    Abstract Adaptive Systems (ASs) are capable to monitor their behavior and make adjustments when quality goals are not achieved through the MAPE-K, a widely recognized reference model that offers abstractions for designing ASs. By making these abstractions evident in the system structure, numerous benefits emerge, particularly in terms of enhancing the architecture's maintenance and comprehensibility. However, it is observed that many existing ASs are not designed in accordance with MAPE-K, causing these abstractions to remain hidden in their architecture. To address this issue, Architectural Conformance Checking (ACC) emerges as a valuable technique for verifying whether the current architecture (CA) of a system adheres to the rules prescribed by the planned architecture (PA) or a reference model, such as MAPE-K. In this paper, we present REMEDY, a domain-specific approach that encompasses the specification of the planned adaptive architecture based on the MAPE-K reference model, the recovery of the current adaptive architecture, the conformance checking process, and architecture visualizations. Furthermore, our approach is specifically tailored for ASs, incorporating well-known rules from the MAPE-K model. The evaluation of the REMEDY DSL involves a comparison with a general-purpose DSL, and the results demonstrate improvements in productivity. REMEDY facilitates the identification and correction of architectural non-conformance issues, thereby enhancing the overall quality of adaptive systems.

    Abstract Las cartas aquí reunidas se caracterizan por su unidad temática - discutir los méritos o insufribles vicios de la obra poética de Luis de Góngora - sino que forman un conjunto autónomo dentro de la dilatada querelle en torno a la obra de Góngora. Francisco Cascales lanzó la polémica en una carta humorística, con fórmulas memorables como la que hace del poeta un ángel de luz convertido en ángel de tinieblas. Esto desde Murcia, muy lejos de Madrid, donde Góngora transcurría entonces sus últimos y amargos años, aureolado de una gloria y de escándalo cuyos ecos se oían en toda la península.Quienes participan en esta controversia son son personajes de segunda fila de la República de las letras españolas, asentados en el Sur de España: respectivamente en Murcia para Cascales, donde ejerció como catedrático de Retórica; en Andújar (provincia de Jaén) para Villar, donde fue vicario y juez eclesiástico; y en Loja (provincia de Granada) para Angulo y Pulgar, un hombre acomodado, de linaje noble.     Abstract Objectives: This study addresses the critical need for accurate summarization in radiology by comparing various Large Language Model (LLM)-based approaches for automatic summary generation. With the increasing volume of patient information, accurately and concisely conveying radiological findings becomes crucial for effective clinical decision-making. Minor inaccuracies in summaries can lead to significant consequences, highlighting the need for reliable automated summarization tools.
    Methods: We employed two language models - Text-to-Text Transfer Transformer (T5) and Bidirectional and Auto-Regressive Transformers (BART) - in both fine-tuned and zero-shot learning scenarios and compared them with a Recurrent Neural Network (RNN). Additionally, we conducted a comparative analysis of 100 MRI report summaries, using expert human judgment and criteria such as coherence, relevance, fluency, and consistency, to evaluate the models against the original radiologist summaries. To facilitate this, we compiled a dataset of 15,508 retrospective knee Magnetic Resonance Imaging (MRI) reports from our Radiology Information System (RIS), focusing on the findings section to predict the radiologist's summary.
    Results: The fine-tuned models outperform the neural network and show superior performance in the zero-shot variant. Specifically, the T5 model achieved a Rouge-L score of 0.638. Based on the radiologist readers' study, the summaries produced by this model were found to be very similar to those produced by a radiologist, with about 70% similarity in fluency and consistency between the T5-generated summaries and the original ones.
    Conclusions: Technological advances, especially in NLP and LLM, hold great promise for improving and streamlining the summarization of radiological findings, thus providing valuable assistance to radiologists in their work.

