Abstract AimsThe aim of the study was to evaluate the prevalence and temporal trend of common mental health problems (CMHPs) in the UK by industrial classification from 2012–2014 to 2016–2018 while evaluating the corresponding gender disparities.MethodsWe used data from the Health Survey for England. CMPH was judged by a 12-item General Health Questionnaire. Industrial classifications were defined using the UK Standard Industrial Classification of Economic Activities. Data were fitted by the logistic models.ResultsIn this study, 19,581 participants covering 20 industries were included. In total, 18.8% of participants screened positive for CMHP in 2016–2018, which significantly increased from 16.0% in 2012–2014 [adjusted OR (AOR) = 1.17, 95% CI 1.08–1.27]. In 2016–2018, the prevalence of CMHP ranged from 6.2% in the industry of mining and quarrying to 23.8% in the industry of accommodation and food service activities. From 2012–2014 to 2016–2018, none of the 20 industries studied experienced significant decreases in the above prevalence; conversely, three industries saw significant increases, including wholesale and retail trade, repair of motor vehicles and motorcycles (AOR for trend = 1.32, 95% CI 1.04–1.67), construction (AOR for trend = 1.66, 95% CI 1.23–2.24), and other service activities, which cannot be classified (AOR for trend = 1.94, 95% CI 1.06–3.55). In total, 11 of the 20 industries studied had significant gender disparities against women, with the smallest gap being in the industry of transport and storage (AOR = 1.47, 95% CI 1.09–2.0) and the highest in the industry of arts, entertainment, and recreation (AOR = 6.19, 95% CI 2.94–13.03). From 2012–2014 to 2016–2018, gender disparities were narrowed only in two industries, including human health and social work activities (AOR for trend = 0.45, 95% CI 0.27–0.74), and transport and storage (AOR for trend = 0.5, 95% CI 0.27–0.91).ConclusionThe prevalence of CMHPs has increased and had a wide variation across industries in the UK. There were disparities against ...

Abstract Background: In China, improving mental health has been far behind its accomplishments for other diseases. With depression as one of the most prevalent mental disorders, the aim of this study was to evaluate temporal trends in the prevalence and treatment of those who screen positive for depression in China, by age, gender, and province.
Methods: We used data from three nationally representative sample surveys: the China Health and Retirement Longitudinal Study (CHARLS), the China Family Panel Studies (CFPS), and the Chinese Longitudinal Healthy Longevity Survey (CLHLS). Depression was judged by the Centre for Epidemiologic Studies Depression Scale. Access to treatment was judged by two items: if respondents received any treatment like anti-depressants, or if respondents received counselling from a mental health professional. Survey-specific weighted regressions were fitted to estimate the temporal trend and subgroup disparities, and then pooled by meta-analysis.
Results: In total 168,887 respondents were investigated. The overall prevalence of China populations who screen positive for depression was 25.7% (95% CI 25.2-26.2) during 2016-2018, decreased from 32.2% (95% CI 31.6-32.8) during 2011-2012. The gender gap increased with age and had no significant improvement from 2011-2012 to 2016-2018. The prevalence of depression in developed areas is more likely to show a lower value and decreasing trend, while the prevalence in underdeveloped areas is more likely to show a higher value and increasing trend, from 2011-2012 to 2016-2018. The overall proportion of those who received any needed treatment or counselling from a mental health professional slightly increased from 2011 (0.5%, 95% CI 0.4-0.7) to 2018 (0.9%, 95% CI 0.7-1.2), and mainly occurred for older adults aged 75 and above.
Conclusion: The prevalence of those who screen positive for depression decreased by about 6.5% from 2011-2012 to 2016-2018 in China, but only tiny improvements were made in accessibility to mental health care. Corresponding disparities were identified in age, gender, and province.

