    Abstract Purpose: To compare malignant pleural effusion (MPE) treatment outcomes and complications among patients receiving indwelling pleural catheter (IPC), talc pleurodesis (TPS), or dual therapy. Outcomes were determined by measuring length of stay (LOS) and post-procedure dyspnea scores. Complications were measured by comparing intervention failures and adverse events.
    Methods: The Veterans Affairs' Corporate Data Warehouse was utilized to retrospectively review the charts of 314 MPE subjects. Dyspnea scores were estimated by researchers and LOS was determined by adding the duration of stay for all admissions post procedure. Complications were recorded through chart review.
    Results: IPC exhibited higher failure rates than the other approaches 1 year post intervention. Pneumonia/chest infection rate and lung entrapment were also more prevalent. There was no significant difference in dyspnea rates. LOS illustrated a significant difference between groups, with talc patients spending a median of 7 days in the hospital immediately post procedure, while IPC and IPC + TPS patients spent a median of 3 and 2 days, respectively.
    Conclusion: Patients receiving IPC or combination treatment spend fewer days in the hospital than TPS patients. However, IPC appears to be associated with more adverse events and higher long-term failure rates than other management strategies.

    Abstract Given America’s leading position in the global economy, the U.S. government has frequently leveraged that power to punish “rogue states”, discourage nuclear proliferation, promote democratization, and create pressure for regime change. Washington relied on economic incentives in relations with Russia after 1991, but since 2012 the United States has utilized a broad range of economic sanctions against Russian side, leading to a significant deterioration in what was already a troubled relationship. In contrast to earlier comprehensive sanctions like those imposed on Iraq and Haiti, the U.S. is now crafting “smart” or targeted sanctions designed to exert maximum pressure on selected Russian elites and firms. Rather than evaluating the effectiveness of these measures on changing Russian behavior, the author explores the neglected domestic dimension of the U.S. sanctions process to improve understanding of U.S. foreign policy. This article draws on primary sources in the form of Congressional legislation, executive orders, and official statements to analyze U.S. sanctions imposed on Russia, and develops three brief case studies - the Magnitsky Act, post-Ukraine sanctions, and the Countering America’s Adversaries Through Sanctions Act - to explicate the main issues and actors driving U.S. sanctions. The author argues that domestic factors, including Congressional pressures and interest group activity, are critical to understanding U.S. sanctions regimes. While President Donald Trump has frequently resisted congressionally imposed sanctions, expectations for a more conciliatory approach towards Russia under the Trump administration have not materialized.

    Abstract Objective: The purpose of this study was to describe the diagnoses and chiropractic services performed by doctors of chiropractic operating within 3 military treatment facilities for patients with low back pain (LBP).
    Methods: This was a descriptive secondary analysis of a pragmatic clinical trial comparing usual medical care (UMC) plus chiropractic care to UMC alone for U.S. active-duty military personnel with LBP. Participants who were allocated to receive UMC plus 6 weeks of chiropractic care and who attended at least 1 chiropractic visit (n = 350; 1547 unique visits) were included in this analysis. International Classification of Diseases and Current Procedural Terminology codes were transcribed from chiropractic treatment paper forms. The number of participants receiving each diagnosis and service and the number of each service on unique visits was tabulated. Low back pain and co-occurring diagnoses were grouped into neuropathic, nociceptive, bone and/or joint, general pain, and nonallopathic lesions categories. Services were categorized as evaluation, active interventions, and passive interventions.
    Results: The most reported pain diagnoses were lumbalgia (66.1%) and thoracic pain (6.6%). Most reported neuropathic pain diagnoses were sciatica (4.9%) and lumbosacral neuritis or radiculitis (2.9%). For the nociceptive pain, low back sprain and/or strain (15.8%) and lumbar facet syndrome (9.2%) were most common. Most reported diagnoses in the bone and/or joint category were intervertebral disc degeneration (8.6%) and spondylosis (6.0%). Tobacco use disorder (5.7%) was the most common in the other category. Chiropractic care was compromised of passive interventions (94%), with spinal manipulative therapy being the most common, active interventions (77%), with therapeutic exercise being most common, and a combination of passive and active interventions (72%).
    Conclusion: For the sample in this study, doctors of chiropractic within 3 military treatment facilities diagnosed, managed, and provided clinical evaluations for a range of LBP conditions. Although spinal manipulation was the most commonly used modality, chiropractic care included a multimodal approach, comprising of both active and passive interventions a majority of the time.

    Abstract Objective: To combine prospective cohort studies, by including HLA harmonization, and estimate risk of islet autoimmunity and progression to clinical diabetes.
    Research design and methods: For prospective cohorts in Finland, Germany, Sweden, and the U.S., 24,662 children at increased genetic risk for development of islet autoantibodies and type 1 diabetes have been followed. Following harmonization, the outcomes were analyzed in 16,709 infants-toddlers enrolled by age 2.5 years.
    Results: In the infant-toddler cohort, 1,413 (8.5%) developed at least one autoantibody confirmed at two or more consecutive visits (seroconversion), 865 (5%) developed multiple autoantibodies, and 655 (4%) progressed to diabetes. The 15-year cumulative incidence of diabetes varied in children with one, two, or three autoantibodies at seroconversion: 45% (95% CI 40-52), 85% (78-90), and 92% (85-97), respectively. Among those with a single autoantibody, status 2 years after seroconversion predicted diabetes risk: 12% (10-25) if reverting to autoantibody negative, 30% (20-40) if retaining a single autoantibody, and 82% (80-95) if developing multiple autoantibodies. HLA-DR-DQ affected the risk of confirmed seroconversion and progression to diabetes in children with stable single-autoantibody status. Their 15-year diabetes incidence for higher- versus lower-risk genotypes was 40% (28-50) vs. 12% (5-38). The rate of progression to diabetes was inversely related to age at development of multiple autoantibodies, ranging from 20% per year to 6% per year in children developing multipositivity in ≤2 years or >7.4 years, respectively.
    Conclusions: The number of islet autoantibodies at seroconversion reliably predicts 15-year type 1 diabetes risk. In children retaining a single autoantibody, HLA-DR-DQ genotypes can further refine risk of progression.

    Abstract Abstract: According to realist approach, three most modern influential states Russia, China and US strive to gain maximum freedom in the international arena while retaining independence in domestic policies. The author suggests that a different constructivist approach should be used. Paying attention to the peculiarities of norms and acknowledging the influence of domestic policy on the norms, this approach allows to explain the differences in understanding sovereignty in different states. The political elites of Russia and China tend to consider that the strong central power secures the Westphalian sovereignty, while the historic experience of the US formed the ideal of decentralized power. Aiming at population protection, democracy promotion, terrorist persecution and maintaining US hegemony, American liberal internationalists and neocons are in general eager to step away from the traditional understanding of sovereignty. Russia and China consider sovereignty from the absolutist standpoint, though they have made a number of concessions to the changing norms of territorial integrity and humanitarian intervention. The article proves that the Russian, American and Chinese understanding of sovereignty (both domestic sovereignty and interdependence sovereignty in S.Krasner’s terms) is changing with the emergence of common interests and the necessity to counteract new global threats.