    Abstract In the surgical treatment of esophageal cancer, robotic surgery allows performing an intrathoracic handsewn anastomosis in a simpler, faster and more comfortable way for the surgeon than open surgery and traditional minimally invasive surgery. With this, we avoid the use of self-suture instruments, some of which require a small thoracotomy for their introduction. However, the retrieval of the specimen requires the practice of this thoracotomy, of variable size, that can be associated with intense chest pain. We describe a technical modification of the classic robotic Ivor Lewis that allows removal of the surgical piece through a minimal abdominal incision, thus avoiding controlled rib fracture, as well as the possible sequelae of making an incision in the chest wall.

    Abstract Background: Syphilis is a sexually transmitted disease caused by the spirochete Treponema pallidum, whose incidence has increased significantly in recent years. Some patients may develop sudden hearing loss (SHL) against the background of otosyphilis.
    Objectives: The objective of our study was to determine whether routine lues serology is useful in patients presenting with sudden hearing loss.
    Methods: For this purpose, all cases of SHL treated in our hospital during a period of 6 years were propectively collected. The frequency of positivity for syphilis in these patients, the treatment received, and their evolution were determined.
    Results: Of the total number of patients evaluated during that period, 71 underwent serological screening for syphilis, of whom 2 (2.8 %) presented positive screening antibodies. In one of them, the RPR was normal and had been treated with lues a few years before. After treatment there was no improvement. The other patient, diagnosed with otosyphilis with unconfirmed suspected neurological disease, showed normalization of hearing after specific treatment.
    Conclusions: Since it is a potentially curable disease, despite the low overall frequency of syphilis in patients with SHL it is advisable to perform serological screening for syphilis in high risk patients (e.g., incarceration, multiple recent sexual partners, men who have sex with men) or atypical clinical presentation (e.g., concurrent neuropathies).

    Abstract Objective: This study aims to determine the prevalence of vestibular schwannoma (VS) among patients presenting with sudden hearing loss (SHL) and to characterize the clinical features of individuals diagnosed with both VS and SHL.
    Methods: We conducted an observational retrospective review at a tertiary referral center, spanning a 30-year period, focusing on patients diagnosed with SHL where VS was confirmed as the underlying cause. We included patients meeting these criteria while excluding those lacking imaging or with a pre-existing diagnosis of VS. We evaluated the audiological characteristics at the time of diagnosis and assessed clinical outcomes following treatment.
    Results: Among the 403 patients presenting with SHL during the study period, 9 (2.2%) were diagnosed with VS, aged between 25 and 72 years. Although audiometric profiles varied, high-frequency hearing loss predominated, mostly categorized as mild to moderate. Six patients (66%) had Koos grade I-II schwannomas. Only 2 patients achieved complete hearing recovery post-treatment, while 4 showed no improvement.
    Conclusions: VS is a rare etiology of SHL, accounting for slightly over 2% of cases. Its symptomatology, severity, and audiometric patterns do not significantly differ from SHL caused by other factors. Tumor size does not correlate with hearing characteristics. Treatment modalities resemble those for other SHL cases, and hearing improvement does not obviate the necessity for follow-up magnetic resonance imaging (MRI) scans.

    Abstract This study examined the incidence of loneliness among Chilean indigenous older adult women (106 Aymara and 180 Mapuche) and how family, community and socio-cultural integration are associated with lower levels of loneliness. A cross-sectional study involving 800 older adults living in a rural context in Chile, of whom 35.8% were indigenous women. The De Jong Gierveld Loneliness Scale (DJGLS-6) were used to assess loneliness and a questionnaire about the maintenance of certain indigenous cultural practices was created. The descriptive findings indicate more loneliness among Mapuche women. Moreover, hierarchical regression models confirmed that women who did not live alone, who participated in social groups and who maintained cultural practices reported lower levels of loneliness, with notable transmission of indigenous knowledge to their children. While, taking part in the indigenous New Year, leading or organizing a ceremony and receiving attention with a health cultural agent were associated with more loneliness. These seemingly contradictory findings are discussed and may be explained by religious changes in indigenous communities; however, this study would confirm that social integration in different dimensions is a protective factor against loneliness.
