    Abstract Objective:  This study aims to compare the two mental foramen obturating techniques in parameters like operator's ease, postoperative displacement of obturation material, and remission of pain after peripheral neurectomy in patients with trigeminal neuralgia.
    Study design:  This study adopts a single-centered comparative analytical approach.
    Place and duration of study: This study was performed in the Department of Oral and Maxillofacial Surgery, Armed Forces Institute of Dentistry (AFID), from October 1, 2023, to February 10, 2024.
    Methodology: Sixty patients fulfilling the inclusion criteria were included and divided into two groups (30 each), and peripheral neurectomy was performed under local anesthesia. In one group, gutta-percha sticks were used to obturate the foramina, and in the other group, titanium screws were used. Postoperatively, the operator's ease, postoperative displacement, and relapse of pain were recorded.
    Results:  Titanium screws were easy to handle, and there was no relapse of pain because of no displacement postoperatively when compared with gutta-percha sticks, which were hard to manipulate and showed a relapse of pain.
    Conclusions:  Titanium screws have better postoperative outcomes when compared with gutta-percha sticks in terms of the operator's ease, relapse of pain, and postoperative displacement.

    Abstract The extensive proliferation of recent coronavirus (COVID-19), all over the world, is the outcome of social interactions through massive transportation, gatherings and population growth. To disrupt the widespread of COVID-19, a mechanism for social distancing is indispensable. Also, to predict the effectiveness and quantity of social distancing for a particular social network, with a certain contagion, a generalized model is needed. In this manuscript, we propose a social distancing mediated generalized model to predict the pandemic spread of COVID-19. By considering growth rate as a temporal harmonic function damped with social distancing in generalized Richard model and by using the data of confirmed COVID-19 cases in China, USA and India, we find that, with time, the cumulative spread grows more rapidly due to weak social distancing as compared to the stronger social distancing, where it is explicitly decreasing. Furthermore, we predict the possible outcomes with various social distancing scenarios by considering highest growth rate as an initial state, and illustrate that the increase in social distancing tremendously decreases growth rate, even it tends to reach zero in lockdown regimes. Our findings not only provide epidemic growth scenarios as a function of social distancing but also provide a modified growth model to predict controlled information flow in any network.

    Abstract Öz: Aralık 2019’da Çin’in Vuhan kentinde başlayan yeni koronavirüs hastalığı (COVID-19) kısa sürede küresel bir salgına dönüşmüştür. Çalışma alanında bu salgına yakalanan çalışanların sosyal güvenlik açısından sahip olacakları haklar, hastalığın iş kazası veya meslek hastalığı olarak tanımlanıp tanımlanmayacağına bağlı olarak değişmektedir. COVID-19’un iş kazası mı, meslek hastalığı mı yoksa normal hastalık olarak mı tanımlanacağı belirsizdir. Bu çalışmada ilk olarak iş kazası ve meslek hastalıkları arasındaki farklar incelenmiştir. Sonrasında COVID-19’un iş kazası veya meslek hastalığı nitelikleri konusunda Türkiye’deki ve dünyadaki görüşler araştırılmıştır. Yapılan iş ile COVID-19 arasında doğrudan bir ilişki bulunan, maruziyet riski yüksek işlerde çalışanların COVID-19’a yakalanmasının meslek hastalığı niteliğinde olacağı ancak diğer çalışanların COVID-19’a yakalanmalarının iş kazası veya meslek hastalığı niteliğinde olmayacağı görüşü tercih edilen görüş olarak sunulmuştur. Çalışmanın son bölümünde COVID-19’a yakalanan çalışanların SGK’dan (Sosyal Güvenlik Kurumu) alabilecekleri parasal yardımlar belirtilmiştir. Yazarın bilgisine göre bu çalışma; iş kazası ve meslek hastalığının farklarını tartışarak çalışma alanında COVID-19’a yakalanmanın iş kazası ve meslek hastalığı niteliğini araştıran ve COVID-19 sonrası çalışanların sosyal güvenlik haklarını inceleyen ülkemizdeki ilk çalışmadır.     Abstract The study aimed to evaluate the gene expression of genes (TLR-4,NOD-2) in patients of Otitis media and healthy persons. This finding included 50 samples that collected from healthy subject and 100 samples from a patients suffering from otitis media who attended Al-Sadr Medical City (ENT Department) in Al-Najaf Governorate during the period from February 2022 to June 2022. The samples had an average age ranging from 5 to 70 years. The gene expression of these genes among those suffering from Otitis media and healthy individuals have been investigated in this case-control research. Using a PCR technology. polymerase chain reactions were carried out to amplify each sample for the patient and control groups. The results of the molecular study (gene expression) showed a high significant increase in the level of gene expression in patients for the two genes NOD-2, TLR-4 genes (14.78 ± 2.369, 16.42 ± 3.158), respectively, with a significant difference at P≤0.05. TLR-4, NOD-2 as used as a molecular diagnosis Otitis Media patients.

