    Abstract Objectives: Explore what 'good' patient and family involvement in healthcare adverse event reviews may involve.
    Design: Data was collected using semi-structured telephone interviews. Interview transcripts were analysed using an inductive thematic approach.
    Setting: NHS Scotland.
    Participants: 19 interviews were conducted with patients who had experienced an adverse event during the provision of their healthcare or their family member.
    Results: Four key themes were derived from these interviews: trauma, communication, learning and litigation.
    Conclusions: There are many advantages of actively involving patients and their families in adverse event reviews. An open, collaborative, person-centred approach which listens to, and involves, patients and their families is perceived to lead to improved outcomes. For the patient and their family, it can help with reconciliation following a traumatic event and help restore their faith in the healthcare system. For the health service, listening and involving people will likely enhance learning with subsequent improvements in healthcare provision with reduction in risk of similar events occurring for other patients. This study suggests eight recommendations for involving patients and families in adverse event reviews using the APICCTHS model (table 3) which includes an apology, person-centred inclusive communication, closing the loop, timeliness, putting patients and families at the heart of the review with appropriate support for staff involved. Communicating in a compassionate manner could also decrease litigation claims following an adverse event.

    Abstract A Black woman in her 40s with past medical history significant for obesity treated with Roux-en-Y bypass surgery and a history of Raynaud's phenomenon, presented with acute pulmonary edema secondary to severe malignant hypertension and critically accelerated acute kidney injury, with evidence of systemic microangiopathic hemolytic anemia in the setting of clinical suspicion of systemic sclerosis sine scleroderma. Renin-angiotensin system blockade (angiotensin-converting enzyme inhibitor) was immediately started at the maximum possible dose in the setting of scleroderma renal crisis. Despite better control of blood pressure and volume status, kidney function continued to rapidly decline, thus a decision was made to go ahead with a kidney biopsy on day 3 of admission, which revealed severe features of scleroderma renal crisis with active thrombotic microangiopathy. The multidisciplinary team elected to treat the patient with terminal complement blockade using eculizumab in addition to high dose lisinopril and blood pressure control. Her serum creatinine peaked at 9.3 mg/dL shortly after eculizumab initiation, but improved soon after, dropping to 2.8 mg/dL after completion of the final eculizumab dose and 1.8 mg/dL 3 years later.

    Abstract Purpose of review: The current review highlights advances in the use of direct-acting antiviral (DAA) agents in the treatment of hepatitis C virus (HCV) in chronic kidney disease (CKD) stages G4-5, end-stage renal disease, and kidney transplantation. The use of DAA to facilitate kidney transplantation of HCV negative recipients with kidneys from HCV-infected donors and in the management of HCV-related cryoglobulinemia are also reviewed.
    Recent findings: DAA treatment results in rates of viral clearance (sustained virological response or SVR) of 90-100% in all studied CKD populations, comparable to SVR rates in the general population. DAA treatment allows safe and effective transplantation of HCV viremic kidneys into uninfected recipients.
    Summary: The high SVR results achieved with DAA allow successful treatment of previously under-treated CKD populations, and encouraged innovative interventions such as the use of HCV-infected donor kidneys to uninfected kidney transplant recipients.

    Abstract In light of the widespread use of high-energy lasers (HELs) for a variety of purposes, for example, standoff (>100 m) applications, will require the ability to monitor in real time the interaction with processed materials. While multiple sensing methods have been successfully developed for industrial HEL systems operating at close range, they are not compatible with the unique requirements of long-distance applications. Here, high-energy laser-induced oxidation spectroscopy (HELIOS) is demonstrated on carbon steel coupons as an efficient standoff assessment method compatible with long distance HEL applications. Acute monitoring of spectral features from thermally excited iron atoms and oxides, corroborated with real-time temperature measurements, reveals the interaction mechanisms at play.
