    Abstract Clinical implementation of new prediction models requires evaluation of their utility in a broad range of intended use populations. Here we develop and validate ancestry-specific Polygenic Risk Scores (PRSs) for Coronary Artery Disease (CAD) using 29,389 individuals from diverse cohorts and genetic ancestry groups. The CAD PRSs outperform published scores with an average Odds Ratio per Standard Deviation of 1.57 (SD = 0.14) and identify between 12% and 24% of individuals with high genetic risk. Using this risk factor to reclassify borderline or intermediate 10 year Atherosclerotic Cardiovascular Disease (ASCVD) risk improves assessments for both CAD (Net Reclassification Improvement (NRI) = 13.14% (95% CI 9.23-17.06%)) and ASCVD (NRI = 10.70 (95% CI 7.35-14.05)) in an independent cohort of 9,691 individuals. Our analyses demonstrate that using PRSs as Risk Enhancers improves ASCVD risk assessments outlining an approach for guiding ASCVD prevention with genetic information.

    Abstract Background: Polygenic risk score for coronary artery disease (CAD-PRS) improves precision in assessing the risk of cardiovascular diseases and is cost-effective in preventing cardiovascular diseases in a health system and may be cost-effective in other settings and prevention programs such as workplace cardiovascular prevention programs. Workplaces provide a conducitve environment for cardiovascular prevention interventions, but the cost-effectiveness of CAD-PRS in a workplace setting remains unknown. This study examined the cost-effectiveness of integrating CAD-PRS in a workplace cardiovascular disease prevention program compared to the standard cardiovascular workplace program without CAD-PRS and no-workplace prevention program.
    Methods: We developed a cohort simulation model to project health benefits (quality-adjusted life years gained) and costs over a period of 5 years in a cohort of employees with a mean age of 50 years. The model health states reflected the risk of disease (coronary artery disease and ischemic stroke) and statin prevention therapy side effects (diabetes, hemorrhagic stroke, and myopathy). We considered medical and lost productivity costs. Data were obtained from the literature, and the analysis was performed from a self-insured employer perspective with future costs and quality-adjusted life years discounted at 3% annually. Uncertainty in model parameter inputs was assessed using deterministic and probabilistic sensitivity analyses. Three programs were compared: (1) a workplace cardiovascular program that integrated CAD-PRS with the pooled cohort equation-a standard of care for assessing the risk of cardiovascular diseases (CardioriskSCORE); (2) a workplace cardiovascular prevention program without CAD-PRS (Standard-WHP); and (3) no-workplace health program (No-WHP). The main outcomes were total costs (US $2019), incremental costs, incremental quality-adjusted life years, and incremental cost-effectiveness ratio.
    Results: CardioriskSCORE lowered employer costs ($53 and $575) and improved employee quality-adjusted life years (0.001 and 0.005) per employee screened compared to Standard-WHP and No-WHP, respectively. The effectiveness of statin prevention therapy, employees' baseline cardiovascular risk, the proportion of employees that enrolled in the program, and statin adherence had the largest effect size on the incremental net monetary benefit. However, despite the variation in parameter input values, base case results remained robust.
    Conclusion: Polygenic testing in a workplace cardiovascular prevention program improves employees' quality of life and simultaneously lowers health costs and productivity monetary loss for employers.

    Abstract Over the past few years, polymer nanocomposites have garnered a significant amount of interest from both the scientific community and industry due to their remarkable versatility and wide range of potential uses in various fields, including automotive, electronics, medicine, textiles and environmental applications. In this regard, this study focuses on the influence of a compatibilizer rubber on a nanocomposite incorporating graphene nanoparticles (GNPs), with a polymer matrix based on a blend of polypropylene (PP) and polyethylene terephthalate (PET). This effect has been investigated on both isotropic samples and on anisotropic/spun fiber samples. The influence of the compatibilizer rubber on morphological, rheological and mechanical properties was analysed and discussed. Mechanical and morphological properties were evaluated on both isotropic samples obtained by compression moulding and melt-spun fibers. The addition of the rubbery compatibilizer increased the viscosity, improving interfacial adhesion, and the same effect was observed for the melt strength and breaking stretching ratios. Mechanical properties, including the elastic modulus, tensile strength and elongation at break, improved in both types of samples but more significantly in the fibers. These improvements were attributed to the orientation of the matrix, the formation of PET microfibrils, and the reduction in the size of graphene nanoparticles due to the action of the elongational flow. This reduction, facilitated by the elongation flow and the action of the compatibilizer, improved matrix-nanofiller adhesion due to the increased contact area between the two polymeric phases and between the filler and matrix. Finally, a transition from brittle to ductile behaviour was observed, particularly in the system with the compatibilizer, attributed to defect reduction and improved stress transmission.

