    Abstract Background: India is among the most severely affected countries in the ongoing Covid-19 pandemic, second to the USA. Until July 2022, there were 43 million confirmed Covid-19 cases, with 0.53 million Covid-19 deaths in India.     Materials and methods: All Covid-19 deaths that occurred between September 2021 to February 2022 were line-listed, and family members of all who died in the hospital were interviewed.
    Results: 356 Covid-19 deaths were included in the present study. The top three districts which observed maximum deaths (63.48%) were Raipur (27.53%), Durg (15.45%), and Bilaspur (7.87%). Around 50% of the deceased were of age 65 years or above. The male deaths were twice as compared to the female deaths. The RT-PCR was the most used testing method, followed by RAT. Most of the admissions (58%) were in a government facility. Among the deceased, 60% had one or two associated comorbidities. Hypertension and Diabetes were the most common comorbidity identified. Almost 90% of patients were symptomatic. Breathlessness was the most common symptom. Half of the deceased were not vaccinated. The median expenditure on Covid-19 management was ₹ 50,000 (€ 565.13).
    Conclusion: The deaths in the study were urban-centric. Higher Covid-19 mortality was observed in unvaccinated older men (≥65 years) with one or more comorbidity.

    Abstract Nearly 57 million people depend on Yamuna's water for their daily needs and agriculture. This is the first study of assessment of the Yamuna River for five major pollutants - Nitrate, Sulfate, Phosphate, Silicon, and Chloride, and the role of inhabitant algal species for phycoremediation. Water samples were collected from 11 different locations across three different seasons and it was found that the concentration of these pollutants varies in different locations and seasons.     Abstract Background The COVID-19 pandemic, caused by the novel coronavirus, has had a profound impact on global health, significantly affecting demographics worldwide. As India's twelfth most affected state, Chhattisgarh has experienced substantial COVID-19-related fatalities. This study aims to analyze the temporal, geographical, and demographic distribution and trends in COVID-19 mortality reported by the Department of Health and Family Welfare, Chhattisgarh, spanning from the onset of the pandemic until March 2022 (two years). Methods Data about all COVID-19 deaths recorded between March 2020 and March 2022 were collected from the State Surveillance Unit, Department of Health and Family Welfare, Chhattisgarh, and subsequently compiled in a Microsoft Excel sheet (Microsoft, Redmond, Washinton) for analysis. Results A comprehensive dataset of 14,038 deaths was examined during the study period. Of these, 24.5% (3446), 72.2% (10141), and 3.3% (451) occurred in 2020, 2021, and 2022, respectively. The top five districts in Chhattisgarh with the highest COVID-19 mortality rates were identified as follows: 1) Raipur (23.5%), 2) Durg (13.4%), 3) Bilaspur (8.9%), 4) Raigarh (7.05%), and 5) Janjgir Champa (6.25%). The mean age of the deceased individuals was determined to be 55.44 years, with a standard deviation of 15.14 years. Furthermore, the impact of the pandemic was found to affect males compared to females in Chhattisgarh disproportionately. Conclusion Over the two-year study period, three distinct waves of COVID-19 were observed, with the second wave being the most devastating, particularly for the elderly population. Understanding the demographic characteristics and trends in COVID-19 mortality is crucial for implementing targeted public health measures and interventions to mitigate the impact of future infectious disease outbreaks.

    Abstract Introduction: Swallowing dysfunction is common after acute stroke. It increases the risk of aspiration pneumonia and affects nutrition. In this study, we aimed to determine the incidence of dysphagia after a single episode of acute stroke in conscious patients and the factors predisposing the patient to dysphagia. We also assessed the course of dysphagia over a period of 8 weeks after stroke.
    Materials and methods: It was a prospective observational study. We included patients of acute stroke (ischemic, hemorrhagic, lacunar, anterior, as well as posterior circulation) with Glasgow Coma Scale (GCS) of ≥12; within 48 hours of onset. Patients were screened for dysphagia by the Gugging Swallowing Screen (GUSS) screening tool; then assessed in detail using by Mann Assessment of Swallowing Ability (MASA) scoring scale. Patients with dysphagia were reassessed at 7 days and at 8 weeks after stroke for the presence and severity of dysphagia.
    Results: We included 150 patients. The incidence of dysphagia at day 1, day 7, and 8 weeks was 42, 24, and 9%, respectively. The proportion of patients with moderate and severe dysphagia also decreased during a follow-up period of 8 weeks from 18 to 3% and from 20 to 6%, respectively. The incidence of dysphagia was significantly greater in moderately severe stroke [National Institutes of Health Stroke Scale (NIHSS 5-14)] than in mild stroke (NIHSS 1-4). It was also more common in total anterior circulation infarct (TACI) than partial anterior circulation or lacunar infarct (LacI) and in posterior circulation strokes than the strokes involving anterior circulation. Patients with dysphagia had longer hospital stays (7.29 ± 3.4 days vs 3.62 ± 1.5 days, p = 0.001) and higher mean modified Rankin score at discharge (3.45 vs 2.17, p = 0.001).
    Conclusion: Swallowing dysfunction should be checked in all cases of strokes, including unilateral hemispheric strokes and in fully conscious patients. Swallowing improves with time, but the patient may require feeding assistance in an acute setting. Dysphagia is more common in strokes with higher NIHSS, involving more brain parenchyma and posterior circulation strokes.

