rules of bubble breakage caused by environmental factors such as temperature and pressure, but the mechanism of bubble bursting under the action of internal factors such as liquid mineralization and oil concentration of the films is still not clearly defined. In this paper, we propose a compound salt-resistant foaming agent, investigated the influence of the aggregation and adsorption behavior of oil droplets on the liquid films and boundaries, and established a relevant aggregation and adsorption model with the population balance equation. We put forward a liquid film drainage mechanism based on the distribution, aggregation, and transport of oil droplets in the liquid films, so as to explain the changes in foam stability under the action of oil droplets. On the other hand, the viscoelastic analysis of foam fluid is performed with a rheometer, and the results show that in comparison with conventional power-law fluid, foam fluid has a complex rheological behavior for low shear thickening, but high shear thinning.
