    Abstract Objetivos: desde la perspectiva de diseño de experiencia de usuario (UX), construimos tres usuarios-persona para ayudar a diseñar productos y servicios, digitales o presenciales, y en la tarea de planificar las bibliotecas públicas de Cataluña con el fin último de fidelizar y atraer usuarios y usuarias. -- Metodología: una encuesta representativa de la población catalana (1.205 respuestas) y un análisis estadístico bivariante permiten identificar los tres públicos objetivos que, gracias al análisis de contenido de tres grupos focales posteriores, dan lugar a los tres usuarios-persona. La técnica de usuario-persona visibiliza los posibles usuarios a partir de datos reales, facilita la generación de empatía entre el personal de la biblioteca y los usuarios, y asegura que el diseño esté centrado en las personas. -- Resultados: los tres usuarios-persona resultantes se describen y se complementan con escenarios basados en evidencias que deben ayudar a diseñar experiencias de usuarios. Estos son, Marc (usuario), estudiante adolescente que pretende codiseñar la biblioteca y que reclama actividades sociales presenciales; Maria (nunca usuaria), jubilada que necesita calor humano y que pone en evidencia la necesidad de una colaboración más estrecha entre servicios sociales y bibliotecas, y David (exusuario), trabajador de más de treinta años que considera que no necesita la biblioteca, aunque desconoce la mayoría de servicios que actualmente le ofrece.     Abstract Summary: The RDAVIDWebService package provides a class-based interface from R programs/scripts to fully access/control the database for annotation, visualization and integrated discovery, without the need for human interaction on its Web site (http://david.abcc.ncifcrf.gov). The library enhances the database for annotation, visualization and integrated discovery capabilities for Gene Ontology analysis by means of GOstats-based direct acyclic graph conversion methods, in addition to the usual many-genes-to-many-terms visualization.

    Abstract Objectius: amb una perspectiva de disseny d'experiència d'usuari (UX), construïm tres usuaris-persona per ajudar a dissenyar productes i serveis, digitals o presencials, i a la tasca de planificació de les biblioteques públiques de Catalunya amb la finalitat última de fidelitzar i atraure usuaris i usuàries. -- Metodologia: una enquesta representativa de la població catalana (1.205 respostes) i una anàlisi estadística bivariant permeten identificar els tres targets que, gràcies a l'anàlisi de contingut de tres grups focals posteriors, donen lloc als tres usuaris-persona. La tècnica d'usuari-persona visibilitza els possibles usuaris a partir de dades reals, facilita la generació d'empatia entre el personal de la biblioteca i els usuaris, i assegura que el disseny estigui centrat en les persones. -- Resultats: els tres usuaris-persona resultants s'acompanyen de descripcions i es complementen amb escenaris basats en evidències que han d'ajudar a dissenyar experiències d'usuaris. Aquests són el Marc (usuari), un estudiant adolescent que pretén codissenyar la biblioteca i que reclama activitats socials presencials; la Maria (mai usuària), una jubilada que necessita caliu humà i que fa evident la necessitat de col·laboració més estreta entre serveis socials i biblioteques, i el David (exusuari), treballador de més de trenta anys que considera que no necessita la biblioteca tot i que en desconeix la majoria de serveis que actualment li ofereix.     Abstract L. David Marquet, a decorated Navy Captain, transformed an underperforming submarine crew by empowering his subordinates to be leaders and reach their full potential. He called this intent-based leadership (IBL). What would happen if Marquet's model were implemented in Graduate Medical Education (GME)?In this letter to the editor, we summarize the potential of the IBL model in graduate medical education as opposed to the traditional leader-follower method. IBL harnesses human productivity toward the shared goals of GME, which are patient care and trainee learning. This shift in mindset could lead both teachers and trainees to focus more on the real reason that we undertake GME and change behaviors for the better. We suggest that IBL can and should be adopted in GME and propose that both patients and providers will benefit from this action.

    Abstract Presentamos nuestra experiencia en el tratamiento de aneurismas de la raíz aórtica según la técnica de David. Métodos: Entre abril de 2004 y febrero de 2010, 112 pacientes fueron intervenidos con el diagnóstico de aneurisma de la raíz de aorta (46 presentaban síndrome de Marfan). La mediana de edad fue de 44 años (rango 7-79 años) y el 78% eran varones. El diámetro medio de la raíz de aorta a nivel de senos fue de 52 ± 6 mm. El 44% presentaba insuficiencia aórtica (IAo) 0-I, el 11% IAo II y el 45% IAo III-IV Resultados: La mortalidad hospitalaria observada fue del 1,8%. Los tiempos medios de circulación extracorpórea (CEC) y clampaje aórtico fueron de 165 ± 47 y 139 ± 34 min, respectivamente. Con una mediana de seguimiento de 18 meses (rango 1-61 meses), el 90% de los pacientes presentan IAo 0-I y un 9% IAo II. A los 5 años, la supervivencia es del 83 ± 13%, y un 90 ± 9% están libres de reoperación (un paciente fue reintervenido a los 4 años por IAo grave). En el grupo de pacientes con síndrome de Marfan no hubo mortalidad hospitalaria, y el 100% de los pacientes están libres de reoperación y de IAo superior a II. Conclusiones: La técnica de David evidencia unos resultados excelentes en cuanto a libertad de reoperación y recurrencia de insuficiencia aórtica, pudiendo considerarse el tratamiento de elección en aneurismas de la raíz de aorta de pacientes jóvenes.

    Abstract The aortic valve sparing operation is of great interest in the treatment of severe aortic insufficiency caused by annulus enlargement in aortic annuloectasia. If valve leaflets are normal, and we want to prevent complications derived from valve prosthesis and chronic anticoagulation, techniques like David s for valve preserving surgery will be performed. Although long-term results of these techniques are usually good, in some cases, like in the one we are dealing with here fibrosis is produced in the native valve, which evolves into a severe aortic insufficiency needing reoperation. We hereby describe the time related events of aortic valve deterioration, when the reintervention was indicated, and how the surgery was performed.
