    Abstract A highly diastereoselective organocatalyzed domino vinylogous sulfa-Michael-aldol-cyclocondensation (VMAC) reaction has been developed using alkylidene Meldrum's acid as dienes highlighting two vinylogous steps, an unprecedented sulfa-1,6-conjugate addition and a diastereoselective aldol reaction triggering a formal (4+2) cycloaddition. This work opens a new route towards bio-relevant and original tricyclic thiochroman derivatives.

    Abstract Operative vaginal delivery significantly increases the risk of anal sphincter injury compared with spontaneous vaginal delivery, particularly when an instrumental application at high station, persistent occipital posterior presentation and/or macrosomia are associated. Risks of vaginal tears, postpartum haemorrhage and urinary retention appear to be increased compared with spontaneous vaginal delivery. There is no evidence that one type of forceps is less safe than another. Different vacuum extractors (soft, rigid and single-use instruments) have similar maternal outcomes. A risk of anal incontinence in the first year postpartum is associated with operative vaginal delivery, particularly forceps. Operative vaginal delivery also appears to increase the risk of occult anal sphincter injury regardless of the instrument used, but does not appear to be associated with the risk of postpartum urinary incontinence and genital prolapse compared with spontaneous vaginal delivery. It does, however, increase the risks of perineal pain, postpartum dyspareunia and sexual disorders compared with spontaneous vaginal delivery, particularly in cases of perineal tears.

    Abstract Objective: To determine the diagnosis criteria and management of intra-uterine inflammation or infection (Triple I, III).
    Methods: PubMed and Cochrane Central databases search.
    Results: III is defined as an infection of the fetal membranes, and/or other components like the decidua, fetus, amniotic fluid or placenta. This word should be preferred to the word chorioamnionitis that is less precise (Professional consensus). III clinical signs exhibit poor limited sensibility and specificity (EL3). The diagnosis of III is retained in case of maternal fever (defined by a body temperature≥38°C) with no alternative cause identified and at least 2 signs among the following: fetal tachycardia>160 bpm for 10min or longer, uterine pain of labor, purulent fluid from the cervical canal (Professional consensus). Maternal hyperleukocytosis>20 giga/L in the absence of corticosteroids treatment or increased plasmatic C-reactive protein also argue for III, despite their limited sensibility and specificity (EL3). III requires prompt delivery (Grade A). III is not by itself an indication for cesarean delivery (Professional consensus). Antibiotic treatment should cover Streptococcus agalactiae and Escherichia coli. Antibiotics should be started immediately and maintained all over delivery, to reduce neonatal and maternal morbidity (Grade B). Treatment should rely on a combination of betalactamin and aminoglycoside (Grade B). After vaginal delivery, one single dose of antibiotic is required. Antibiotic duration should be longer in case of bacteremia. Longer duration could be considered in case of persistent fever or of cesarean delivery (Professional consensus).