Abstract Background: Dementia's growing impact, especially in ageing societies such as the UK, emphasises the importance of modifiable risk factors as primary prevention targets. Despite this, the temporal progression and the population attributable fraction (PAF) of dementia attributable to these factors remain unclear. This investigation aims to examine the temporal trajectories of the modifiable risk factors for dementia in England from 2004 to 2019.
Methods: We used data from the English Longitudinal Study of Ageing collected between June, 2004, and July, 2019, covering 76 904 participants. We calculated the PAFs for 12 modifiable risk factors, as recommended by the Lancet Commission on dementia prevention, intervention, and care, and the individual weighted PAFs (IW-PAFs) for each risk factor. We analysed temporal trends to understand the changes in the overall PAF and IW-PAF over the study period.
Findings: The overall PAF for dementia showed a decrease from 46·31% in 2004-05 to 43·95% in 2018-19, but this trend was not significant (p=0·226). Hypertension, with an average IW-PAF of 8·67%, has been the primary modifiable determinant of dementia, trailed by obesity (6·42%), social isolation (5·84%), hearing loss (5·02%), depression (4·89%), low education (4·80%), physical inactivity (3·40%), diabetes (2·61%), smoking (2·08%), excessive alcohol consumption (1·22%), air pollution (0·44%), and traumatic brain injury (0·28%). During 2004-19, only IW-PAFs of low education (p=0·001), social isolation (p=0·034), and smoking (p=0·007) showed significant decreasing trends, whereas IW-PAFs of other factors had either stagnated with insignificant changes or, worryingly, climbed upwards.
Interpretation: This investigation provides valuable insights into the temporal trends of modifiable risk factors for dementia in England. The observed trends underscore the continuing relevance of these risk factors and the need for targeted public health strategies to address them. Notable, PAF was based on a theoretical scenario in which dementia risk can be wholly eliminated by removing risk factors, which should be explained with caution in practice.

Abstract Code-based algorithms are crucial tools in the detection of dementia using electronic health record data, with broad applications in medical research and healthcare. Vassilaki et al.'s study explores the efficacy of code-based algorithms in dementia detection using electronic health record data, achieving approximately 70% sensitivity and positive predictive value. Despite the promising results, the algorithms fail to detect around 30% of dementia cases, highlighting challenges in distinguishing cognitive decline factors. The study emphasizes the need for algorithmic improvements and further exploration across diverse healthcare systems and populations, serving as a critical step toward bridging gaps in dementia care and understanding.

Abstract Background: The economic burden of Pompe disease (PD) is under-researched. This study aimed to fill this gap and provide evidence-based suggestions for policy improvement based on policy simulation.
Methods: Data were derived from a nationally based cross-sectional survey on rare diseases in early 2018. Answers from 92 PD patients were used for data analysis and simulation. Catastrophic health expenditure (CHE) and impoverishment due to illness (IDI) were adopted to measure PD patients' economic burden. Two typical reimbursement patterns, a dosage-based model and a cost-based model, in China were simulated.
Results: Twenty-four pediatric and 68 adult PD patients were investigated. Families with pediatric PD patients on average had lower annual household incomes than families with adult PD patients (RMB 37 890 vs. RMB 66 120). The direct medical expense and out-of-pocket expenses were almost double for pediatric patients compared with adult patients (RMB 120 050 vs. RMB 66 350; RMB 112 710 vs. RMB 57 940, respectively). The direct non-medical expense for patients was almost six times the expense of adult patients (RMB 73 790 vs. RMB 13 080, respectively). About 88.24% of families with pediatric PD patients and 67.21% of families with adult PD patients suffered from CHE. Around 84.21% of families with pediatric PD patients and 45.90% of families with adult PD patients were forced to live in poverty due to illness. The simulation indicated that, although the two current reimbursement schemes helped reduce CHE, they almost had no effect on reducing IDI; the dosage-based model was more sensitive to changes in policy parameters.
Conclusion: Our study highlighted the alarmingly high disease burden faced by PD patients with first-hand patient-reported evidence. Our series of simulations could be a good reference for China and other countries to improve their reimbursement policy regarding PD.
