Abstract Light chain amyloidosis (AL) is a complex disorder defined by the extracellular deposition of insoluble amyloid fibrils formed by intact or fragmented immunoglobulin light chains, leading to cell dysfunction, rapid organ deterioration, and, ultimately, death. Although the clinical presentation of AL is directly connected to organ involvement, signs and symptoms of AL are frequently nonspecific, misinterpreted, and late recognized. Thus, an early diagnosis combined with effective therapies to cease disease progression and rescue organ function is essential. The aim of this study was to assess the knowledge and characterize the current clinical practice regarding AL diagnosis and referral among Portuguese physicians. A Delphi-like panel (one round only) with a group of national experts from different medical specialties (cardiology, hematology, internal medicine, nephrology, and neurology) was carried out online, in which 30 statements were classified using a 4-point Likert scale. For each statement, the consensus level was set at 70% for "fully agree/disagree" and the majority level was defined as >70% in agreement or disagreement. Although the results suggest the existence of adequate general knowledge of AL amyloidosis, they also disclosed the necessity to raise awareness for this disease. Overall, this Delphi panel revealed a high lack of consensus regarding the diagnosis and early management of patients with AL among different specialties despite the qualified majority obtained in 26 statements. An optimized strategy for AL early diagnosis, transversal to several medical fields, is urgently needed. Moreover, referral centers with access to diagnostic technology and a network of diverse specialties should be established to foster an early diagnosis and better disease approach to boost the possibility of a better outcome for patients with AL.

Abstract Objective: To evaluate and compare several anatomical parameters of mandibular first premolars from individuals from different Latin American countries using micro-computed tomography.
Design: Five hundred extracted mandibular first premolars from Brazilian, Argentinian, Chilean, Colombian, and Ecuadorian individuals were scanned using micro-computed tomography (n = 100 teeth/country). Root canal configurations were classified according to established parameters. Analyses also included: canal volume and surface area, structure model index, distances from the apical foramen to the root apex or the cementoenamel junction, major/minor apical canal diameters, canal orifice shape, and prevalence of ramifications.
Results: A single root was the most common anatomy in all countries (range, 97%-100%). Vertucci's type-I canal was the most frequent configuration (range, 36%-66%), followed by C-shaped and type-V canals. The oval-shaped canal orifice was the most predominant in all countries (range, 34%-58%), followed by the circular shape (range, 16%-47%). C-shaped canals occurred in all subpopulations (range, 14%-26%), always associated with radicular grooves. Ranges for canal ramifications were as follows: accessory canals, 36%-73%; lateral canals, 4%-12%; and apical delta, 4%-14% of the teeth. Many anatomic parameters differed significantly between countries (P < .05).
Conclusions: Vertucci's types-I and -V, and C-shaped canals were the most prevalent configurations in the subpopulations investigated. Accessory canals and several complex anatomies were found, with some significantly different frequencies between countries.

Abstract Objectives: COVID-19 sanitary measures (social distancing, school closures) have deeply impacted social life, support networks, and their protective role in mental health. The present study aims to understand how attachment styles influence the way individuals experience social support. Particularly, investigating its moderating role in the relationship between social support and depression. Methods: An online survey was designed to clarify the role of adult attachment styles (ECR-S) in the perceived social support (MOSS) and self-reported depressive symptoms (BDI-SF) in the COVID-19 context. Results: Positive social interactions was the most important dimension of social support for lower depression symptoms. Individuals attachment strategies have a moderating role in the relation between of social support and depression. Crucially, insecure attachment style wanes the positive impact of social support in depression. Conclusion: Aligned with the existing literature, attachment security is an essential factor in our current understanding of relationships and mental health. Exploring specific and indivual attachment strategies might be a powerful tool to protect population’s mental healt.

Abstract According to the principles of community ecology, sympatric species may suffer a selective pressure to decrease their niche overlap through mechanisms of niche partitioning. However, there is still a gap in knowledge of the main drivers influencing niche differentiation, particularly in communities composed by small-sized and inconspicuous species. The coastal epipelagic community structure of mid-trophic level species in the Northeast Atlantic was examined using stable isotope analysis to (1) decipher intrinsic and extrinsic factors promoting niche partitioning and (2) examine species divergence in the use of resources. A total of 474 individuals from a guild of 11 species were sampled during three years, and the influence of potential drivers of isotopic divergence was assessed. The niche partitioning was mainly determined by the taxonomic group in accordance with the fundamental principles of niche theory, which was consistent for both δ¹³C (i.e. habitat) and δ¹⁵N (i.e. prey). Body size was highly important in driving niche partitioning within species. Our results show that the predominant species in the guild were the most generalist, when compared with more specialised species with which they coexist. This study reveals that the intrinsic factors are determinant to decrease inter- and intra-specific niche overlap at the community level.

Abstract Reaction to stressful events has an impact on several cognitive processes. High levels of stress can be detrimental to working memory, attention and decision-making. Here, we investigated whether individuals' reactivity to stress is related to their introspective sensitivity (i.e., how well individuals monitor their own cognitive processes). To this aim, 27 participants (16 women, mean 20 years old) were exposed to a psychosocial stress protocol (trier social stress test, TSST), where individuals were asked to simulate a job interview and perform arithmetic calculations in front of a panel of experts. The salivary cortisol concentration, which is considered a hormonal index of stress reactivity, was collected during the TSST through the enzyme immunoassay DRG cortisol ELISA kit. Based on literature recommendations, we classified participants as responders and non-responders to the TSST. In a second session, through a visual search paradigm, we evaluated the introspective sensitivity of the participants. We evaluated how these individuals (i) monitor their own performance (through a confidence estimation), (ii) monitor their own attentional shifts (through a subjective number of scanned items estimation, SNSI), and (iii) monitor their own response times (through an introspective response time estimation, iRT). We found that individuals with lower biological reactivity to stress are more accurate in estimating their SNSI (

Abstract The pandemic outbreak in March 2020 and its associated sanitary regulations and restrictions triggered an abrupt and significant change for society in general and for families' organization in particular. In Chile, the Santiago Metropolitan District was under a strict lockdown that involved the closure of the entire educational system. From a systemic-family stress perspective, the impact of these changes might have consequences not only for each individual family member, but for the parental dynamic and, consequently, for children's well-being. This paper presents the results of a follow-up study showing changes in self-reported parental depression and the perceived home organization of mothers and fathers assessed at three different moments: before the pandemic, at the initial outbreak, and after 1 month of strict lockdown. Relevant moderators were explored using linear mixed models to understand the within-subject changes in mothers' and fathers' self-reports across the different assessment times. Financial strain, personality traits of self-criticism and dependency, previous parent-child quality interaction, recent major stressful events, and number of children are highlighted as relevant factors that moderate changes in home chaos and parental mental health perception. Significant risks and protective factors are described for fathers and mothers. The use of pre-pandemic measures as baseline levels enabled the identification of personal and family characteristics that were related to better outcomes. The results help increase our understanding of the sanitary regulations' impacts on the family system and identify vulnerability indicators that should be considered.