    Abstract Fabry disease (FD) is a rare, lysosomal storage disorder characterized by multiorgan accumulation of predominantly globotriaosylceramide (GL3) and its metabolite. Resulting renal, cardiac, and cerebrovascular complications are crucial causes of morbidity and mortality in FD. Enzyme replacement therapy (ERT) shows promising outcomes for these patients, provided that therapy is initiated early. Thus, precise and early diagnosis of the disease is a pivotal factor determining the corollary of the disease. We report two cases of young adult males who presented to the nephrology department with proteinuria. A kidney biopsy was performed in both cases, which was suggestive of FD. The final conclusive diagnosis of FD was provided by electron microscopy.

    Abstract Lung cancer is the leading cause of mortality worldwide of which non-small cell lung carcinoma constitutes majority of the cases. High mortality is attributed to early metastasis, late diagnosis, ineffective treatment and tumor relapse. Chemotherapy and radiotherapy form the mainstay of its treatment. However, their associated side effects involving kidneys, nervous system, gastrointestinal tract, and liver further adds to dismal outcome. These disadvantages of conventional treatment can be circumvented by use of engineered nanoparticles for improved effectiveness with minimal side effects. In this study we have synthesized silver gold nanocomposite (Ag-Au NC) using polyethylene glycol and l-ascorbic acid as surfactant and reducing agent respectively. Synthesized nanocomposite was characterized by ultraviolet-visible absorption, dynamic light scattering, scanning and transmission electron microscopy. Compositional analysis was carried out by energy dispersive X-ray analysis and average pore diameter was estimated using Barrett-Joyner-Halenda method. In-silico molecular docking analysis of the synthesized NC against active regions of epidermal growth factor receptor revealed good binding energy. Subsequently, we investigated the effect of NC on growth and stem cell attributes of A549 lung cancer cells. Results showed that NC was effective in inhibiting A549 cell proliferation, induced DNA damage, G2/M phase arrest and apoptosis. Further, tumor cell migration and spheroid formation were also negatively affected. NC also enhanced reactive oxygen species generation and mitochondrial depolarization. In addition, the effect of NC on putative cancer stem cells in A549 cells was evaluated. We found that Ag-Au NC at IC50 targeted CD44, CD24, CD166, CD133 and CD326 positive cancer stem cells and induced apoptosis. CD166 positive cells were relatively resistance to apoptosis. Together our results demonstrate the anticancer efficacy of Ag-Au NC mediated by a mechanism involving cell cycle arrest and mitochondrial derangement.

    Abstract Background: Diffuse crescentic IgAN (CIgAN) is an uncommon phenotype of IgAN, which presents as rapidly progressive renal failure, similar to patients with pauci-immune crescentic glomerulonephritis(PCGN). There are limited data on outcomes comparisons between the two.
    Methods: In this single-center, retrospective cohort study, we compared the clinical features, pathological presentation, and renal outcomes of 52 patients with CIgAN and 42 patients with renal-limited PCGN from January 2007 to December 2019.
    Results: The CIgAN patients were younger (30.5 ± 13.8 years) than PCGN patients (46.1 ± 11.8 years) (P = 0.001). The CIgAN patients had a higher prevalence of hypertension (86.5% Vs. 41.3%, P = 0.001); and degree of proteinuria (4.2 ± 2.7 g/24 h Vs. 2.3 ± 1.16 g/24 h; P = 0.001) than PCGN patients. The chronicity in terms of global glomerulosclerosis, interstitial fibrosis, and tubular atrophy was higher in the CIgAN group than in the PCGN group. The remission rate with immunosuppression was significantly higher in the PCGN group than in the CIgAN group (P = 0.016). The end-stage renal disease (ESRD) or death within 1 year of diagnosis was significantly more in the CIgAN group (62.3% Vs. 39.1%) than PCGNgroup. For patients who were dialysis-dependent at presentation, the primary outcome of ESRD or death within one year was seen in 90.9% of patients of CIgAN and 44.1% in the PCGN group (P = 0.001). The long-term death non-censored renal survival is poor in the CIgAN group than in PCGN patients. However, patient survival is poor in PCGN patients.
    Conclusion: CIgAN is a different form of RPGN compared to PCGN and carries a poor prognosis despite similar immunosuppressive therapy in the long term.
