    Abstract Background: The potential benefit of adding recombinant human luteinizing hormone (r-hLH) to recombinant human follicle-stimulating hormone (r-hFSH) during ovarian stimulation is a subject of debate, although there is evidence that it may benefit certain subpopulations, e.g. poor responders.
    Methods: A systematic review and a meta-analysis were performed. Three databases (MEDLINE, Embase and CENTRAL) were searched (from 1990 to 2011). Prospective, parallel-, comparative-group randomized controlled trials (RCTs) in women aged 18-45 years undergoing in vitro fertilization, intracytoplasmic sperm injection or both, treated with gonadotrophin-releasing hormone analogues and r-hFSH plus r-hLH or r-hFSH alone were included. The co-primary endpoints were number of oocytes retrieved and clinical pregnancy rate. Analyses were conducted for the overall population and for prospectively identified patient subgroups, including patients with poor ovarian response (POR).
    Results: In total, 40 RCTs (6443 patients) were included in the analysis. Data on the number of oocytes retrieved were reported in 41 studies and imputed in two studies. Therefore, data were available from 43 studies (r-hFSH plus r-hLH, n=3113; r-hFSH, n=3228) in the intention-to-treat (ITT) population (all randomly allocated patients, including imputed data). Overall, no significant difference in the number of oocytes retrieved was found between the r-hFSH plus r-hLH and r-hFSH groups (weighted mean difference -0.03; 95% confidence interval [CI] -0.41 to 0.34). However, in poor responders, significantly more oocytes were retrieved with r-hFSH plus r-hLH versus r-hFSH alone (n=1077; weighted mean difference +0.75 oocytes; 95% CI 0.14-1.36). Significantly higher clinical pregnancy rates were observed with r-hFSH plus r-hLH versus r-hFSH alone in the overall population analysed in this review (risk ratio [RR] 1.09; 95% CI 1.01-1.18) and in poor responders (n=1179; RR 1.30; 95% CI 1.01-1.67; ITT population); the observed difference was more pronounced in poor responders.
    Conclusions: These data suggest that there is a relative increase in the clinical pregnancy rates of 9% in the overall population and 30% in poor responders. In conclusion, this meta-analysis suggests that the addition of r-hLH to r-hFSH may be beneficial for women with POR.

    Abstract La flora epífita vascular de los bosques templados y su relación con el hábitat no ha sido estudiada en profundidad. En este trabajo analizamos estos componentes en un bosque de la asociación Lapagerio-Aextoxiconetum, en un cordón montañoso de la Depresión Intermedia de la Región de La Araucanía, Chile. Se identificaron ocho especies de epífitas vasculares, distribuidas en cuatro géneros y tres familias: Asplenium dareoides (Aspleniaceae), Hymenoglossum cruentum, Hymenophyllum caudiculatum, H. cuneatum, H. pectinatum, H. plicatum e H. secundum (Hymenophyllaceae) y Sarmienta scandens (Gesneriaceae). Estas especies constituyen el 72.7% de la flora epifítica del bosque, de la cual un 62.5% se encuentra en la categoría vulnerable. Se determinó que la especie más importante es Hymenophyllum cuneatum, que también es la más higrófila. En el otro extremo, Hymenophyllum plicatum fue la más resistente a la sequía. Finalmente, debido a que la presencia de esta diversa comunidad de epífitas se desarrolla sobre diferentes forófitos presentes en el bosque primario de la asociación Lapagerio-Aextoxiconetum (Aextoxicon punctatum, Persea lingue, Nothofagus obliqua, Eucryphia cordifolia y Laureliopsis philippiana), se concluye que la vegetación epifítica vascular es altamente dependiente de la permanencia del bosque.

    Abstract El río Maullín es uno de los sitios prioritarios identificados por la CONAMA como de interés para la conservación in situ presentando en su curso vegetación de asociaciones endémicas que alberga y protege una amplia diversidad de organismos, algunos de los cuales, presentan problemas de conservación. A pesar de esto, esta vegetación está siendo talada y reemplazada por especies exóticas. Con la finalidad de generar un diagnóstico de la calidad de la vegetación ribereña de este río, se aplicó en 24 estaciones el índice español QBR, el cual se basa en el registro in situ de diferentes atributos y componentes del área ribereña. Los resultados indican que el 16,7 % de las estaciones están con degradación extrema y calidad pésima (QBR < 25), el 20,8 % con alteración fuerte y calidad mala (QBR= 30-50), el 29,2 % muestra inicio de alteraciones con una calidad de tipo intermedia (QBR= 55-70), el 25,0 % se encuentra con perturbaciones ligeras y buena calidad (QBR= 75-90). Por último, el 8,3 % de las estaciones mostró una calidad de tipo muy buena, con vegetación ribereña en condiciones prístinas (QBR > 90).

