    Abstract El doctor David Matto Usandivaras realizó importantes contribuciones a la medicina peruana, en particular en las especialidades de Patología Clínica y Psiquiatría, hacia finales del siglo XIX e inicios del siglo XX, que coincidió con la corriente cientificista que comenzó a prevalecer paulatinamente en los años posteriores a la Guerra del Pacífico. No obstante, el trascurrir del tiempo difuminó tales aportes, por lo que el objetivo de este artículo es rescatarlos y situarlos en el lugar que merecen para memoria de todos los miembros de la comunidad médica nacional y en especial de los facultativos de ambas especialidades.

    Abstract Background: The best approach for aortic root disease remains controversial. Composite valve-graft conduit (CVG) replacement offers good results at short-term and long-term follow-up; on the other hand, valve-sparing aortic root replacement (VSARR) has proven to be an excellent treatment alternative. This study aimed to analyse the outcomes after VSARR and compare whether preoperative moderate or severe aortic regurgitation (AR) and or the need for aortic valve repair (AVR) during this procedure influenced survival and freedom from reoperation rates.
    Methods: From September 2005 to June 2018, 104 patients underwent VSARR using the reimplantation technique: 64% presented with preoperative moderate or severe AR, concomitant AVR was performed in 43.3%, Marfan syndrome was present in 16.3%, and 12.5% had a bicuspid aortic valve. Complete follow-up was obtained in 91% of the sample, echocardiographic results were available for 86% and the mean follow-up time was 1,893 days.
    Results: In-hospital mortality was 2.9% and one death occurred 42 days after hospital discharge. In the latest echocardiographic assessment, 88.3% presented with mild AR or better. Freedom from reoperation at 8 years was 95.4%. There was no case of endocarditis and one patient had a stroke 2 years after the operation. There were no between-group differences in morbidity, mortality and complications during the follow-up.
    Conclusion: VSARR can be performed with low mortality rates and reasonable durability of the aortic valve. Neither moderate or severe AR nor the need for aortic valve repair during the procedure altered survival and freedom from reoperation.

    Abstract Las técnicas conservadoras en cirugía valvular aórtica ofrecen una alternativa terapéutica a la prótesis en la insuficiencia aórtica. La técnica de David o reimplante valvular es una técnica establecida con buenos resultados pero de difícil reproducción en centros con menor experiencia. Analizar los resultados iniciales de la cirugía de reimplante valvular aórtico (RVA). Material y métodos: Análisis retrospectivo de los primeros 5 pacientes en los que se realizó RVA en nuestro centro entre marzo-noviembre de 2011. Resultados: La edad media del grupo es 66 ± 10 años, el 20% son mujeres y el EuroSCORE medio es 10,2 ± 3,7. El 40% presentaban dilatación de raíz aórtica y aorta ascendente y el 60% ectasia anuloaórtica. El grado de insuficiencia era III (40%) y IV (60%). El 40% recibió bypass coronario asociado. Tras RVA la insuficiencia era grado 0 (40%), grado I (40%) y grado II (20%); en este paciente se implantó prótesis aórtica en la misma cirugía. No hubo reoperación por sangra-do ni mortalidad postoperatoria El seguimiento medio de 4,8 ± 3,5 meses mostró una mejoría del grado funcional pasando de grado II (40%), grado III (40%) y grado IV (20%) precirugía a grado II (20%) y grado I (80%) poscirugía. No hubo reoperación por reaparición de insuficiencia aórtica durante el seguimiento. Conclusiones: a) los resultados iniciales en nuestro centro del reimplante valvular aórtico son óptimos a corto plazo, y b) la curva de aprendizaje ha sido rápida, con escasa repercu-sión en los pacientes.

    Abstract El presente trabajo tiene por objeto el estudio de una parte específica del pensamiento ético-político del filósofo escocés David Hume: sus descripciones acerca del origen de la justicia y el gobierno. Ambas son analizadas con la intención de esclarecer el tratamiento de la desigualdad que en ellas se ofrece. Nuestro objetivo es describir el particular proceso de criminalización de la desigualdad natural que comienza a producirse con la moralización de las leyes de la justicia tras la primera convención y se consolida tras la génesis del gobierno y el Estado.El principal objetivo durante el ejercicio descriptivo de esta investigación será identificar el perfil y la función de los diversos protagonistas, así como la aparición del paternalismo moral y el dominio en la interactuación de los mismos. Por último, trataremos de ofrecer una solución verosímil a uno de los conflictos perennes en la literatura secundaria sobre la filosofía ético política humeana ?la paradoja de la motivación?, reinterpretando el sentido de la estrategia adoptada por la figura del sensible knave.