    Abstract Objective: To evaluate the effects of the type of ceramic, and the influence of the type of cervical substrate on the microleakage of aged Porcelain laminate veneers (PLVs).
    Materials and methods: A total of 48 sound human maxillary premolars were divided randomly into two groups (n=24), Group A: lithium disilicate PLVs; Group B: zirconia reinforced lithium silicate PLVs. The groups were further subdivided into four subgroups (n=12): (A1, B1): finishing line placed in Class V composite filling; (A2, B2): finishing line placed in sound enamel. In subgroups A1 and B1 standardized Class V cavities were prepared and restored with nanocomposite. Standardized PLVs tooth preparation was done for the specimens in all subgroups. Cementation of PLVs was done with a light cured resin cement and specimens were stored in distilled water for 2 weeks. Mechanical load cycling (45,000 cycle, 49 N at 2.5 Hz) and thermocycling procedure (500 cycles, 5-55°C) were done. A microleakage test was done with dye penetration (2% methylene blue) and the microleakage percentage was recorded and calculated using a stereomicroscope and ImageJ program.
    Results: Means of microleakage percentage of the subgroups were: A1 (6.6075%), A2 (4.6058%), B1 (7.3158%), and B2 (6.105%), Two-way ANOVA showed a significant effect of ceramic type and cervical composite substrate. According to samples
    Conclusion: The type of ceramic and the type of substrate both affects PLV microleakage. Lithium disilicate PLVs had significantly lower microleakage compared to zirconia reinforced lithium silicate PLVs. Teeth with cervical composite substrate had a significantly higher microleakage compared to teeth with enamel substrate.

    Abstract Immunoglobulin-G4-related disease (IgG4-RD) is a fibro-inflammatory condition that can impact any organs/tissues, including the vascular systems, resulting in aortitis/periaortitis/periarteritis (PAO/PA). The complex nature of this disease and limited understanding have led to potential delays in identifying and managing irreversible organ damage. Herein, we report a 17-year-old female with hyper IgG4 disease, sclerosing mesenteritis, short stature, and insulin resistance who presented with symptoms of fever, epigastric pain, left flank pain, vomiting, dizziness, decreased urine output, and diarrhea. Imaging studies revealed an arterial wall thickening of the ascending aorta and aortic arch, splenic abscesses, and enlarged lymph nodes, consistent with IgG4-related aortitis. Treatment with steroids and antifungal agents was initiated. However, the patient developed septic shock and multi-organ failure requiring inotropes and mechanical ventilation. Ascending aortic aneurysm rupture, in this case, probably led to the patient's demise, but unfortunately, no autopsy was done to confirm it. This case highlights the importance of identifying and addressing vascular involvement in IgG4-RD to prevent irreversible organ damage and mortality.

    Abstract The new coronavirus disease (COVID-19), which started in Wuhan, China in December 2019, rapidly became a global epidemic. In terms of social security, the rights of workers who was infected in the work field, depend on whether the disease is identified as a work accident or an occupational disease. It is uncertain whether COVID-19 to be identified as work accident, occupational disease or normal disease. In this study, firstly, the differences between work accidents and occupational diseases are examined. Afterwards, the concepts in Turkey and in the world about identifying COVID-19 as work accident or occupational disease were investigated. It is suggested that, the case for infected employees, whose duties directly related with COVID-19 and who work at high risk of exposure to COVID-19, should be identified as an occupational disease. However, in the case for other employees, COVID-19 should not be identified as a work accident or occupational disease. In the last part of the study, financial supports of SGK (Social Security Institution in Turkey) for COVID-19 infected employees are specified. To the author's knowledge, this is the first study in Turkey to investigate the differences of work accident and occupational disease in order to identify infection of COVID-19 in work environment and the social security rights of employees after COVID-19 infection.

    Abstract Objective The aim of this study was to assess the cognitive, emotional, social, and physical domains of quality of life (QoL) in pediatric patients with intractable epilepsy with an emphasis on depressed mood and suicidal ideation (SI). Methods This is a cross-sectional study conducted in pediatric neurology outpatient clinics in King Abdulaziz Medical City, Jeddah, Saudi Arabia. The sample consisted of 59 parents whose children aged 4-14 years of either sex had intractable epilepsy. The Quality of Life in Childhood Epilepsy Questionnaire - 55 (QOLCE-55) scale examined four domains of life: cognitive, emotional, social, and physical. Depressed mood and SI were part of the emotional domain. Results  The mean ± SD age of children was 8.2 ± 3.25. The mean ± SD of overall QoL was 43.02 ± 15.70, which reflected a poor QoL. Age was not related to the QoL. Female gender was significantly associated with a lower overall QoL (P = 0.0477). Patients with comorbidities had statistically insignificant lower QoL in the cognitive, social, and physical domains in addition to lower overall QoL. Seven of nine participants who reported feeling down reported having SI in the last four weeks (P < 0.001). Conclusions An intractable epilepsy-imposed burden negatively impacts all domains of QoL. Furthermore, females experience lower overall QoL compared to males. Children with comorbidities also tend to have lower QoL scores, although the differences were statistically insignificant. Additionally, a history of feeling down is associated with SI.

    Patients and methods: Materials and method: In this case control study, 43 patients with IBS (13 males and 30 females) diagnosed according to Rome IV criteria and other 43 controls where included, the patients and control are matched to the gender and age which ranged from 18 to55 years, the study groups were referred to perform a stool antigen test for H. pylori.
    Results: Results: Presence of H. pylori was compared between patients with IBS and controls using chi-square test. There was significant correlation between the presence of H. pylori and IBS, chi-square = 4.09, P-value = 0.043. The odd ratio for patients with H. pylori to have IBS was 2.53 (95% confidence interval: 1.02- 6.29). There is no signi"cant relationship between type of IBS and presence of H. pylori, chi-square = 2.87, P-value = 0.238. There is no significant relationship between the presence of H. pylori and age, BMI, gender, occupation, or marital state.
    Conclusion: Conclusions: Results of our study had shown an association of H. pylori infection in patients with IBS, which may propose a link of this infection in IBS pathophysiology.
