    Abstract Background: Vitamin D toxicity is rare in pediatric population. Falsely elevated levels of 25-hydroxyvitamin D have been reported as a major challenge with immunoassay methods for quantifying vitamin D metabolites.
    Case presentation and method: Here, we present two pediatric cases of falsely elevated 25-hydroxyvitamin D that resulted in unnecessary further testing. We also report significant same-day variation in the measurement of 25-hydroxyvitamin D using the Abbott i2000SR immunoassay. Samples were spun twice and their values were confirmed with the gold standard liquid chromatography-tandem mass spectrometry (LC-MS/MS) method for confirmation.
    Conclusion: The addition of a centrifugation step prior to sample testing resolved the variation observed in the measurement of 25-hydroxyvitamin D levels. The patient samples were confirmed with instruments from a different vendor and LC-MS/MS. Re-centrifugation of samples resolved the variation in the 25-hydroxyvitamin D values.

    Abstract The Mermoz multi-professional health center is a healthcare practice, including 24 professionals, on the outskirts of a major French metropolis. This article presents the challenges faced from the idea to the implementation of an interprofessional primary care offer. The difficulty of freeing up time for the construction of the common real estate and sanitary project has been overcome thanks to powerful motivations: the desire for more interprofessionality in practices that are sometimes fragmented, the need for support in the management of complex patients, the pleasure of being a group, and the improvement of healthcare. The horizontal mode of governance enabled a health care offer adapted to the specific health needs of a multicultural and precarious sector to be combined with a place for teaching and research.

    Abstract Background: d-mannose is used as preventive measure against recurrent urinary tract infections (RUTIs). We studied d-mannosuria after a challenge test to identify favorable responders that could be targeted for long-term preventive therapy.
    Material and methods: Following institutional review board approval, women attending a specialized tertiary care center urology clinic with a history of RUTIs were invited to participate by providing a urine sample (baseline), followed by the intake of home-dose d-mannose, and a second urine sample 1 h later (post). Urine samples were processed according to a d-mannosuria assay technique reported previously by our group. d-mannose concentrations were normalized to urinary creatinine.
    Results: From July 2020 to March 2021, 26 patients met study criteria. Thirteen had a lower or unchanged ratio of baseline to post d-mannose, whereas 13 were responders. Among 19 taking 2 g, 12 had a lower or unchanged trend and 7 were responders with >20% increase in the d-mannose/creatinine ratio. Comparison of urinary baseline d-mannose/creatinine ratios was significantly different between the responder (mean = 0.337 ± 0.158) and nonresponder (mean = 0.692 ± 0.444; p = 0.016) groups. Urinary post d-mannose/creatinine ratios did not significantly differ between the two groups (p = 0.46). d-mannose-naïve patients had few responders, and age and urinary creatinine did not affect the findings.
    Conclusion: This preliminary study on d-mannose challenge tests indicates a urine response if urinary d-mannose/creatinine ratio is low, which it was in some women with a history of RUTIs.

    Abstract The better understanding of the global activity of vitamin D has led to an intense search for its involvement in non-skeletal diseases. This article presents an updated review of the relationship between vitamin D and pediatric respiratory pathology. A literature search was performed in PUBMED using free terms and MESH terms: vitamin D, asthma, respiratory system diseases, and bronchiolitis. Stu dies in human patients younger than 18 years and animals, published in English and Spanish until 2017 were included. 507 articles were found, of which 43 were included. Indirect evidence suggests a role of vitamin D and fetal lung maturation. In relation to pediatric pulmonary pathology, studies are scarce and inconclusive. Recent meta-analyses performed with individualized evaluation of the participants shows an important protective role of vitamin D supplementation in the prevention of severe asthma exacerbations and acute viral infections. In bronchiolitis, the results are contradictory, with no clear relationship between plasma levels and severity. There is not enough evidence to assess the benefits of vitamin D supplementation in cystic fibrosis and tuberculosis. A direct relationship between the severity of sleep-related breathing disorders and vitamin D plasma levels has recently been proposed, although the exact mechanisms involved in this association are unknown. Current information suggests that vitamin D supplementation may represent a cost-effective strategy in redu cing important causes of infant morbidity and mortality.

    Abstract Antecedentes: a nivel mundial, más de la mitad de la población reporta niveles séricos de 25(OH)D deficitarios. La vitamina D se ha asociado con múltiples funciones biológicas, como la regulación sérica del fósforo y del calcio, además de poseer posibles efectos sobre el dolor. En esta reflexión, se planteó el objetivo de comprobar la validez y la aplicabilidad de los resultados con respecto a la efectividad de la vitamina D como factor protector del dolor. Reflexión: las limitadas fuentes dietéticas y los estados deficientes de vitamina D permiten establecer la importancia de los cambios deficitarios como un posible factor de riesgo para el desarrollo del dolor, lo que hace necesario el establecimiento de consensos que permitan confirmar la asociación entre niveles óptimos de vitamina D y el riesgo de presentar dolor. Conclusión: existe alguna evidencia de que la deficiencia de vitamina D se relaciona con diferentes tipos de dolor, aunque los datos no son concluyentes, ya que los suplementos de vitamina D no siempre se han asociado con mejoría del dolor.