    Abstract Classical theoretical models suggest that visual short-term memory can be divided in two main memory systems: sensory memory, a short-lasting but high-capacity memory storage and working memory, a long-lasting but low-capacity memory store. Whilst, previous research has systematically shown a strong interplay between attentional mechanisms and working memory, less clear is the role of attention in sensory memory. In the present study we approach this issue by asking whether withdrawing attentional resources by a dual task (Experiment 1) or by presenting task irrelevant information during memory maintenance (Experiment 2 and 3) similarly or differently affect sensory and working memory. Overall, results showed that sensory memory content was undermined not only by a simultaneous high-demanding cognitive task but even when purely task-irrelevant and non-masking visual distractors were presented during maintenance. Our data provide support against theories that consider sensory memories as a case of visual awareness free of attention.

    Abstract Previous studies suggest that the risk of human infection by hantavirus, a family of rodent-borne viruses, might be affected by different environmental determinants such as land cover, land use and land use change. This study examined the association between land-cover, land-use, land use change, and human hantavirus infection risk. PubMed and Scopus databases were interrogated using terms relative to land use (change) and human hantavirus disease. Screening and selection of the articles were completed by three independent reviewers. Classes of land use assessed by the different studies were categorized into three macro-categories of exposure ('Agriculture', 'Forest Cover', 'Urban Areas') to qualitatively synthesize the direction of the association between exposure variables and hantavirus infection risk in humans. A total of 25 articles were included, with 14 studies (56%) conducted in China, 4 studies (16%) conducted in South America and 7 studies (28%) conducted in Europe. Most of the studies (88%) evaluated land cover or land use, while 3 studies (12%) evaluated land use change, all in relation to hantavirus infection risk. We observed that land cover and land-use categories could affect hantavirus infection incidence. Overall, agricultural land use was positively associated with increased human hantavirus infection risk, particularly in China and Brazil. In Europe, a positive association between forest cover and hantavirus infection incidence was observed. Studies that assessed the relationship between built-up areas and hantavirus infection risk were more variable, with studies reporting positive, negative or no associations.

    Abstract Decarbonization of plastics is based on two main pillars: bio-based polymers and recycling. Mechanical recycling of biodegradable polymers could improve the social, economic and environmental impact of the use of these materials. In this regard, the aim of this study was to investigate whether concentrations of the same recycled biopolymer could significantly affect the rheological and mechanical properties of biodegradable monopolymer blends. Monopolymer blends are blends made of the same polymers, virgin and recycled. A sample of commercially available biodegradable blend was reprocessed in a single-screw extruder until two extrusion cycles were completed. These samples were exposed to grinding and melt reprocessed with 75% and 90% of the same virgin polymer. The blends were characterized by tensile tests and rheological tests. The results obtained showed that while multiple extrusions affected the mechanical and rheological properties of the polymer, the concentration of the reprocessed material present in the blends only very slightly affected the properties of the virgin material. In addition, the experimentally observed trends were accurately predicted by the additive model adopted.

    Abstract Background: A strong relationship has been observed between comorbidities and the risk of severe/fatal COVID-19 manifestations, but no score is available to evaluate their association in cancer patients. To make up for this lacuna, we aimed to develop a comorbidity score for cancer patients, based on the Lombardy Region healthcare databases.
    Methods: We used hospital discharge records to identify patients with a new diagnosis of solid cancer between February and December 2019; 61 comorbidities were retrieved within 2 years before cancer diagnosis. This cohort was split into training and validation sets. In the training set, we used a LASSO-logistic model to identify comorbidities associated with the risk of developing a severe/fatal form of COVID-19 during the first pandemic wave (March-May 2020). We used a logistic model to estimate comorbidity score weights and then we divided the score into five classes (<=-1, 0, 1, 2-4, >=5). In the validation set, we assessed score performance by areas under the receiver operating characteristic curve (AUC) and calibration plots. We repeated the process on second pandemic wave (October-December 2020) data.
    Results: We identified 55,425 patients with an incident solid cancer. We selected 21 comorbidities as independent predictors. The first four score classes showed similar probability of experiencing the outcome (0.2% to 0.5%), while the last showed a probability equal to 5.8%. The score performed well in both the first and second pandemic waves: AUC 0.85 and 0.82, respectively. Our results were robust for major cancer sites too (i.e., colorectal, lung, female breast, and prostate).
    Conclusions: We developed a high performance comorbidity score for cancer patients and COVID-19. Being based on administrative databases, this score will be useful for adjusting for comorbidity confounding in epidemiological studies on COVID-19 and cancer impact.