    Abstract Resumen: El presente artículo tiene como objetivo describir y analizar los 30 años de historia de la revista, desde sus comienzos en el año 1989, hasta este año 2019. En diciembre del año 1989, el Dr. José Antonio Del Solar, médico broncopulmonar de Clínica Las Condes, con el apoyo institucional y del cuerpo médico, lideró la edición del primer número de la revista, asumiendo el desafío de actualizar en cada número, temas de interés en salud, y transmitiendo a los sucesivos comités editoriales hasta la fecha, esta responsabilidad de contribuir a la educación continua y actualización de los temas médicos más relevantes, para todos los profesionales de la salud. Requiere mucha creatividad y energía construir y echar a andar una publicación, pero requiere mucho más esfuerzo y dedicación, que una publicación no solo persista en el tiempo en forma ininterrumpida, sino que además crezca y mantenga el interés de sus lectores. Abstract: This article aims to describe, analyze and reflect on the 30 year history of the Journal, from their beginnings in november 1989, until this year 2019. In december of 1989, Dr Jose Antonio Del Solar, a physician staff of Clinica Las Condes, with institutional and medical staff support, led the edition of this magazine, assumuing the challenge of bringing updated information in health issues, and transmitting to the succesive editorial boards to date, this responsibily to contribute to the continuing education and updating of the relevant medical topics, for all health professionals .It requires a lot of creativity and energy to create and start a publication, buy it requires much more effort and dedication that a publication not only persist in time uninterruptedely, but also to grow and maintin an incresing interes on its readers.     Abstract Oral leukoplakia is the most frequent and potentially malignant lesion of the oral cavity. Although dysplasia grading remains the main factor for risk assessment, challenges persist in determining the exact risk of transformation, and the literature has focused on studying alternative biomarkers. The interaction between dysplastic epithelial cells and the microenvironment starts early, and the communication is mainly mediated by lymphocytes, inflammatory factors, fibroblasts, and the extracellular matrix, leading to dysplastic progression. Leukoplakia-infiltrating leukocytes (LILs) and leukoplakia-associated fibroblasts (LAFs) play crucial roles in the dysplastic microenvironment. The immune response is related to intraepithelial T lymphocyte infiltration, mechanisms of immunosuppression coordinated by regulatory T cells, M2 macrophage polarization, and increased numbers of Langerhans cells; in contrast, fibroblastic and extracellular matrix factors are associated with increased numbers of pro-tumorigenic myofibroblasts, increased expression of metalloproteinases vs. decreased expression of TIMPs, and increased expression of chemokines and other inflammatory mediators. The microenvironment offers insights into the progression of leukoplakia to carcinoma, and understanding the complexity of the oral microenvironment in potentially malignant diseases aids in determining the risk of malignant transformation and proposing new therapeutic alternatives.

    Abstract Respiratory physiotherapy, including the management of invasive mechanical ventilation (MV) and noninvasive mechanical ventilation (NIV), is a key supportive intervention for critically ill patients. MV has potential for inducing ventilator-induced lung injury (VILI) as well as long-term complications related to prolonged bed rest, such as post-intensive care syndrome and intensive care unit acquired weakness. Physical and respiratory therapy, developed by the critical care team, in a timely manner, has been shown to prevent these complications. In this pathway, real-time bedside monitoring of changes in pulmonary aeration and alveolar gas distribution associated with postural positioning, respiratory physiotherapy techniques and changes in MV strategies can be crucial in guiding these procedures, providing safe therapy and prevention of potential harm to the patient. Along this path, electrical impedance tomography (EIT) has emerged as a new key non-invasive bedside strategy free of radiation, to allow visualization of lung recruitment. This review article presents the main and potential applications of EIT in relation to physiotherapy techniques in the ICU setting.

    Abstract Today, the high concentrations of copper found in water resources result in an urgent problem to solve since human health and aquatic ecosystems have been affected. Functionalized crosslinked polyketone resins (XLPK) have demonstrated high performance for the uptake of heavy metals in water solutions. In addition, its green chemical synthesis makes these resins very attractive as sorbents for metal ions contained in wastewater. XLPK are not soluble in aqueous media and do not require any catalyst, solvent, or harsh conditions to carry out the uptake process. In this paper, a series of functionalized XLPK with pending amino-derivatives namely; butylamine (BA), amino 2-propanol (A2P), 4-(aminomethyl) benzoic acid (HAMC), 6-aminohexanoic acid (PAMBA), and 1,2 diamino propane (DAP) directly attached to the pyrrole backbone of the polymers and crosslinked by di-amine derivatives was investigated using Density Functional Theory (DFT) calculations.     Abstract This paper exploits individual-level data before the implementation of a national policy to understand the factors driving avoidance of plastic consumption and explore potential inconsistencies between revealed and stated preferences for a plastic bag ban policy. We estimate a bivariate ordered probit model that allows us to account for a potential correlation between these types of preferences. The data reveals that while 71% of respondents take a reusable bag for shopping, only 58% of the sample state to strongly agree with prohibiting plastic bags. We find that gender, age, environmental concerns, environmental efforts, participation in environmental causes and pro-environmental behavior determine avoidance of plastic consumption, but no effects are found for being in favor of the prohibition. We also find inconsistencies decrease for individuals demonstrating higher pro-environmental behavior in related domains. This indicates waste management policies may benefit from local grounded knowledge from related environmental initiatives.