    Abstract Pristine peatlands being excellent storage for terrestrial Carbon (C) play a crucial role in regulating climate and water and provide several important ecosystem services. However, peatlands have been heavily altered (e.g., by draining the water table), increasing greenhouse gas (GHG) emissions. Restoring peatlands requires a comprehensive characterization, including knowledge of peat depth (PD; m). Traditionally, this requires the physical insertion of a push probe, which is time-consuming and labor-intensive. It has been shown that non-invasive proximal sensing techniques such as electromagnetic induction and ground penetrating radar can add value to PD data. In this research, we want to assess the potential of proximally sensed gamma-ray (γ-ray) spectrometry (i.e., potassium [K], thorium [Th], uranium [U], and the count rate [CR]) and terrain attributes data (i.e., elevation, slope, SAGAWI, and MRVBF) to map PD either alone or in combination across a small (10 ha) peatland area in ØBakker, Denmark. Here, the PD varies from 0.1 m in the south to 7.3 m in the north. We use various prediction models including ordinary kriging (OK) of PD, linear regression (LR), multiple LR (MLR), LR kriging (LRK), MLR kriging (MLRK) and empirical Bayesian kriging regression (EBKR). We also determine the minimum calibration sample size required by decreasing sample size in decrements (i.e., n = 100, 90, 80,…, 30). We compare these approaches using prediction agreement (Lin’s concordance correlation coefficient; LCCC) and accuracy (root mean square error; RMSE). The results show that OK using maximum calibration size (n = 108) had near perfect agreement (0.97) and accuracy (0.59 m), compared to LR (ln CR; 0.65 and 0.78 m, respectively) and MLR (ln K, Th, CR and elevation; 0.85 and 0.63 m). Improvements are achieved by adding residuals; LRK (0.95 and 0.71 m) and MLRK (0.96 and 0.51 m).     Abstract Anaemia is frequently diagnosed during pregnancy. However, there are few data regarding its incidence, and the association with severe maternal morbidity remains uncertain and potentially biased in high-resource countries. The purpose of this study was to explore the association between gestational anaemia and severe acute maternal morbidity during and after delivery. We performed a cohort-nested case-control analysis from the epidemiology of severe maternal mortality (EPIMOMS) prospective study conducted in six French regions (2012-2013, n = 182,309 deliveries). There were 1669 women with severe acute maternal morbidity during or after delivery, according to a standardised definition obtained by expert consensus. The control group were randomly selected among women without severe morbidity who delivered in the same health centres (n = 3234). We studied the association between gestational anaemia and severe acute maternal morbidity during or after delivery overall, by cause, and by mode of delivery, using multivariable logistic regression and multiple imputation. Gestational anaemia was significantly more frequent in women with severe acute maternal morbidity (25.3%) than in controls (16.3%), p < 0.001, and mostly mild in both groups. After adjustment for confounders, women with gestational anaemia were at increased risk of overall severe acute maternal morbidity during and after delivery (adjusted OR (95%CI) 1.8 (1.5-2.1)). This association was also found for severe postpartum haemorrhage (adjusted OR (95%CI) 1.7 (1.5-2.0)), even after omitting the transfusion criterion (adjusted OR (95%CI) 1.9 (1.6-2.3)), and for severe acute maternal morbidity secondary to causes other than haemorrhage or pregnancy-related hypertensive disorders (adjusted OR (95%CI) 2.7 (1.9-4.0)). These results highlight the importance of optimising the diagnosis and management of anaemia during pregnancy.

    Abstract VEXAS (vacuoles, E1 enzyme, X-linked, auto-inflammatory, somatic) syndrome is an inflammatory disorder with hematological and systemic features. A recent study demonstrated that the dermal infiltrate in neutrophilic dermatosis from VEXAS patients is derived from the pathological UBA1-mutated myeloid clone. Neutrophilic dermatosis is, however, only one of the various skin involvements observed in VEXAS syndrome. We analyzed 10 formalin-fixed paraffin-embedded skin biopsies from genetically confirmed VEXAS syndrome. UBA1 mutation was found in the biopsies related to neutrophilic dermatitis but in none of the other histological patterns (leukocytoclastic vasculitis and septal panniculitis). This could lead to a distinction between clonal and paraclonal cutaneous involvements in VEXAS syndrome, which could in turn improve therapeutic outcomes.

    Abstract The morphological reconstruction based on geodesic operators, is a powerful tool in mathematical morphology. The general definition of this reconstruction supposes the use of a marker function f which is not necessarily related to the function g to be built. However, this paper deals with operations where the marker function is defined from given characteristic regions of the initial function f, as it is the case, for instance, for the extrema (maxima or minima) but also for the saddle zones. Firstly, we show that the intuitive definition of a saddle zone is not easy to handle, especially when digitised images are involved. However, some of these saddle zones (regional ones also called overflow zones) can be defined, this definition providing a simple algorithm to extract them. The second part of the paper is devoted to the use of these overflow zones as markers in image reconstruction. This reconstruction provides a new function which exhibits a new hierarchy of extrema. This hierarchy is equivalent to the hierarchy produced by the so-called waterfall algorithm. We explain why the waterfall algorithm can be achieved by performing a watershed transform of the function reconstructed by its initial watershed lines. Finally, some examples of use of this hierarchical segmentation are described.
