Abstract Purpose: The relevance of dual-energy computed tomography (DECT) for the detection of chronic thromboembolic pulmonary hypertension (CTEPH) still lies behind V/Q-SPECT in current clinical guidelines. Therefore, our study aimed to assess the diagnostic accuracy of DECT compared to V/Q-SPECT with invasive pulmonary angiogram (PA) serving as the reference standard.
Methods: A total of 28 patients (mean age 62.1 years ± 10.6SD; 18 women) with clinically suspected CTEPH were retrospectively included. All patients received DECT with the calculation of iodine maps, V/Q-SPECT, and PA. Results of DECT and V/Q-SPECT were compared, and the percent of agreement, concordance (utilizing Cohen's kappa), and accuracy (kappa
Results: In total, 18 patients were diagnosed with CTEPH (mean age 62.4 years ± 11.0SD; 10 women) and 10 patients had other diseases. Compared to PA, accuracy and concordance for DECT were superior to V/Q-SPECT in all patients (88.9% vs. 81.3%; k = 0.764 vs. k = 0.607) and in CTEPH patients (82.4% vs. 70.1%; k = 0.694 vs. k = 0.560). Furthermore, the mean radiation dose was significantly lower for DECT vs. V/Q-SPECT (
Conclusion: In our patient cohort, DECT is at least equivalent to V/Q-SPECT in diagnosing CTEPH and has the added advantage of significantly lower radiation doses in combination with simultaneous assessment of lung and heart morphology. Hence, DECT should be the subject of ongoing research, and if our results are further confirmed, it should be implemented in future diagnostic PH algorithms at least on par with V/Q-SPECT.

Title translation The German version of the Visual Function and Corneal Health Status (V‑FUCHS): a Fuchs dystrophy-specific visual disability instrument.
Abstract Background: In Fuchs' endothelial corneal dystrophy (FECD), the timing and expectation of interventions has changed in recent years because of improved therapeutic options. To assess outcomes and compare interventions, a patient-reported quantification of visual disability is important.
Objective: To develop the German version of the Visual Function and Corneal Health Status (V‑FUCHS), an instrument specifically for patients with FECD, and to assess its retest reliability and validity.
Material and methods: The V‑FUCHS was translated to German by certified forward and backward translators. Patients tested the translated questions regarding content areas, language and problem questions. After the pretest, V‑FUCHS was assessed for retest reliability and validity in FECD patients. Patients were also asked to complete a cataract-specific questionnaire (Catquest-9SF) and to complete V‑FUCHS again after 6 weeks to assess retest reliability.
Results: After successful pretesting in 30 patients, V‑FUCHS was completed by another 52 FECD patients before endothelial keratoplasty. Participants (median age 69 years) were (self-)employed or homemakers (42%), 49% were active vehicle drivers. The retest reliability confirmed agreement and consistency of response (intraclass correlation coefficient, > 0.60, both factors). The V‑FUCHS Visual Acuity score correlated with the Catquest-9SF score (r = 0.77; 95% CI, 0.67-0.85), whereas correlation of the Glare Factor specific for Fuchs' endothelial dystrophy with Catquest-9SF was weaker (r = 0.65; 95% CI, 0.50-0.76).
Conclusion: V‑FUCHS allows for standardized quantification of visual disability in patients with FECD. The German version of V‑FUCHS is a valid and reliable visual disability instrument. It may serve as a patient-reported outcome measure and needs to be studied further in clinical practice, research and trials.

Abstract Background: Evidence has accumulated that birch pollen immunotherapy reduces rhinoconjunctivitis to pollen of birch homologous trees. Therapeutic efficacy has been associated with IgE-blocking IgG antibodies. We have recently shown that sera collected after 16 weeks of sublingual immunotherapy with recombinant Bet v 1 (rBet v 1-SLIT) display strong IgE-blocking bioactivity for Bet v 1. Here, we assessed whether rBet v 1-SLIT-induced IgG antibodies display cross-blocking activity to related allergens in Fagales pollen.
Methods: IgE, IgG1 and IgG4 reactivity to recombinant Bet v 1, Aln g 1, Car b 1, Ost c 1, Cor a 1, Fag s 1, Cas s 1 and Que a 1 were assessed in pre- and post-SLIT samples of 17 individuals by ELISA. A basophil inhibition assay using stripped basophils re-sensitized with a serum pool containing high Bet v 1-specific IgE levels was established and used to assess CD63 expression in response to allergens after incubation with pre-SLIT or post-SLIT samples. IgG1 and IgG4 were depleted from post-SLIT samples to assess its contribution to IgE-cross-blocking.
Results: Sublingual immunotherapy with recombinant Bet v 1 boosted cross-reactive IgE antibodies and induced IgG1 and IgG4 antibodies with inter- and intra-individually differing reactivity to the homologs. Highly variable cross-blocking activities of post-SLIT samples to the different allergens were found. IgG1 and IgG4 antibodies displayed cross-blocking activity with individual variance.
Conclusions: Our mechanistic approach suggested that immunotherapy with the reference allergen Bet v 1 induces individual repertoires of cross-reactive IgG1 and IgG4 antibodies. The cross-blocking bioactivity of these antibodies was also highly variable and neither predictable from protein homology nor IgE-cross-reactivity.

O = V (OSiR₃)₃ (R = Me , Ph) undergoes a reaction with Lawesson reagent to yield dithiophosphonate complexes. All compounds obtained are characterized by ¹H and ⁵¹V NMR spectroscopy; the stability of thiovanadium(V) compounds is discussed. Abstract 1,2-Dimethylhydrazine co-ordinates to numerous metal cations. The structures of the complexes of Cr, Mn, Co, Ni, Cu, Zn, Cd, Hg and Pt are discussed and the complexing abilities of the 1,2-dimethylhydrazine molecule are determined on the basis of the ligand field spectra of the nickel and the chromium derivative. The results of polarography, cyclic voltammetry and photoelectron spectroscopy yield evidence on the redox properties of the neutral molecule. By means of EPR spectroscopy the nature of the radical products obtained by irradiation of 1,2-dimethylhydrazine, especially of the radical cation, is determined.
