    Abstract Background: Entrustable professional activities (EPAs) have been defined to promote the workplace participation of undergraduate medical students, generally in the context of high-income countries with a focus on the secondary and tertiary health care sectors. These EPAs have limited applicability to training and health care contexts in low- to middle-income countries that have a focus on primary health care, for instance, the context of community medicine. The purpose of this article is to report the process and results of defining EPAs for undergraduate medical training in a community health care setting.
    Methods: A modified Delphi study was performed to develop EPAs for the training of medical students in community medicine during their first and second years of education at the Marília Medical School (FAMEMA), Brazil. The supervision level was operationalized in terms of a student's ability to perform the EPA autonomously in an effective and safe manner with supervision readily available on request. Panellists (9 physicians and 6 nurses) rated the completeness of the proposed list of EPAs and EPA categories on four-point Likert scales. The threshold for consensus among panellists was a mean content validity index of at least 80%.
    Results: Consensus was reached after two Delphi rounds, resulting in 11 EPAs for undergraduate medical education and training in community medicine. These EPAs were organized into three overarching EPA domains: integrality of care for individual health needs in all phases of the life cycle (5 EPAs), integrality of care for family health needs (3 EPAs), and integrality of care for community health needs (3 EPAs). For each EPA, descriptions of the following categories were created: title; specifications and limitations; conditions and implications of the entrustment decision; knowledge, skills, and attitudes; links to competencies; and assessment sources.
    Conclusion: The resulting 11 EPAs for training medical students in community medicine expand the application of the EPA framework to both early undergraduate medical education and the context of primary health care. This report can support and guide other medical schools in their attempts to train students in primary health care contexts and to incorporate EPAs into their curricula.

    Abstract As competências médicas apresentadas pelas Diretrizes Curriculares Nacionais (DCN) e as políticas de incentivo à formação de mais médicos popularizaram temas como aprendizado ativo e competência. Distorções na implementação curricular resultaram em abordagens fragmentadas e reprodutivas das práticas com distanciamento do processo de trabalho. Vários países ocidentais, ao buscarem o aprimoramento da Educação Médica Baseada em Competência, adotaram as Atividades Profissionais Confiáveis (APCs) na tradução bem-sucedida desse referencial para a prática clínica. Este estudo perspectivo apresenta as APCs no âmbito da educação médica brasileira como possível solução para a melhor efetivação do Currículo Médico Baseado em Competência (CMBC). O estudo descreve as APCs, de seu conceito à realização; provê subsídios para seu entendimento e análise de sua capacidade em mediar uma formação médica mais qualificada e responsiva às necessidades de saúde locais; e contribui com a literatura brasileira na área.

    Abstract Resumo Este estudo caracteriza a dimensão social nos contextos representados nas situações-problema em um curso médico estadual paulista que utiliza a Aprendizagem Baseada em Problema. Foram analisadas 69 situações-problema, disparadoras do processo de aprendizagem da primeira à quarta série e seus respectivos guias de tutor. Para a leitura do material, utilizou-se o Roteiro de Análise de Dimensão Social, elaborado pelas pesquisadoras com base em documentos e literatura relacionados. Para análise, adotou-se o método de análise textual discursiva. A comparação entre os objetivos de aprendizagem apresentados nas situações-problema e nos guias evidenciou discordâncias e dicotomias. A caracterização dos contextos explorados não retratou o território real: equipamentos sociais foram pouco abordados, já as áreas de competência do cuidado individual, coletivo e gestão deram ênfase satisfatória às políticas públicas. As análises de áreas de riscos, diversidades e grupos vulneráveis foram superficiais e a intersetorialidade foi pouco acionada. Espera-se, com estes resultados, suscitar reflexões acerca da construção de situações-problema na aprendizagem baseada em problemas, pois o aprofundamento na descrição, como a ampliação de sua abordagem, poderá favorecer a articulação teórico-prática, o aprendizado ampliado e a formação médica voltada ao cuidado integral.

    Abstract Considerada a parceria de mais de 20 anos da Legião Mirim de Marília e a Faculdade de Medicina de Marília no Programa Legionário-bolsista era desejável e de grande relevância a realização de um estudo para sua documentação e fortalecimento. O mesmo teve como objetivo avaliar os sentidos, as fortalezas e as fragilidades do programa por meio de questionários aplicados aos adolescentes que participaram das atividades e foram egressos em 2008, 2009 e 2010. As repercussões dessa experiência em suas vidas, no âmbito pessoal e profissional foram discutidas por meio de pesquisa qualitativa. Ficou, desta sorte, evidenciada a importância desta atividade não apenas devido à pré-profissionalização, mas, sobretudo, pela capacidade de socializar e construir referenciais humanísticos para estes jovens.

    Abstract Resumen: El concepto de actividades profesionales a confiar surgió como un intento de superar las críticas al enfoque de competencias, y ha tenido gran impacto tanto en la práctica como en la investigación de la educación en profesiones de la salud. A nivel internacional, se ha difundido por sus siglas en inglés: EPA (Entrustable Professional Activities). Esta perspectiva propone orientar la evaluación y la enseñanza hacia las actividades específicas de la profesión que permitan integrar varias competencias y cuya responsabilidad se pueda confiar, de forma gradual y explícita, a quien se forma. Supone la definición de niveles de supervisión que otorgan autonomía creciente por cada EPA, en la medida que, estudiantes o residentes, muestran la competencia requerida. La práctica, la supervisión y la retroalimentación en escenarios clínicos reales son claves para el desarrollo de la autonomía en el desempeño de las EPA. En Latinoamérica, la difusión del enfoque de EPA es aún limitada. No obstante, puede constituir un aporte significativo para la revisión de los currículos y las prácticas de evaluación de las carreras y revisar las bases sobre las que se toman las decisiones de habilitación para el ejercicio profesional en el cuidado de la salud a nivel de grado o de especialidades en el posgrado. Abstract: The concept of entrustable professional activities emerged as an attempt to overcome some of the criticisms to the competency-based medical education approach; it has had a broad impact in practice and health professions education research. It has been disseminated internationally with its English acronym: EPA. This approach proposes to orient assessment and teaching to specific activities in the profession, which allows the integration of several competencies, and to determine which responsibilities can be entrusted to the trainee, in a gradual and explicit manner.     Abstract The events of ischemia-reperfusion injury triggers a systemic inflammatory response and can lead to cellular injury and organ failure. Such effects are noted in the postoperative recovery, especially with the use of cardiopulmonary bypass. Nowadays, it is known that leukocytes play an important role in this process. Therefore, this study addresses the role of leukocytes in the physiopathology of ischemia-reperfusion injuries and activation of inflammatory cascades through this process and seek to help in the understanding of these mechanisms as well as to bring contributions on the therapeutic approaches that can mitigate them. This retrospective review was performed from indexed scientific papers published over the last ten years in Portuguese and English in international databases MEDLINE and SciELO and related classic texts. The descriptors investigated were: ischemia-reperfusion, leukocytes, inflammatory response, cardiopulmonary bypass, adverse effects and apoptosis.

    Abstract Os eventos de isquemia-reperfusão desencadeiam uma resposta inflamatória sistêmica que pode levar a lesões celulares e até falência de órgãos. Tais repercussões são notadas no pós-operatório de cirurgias, em especial, com o uso de circulação extracorpórea. Sabe-se, atualmente, que os leucócitos exercem importante papel neste processo. Assim, este estudo aborda o papel dos leucócitos na fisiopatologia das lesões de isquemia-reperfusão e a ativação das cascatas inflamatórias por esse processo e procura auxiliar na compreensão destes mecanismos assim como trazer contribuições acerca das abordagens terapêuticas que possam atenuá-los. Esta revisão bibliográfica retrospectiva foi realizada a partir de documentos científicos publicados nos últimos dez anos, em português e inglês, indexados em bases de dados internacionais Medline e SciELO e de textos clássicos relacionados. Os descritores pesquisados foram: isquemia-reperfusão, leucócitos, resposta inflamatória, circulação extracorpórea, efeitos adversos e apoptose.     Abstract Insulin improves contractile function after ischemia, but does not increase glucose uptake in the isolated working rat heart. We tested the hypothesis that the positive inotropic effect of insulin is independent of the signaling pathway responsible for insulin-stimulated glucose uptake. We inhibited this pathway at the level of phosphatidyl inositol 3-kinase (PI3K) with wortmannin. Hearts were perfused for 70 min at physiological workload with Krebs-Henseleit buffer containing [2-(3)H] glucose (5 mM, 0.05 microCi/ml) and oleate (0.4 mM, 1% BSA) in the presence (WM, n = 5) or absence (control, n = 7) of wortmannin (WM, 3 micromol/L). After 20 min, hearts were subjected to 15 min of total global ischemia followed by 35 min of reperfusion. Insulin (1 mU/ml) was added at the beginning of reperfusion (WM + insulin n = 8, insulin n = 8). Cardiac power before ischemia was 8.1 +/- 0.7 mW. Recovery of contractile function after ischemia was significantly increased in the presence of insulin (73.5 +/- 8.9% vs. 38.5 +/- 6.7%, p < 0.01). The addition of wortmannin completely abolished the effect of insulin on recovery (32.6 +/- 6.4%). Glucose uptake was 1.84 +/- 0.32 micromol/min/g dry before ischemia and was slightly elevated during reperfusion (2.68 +/- 0.35 micromol/min/g dry, n.s.). Insulin did not affect postischemic glucose uptake. In the presence of wortmannin, glucose uptake was lowest during reperfusion (n.s.). The results suggest that PI3K is involved in the insulin-induced improvement in postischemic recovery of contractile function. This effect of insulin is independent of its effect on glucose uptake.

    Abstract A análise dos formatos de avaliação do Programa de Desenvolvimento Docente (PDD) da faculdade de Medicina de Marilia, SP, mostra as seguintes fragilidades: baixa motivação, atitude passiva dos participantes e pouco interesse em capacitação para o processo tutorial. Considerando a necessidade de inovação das estratégias de capacitação docente, optou-se, nas oficinas, pelo uso da técnica de role-playing, a qual possibilita a troca de papéis e o aprender na ação. Analisou-se qualitativamente o discurso docente frente ao uso do role-playing nas oficinas desenvolvidas, segundo as narrativas dos formatos de avaliação aplicados. Os núcleos de sentido das respostas foram classificados em unidades temáticas. Os resultados demonstraram a relevância das atividades e que o role-playing foi um importante instrumento facilitador do processo de aprendizagem, à medida que colocou o tutor como protagonista, no papel de estudante, favorecendo novos olhares e percepções e sensibilizando-o para o processo tutorial.
