    Abstract Background: Extrarenal rhabdoid tumours (ERT) are highly aggressive tumours that are poorly responsive to standard cytotoxic chemotherapy and are associated with a grim prognosis. Primary ERT of the liver are most commonly observed in early childhood and exceptionally rare later in life.
    Case presentation: We report the case of a 16-year-old male patient, presenting with flu-like symptoms after his second COVIDvaccination. During the work-up, a large solid liver lesion was incidentally discovered upon abdominal ultrasound examination. Pathological examination rendered the diagnosis of primary ERT of the liver, characterized by the loss of expression of INI-1 protein, encoded by the SMARCB1 gene. We summarized and discuss the existing literature by reviewing 53 pediatric and 6 adult cases, including the histological features treatment and outcomes of primary hepatic ERT.
    Conclusion: Primary ERT of the liver are usually not associated with specific signs or symptoms, making the diagnosis very challenging. As ERT are associated with a high metastatic rate, delayed diagnoses lead to increased mortality, as complete resection is not possible in advanced-stage cases. Therefore, early diagnoses, enabling complete resection of the tumour are crucial to improve patient outcomes. Of interest, primary ERT of the liver, is associated with biallelic loss of the SMARCB1 (SWI/ SNF Related, Matrix Associated, Actin Dependent Regulator Of Chromatin, Subfamily B, Member 1) gene, a potential target for cancer therapeutics. This is, to our knowledge, the first case of a hepatic rhabdoid tumour treated with liver transplantation.

    Abstract Hypophosphatemia is a recognized side effect of treatment of iron deficiency anemias with injectable iron. We analyzed 35 clinical trials that used ferric carboxymaltose (FCM) or iron sucrose (IS). Hypophosphatemia prevalence ranged from 0 to 91.7%. FCM-induced a significant (P<0.001) greater hypophosphatemia prevalence and phosphatemia decrease than IS (52.0% [95% CI: 42.2-61.8%] vs. 7.7% [95% CI: -2.8 to 18.2%] and -1.12mmol/L [95% CI: -1.36 to -0.89mmol/L] vs. -0.13mmol/L [95% CI: -0.59 to 0.32mmol/L]). FCM-induced hypophosphatemia was dose-dependent. The nadir of hypophosphatemia was reached in almost all studies after 7 and 14days. Hypophosphatemia persisted at the end of the study in 53.8% of the reported studies that used FCM and lasted up to 6months. FCM-induced an increase in intact circulating fibroblast growth factor 23 and in renal phosphorus excretion while serum 1-25 dihydroxyvitamin D was decreased. Risk factors for hypophosphatemia after FCM therapy were low basal circulating phosphate or ferritin, low body weight, high glomerular filtration rate, serum parathyroid hormone or hemoglobin and age, whereas renal insufficiency was associated with a lower risk. In conclusion, hypophosphatemia is common after treatment with injectable iron, FCM being associated with a higher risk than IS and with disorders of phosphocalcium metabolism. Monitoring of blood phosphate and 1-25 dihydroxyvitamin D could be considered during FCM therapy.

    Abstract Patients with Ehlers-Danlos Syndrome (EDS) frequently suffer from severe chronic pain. We carried out an observational cohort study to assess the effectiveness of compression garments (CGs) for reducing this pain. Patients with non-vascular EDS were given custom-made Cerecare ® CGs during a visit to a specialist clinic (visit V0). They were followed up over 2 years with visits every 6 months (V1–V4). At each visit, pain was assessed for the joints treated with CGs using a visual analogue scale (VAS; 0–100 mm). Additional measures were obtained to assess neuropathic pain (painDETECT questionnaire), proprioception/balance (Berg Balance Scale), and functional independence, amongst others. Data were analyzed for 67 patients with EDS (hypermobile: 91%; classical: 6%; kyphoscoliotic: 3%). For the most painful joint, the mean VAS rating was 71.5 ± 22.8 mm at V0; this decreased to 53.5 ± 25.5 mm at V1 and 45.7 ± 29 mm at V4 ( t -tests: p < 0.0001). From V0 to V4, improvements were also seen for pain at the other joints, neuropathic pain, functional independence, proprioception/balance, and the incidence of sprains and dislocations/subluxations, although not all comparisons were statistically significant ( p < 0.05 level). These results indicate that CGs may effectively reduce the pain and joint instability in non-vascular EDS patients.

    Abstract The study took place in southern Thailand, in Phatthalung province, and is part of “Heveadapt,” a Franco-Thai research project. The aim of this project was to show how smallholder rubber plantations can adapt and remain sustainable in the face of variable climatic conditions and deep changes in socio-economic context. Among the various types of cropping systems, agroforestry practices were identified as promising. Rubber agroforestry systems (AFS) are economically more productive than rubber monocrop plantations and give more flexibility to smallholders, in particular when rubber prices are low, which has been the case since 2013. However, adoption of AFS during the mature period of the plantations is very low in Thailand. The policy to boost rubber agroforestry practices still needs to be developed with all local stakeholders. The objectives of the study were to identify the possibilities and capacities to use current AFS dynamic networks as a basis to set up a rubber agroforestry innovation platform. The research team studied: i) farmers’ collective organizations, groups or networks with rubber AFS partially or entirely and ii) the social dynamic enabling the sharing of knowledge and know-how. An individual producer’s grid, with original farmers or strong knowledge with ease of sharing, was also studied. The role of local institutions involved in the promotion of rubber AFS was also analyzed. This allowed us to design an innovation platform and to define activities that suit the socio-economic context of Phatthalung province. The main aims of the platform are to promote cooperation among innovative producers and the transmission between them of knowledge and know-how about rubber AFS. This could an efficient tool that Thai rubber institutions could set up to encourage the adoption of rubber AFS by farmers.

    Purpose: Recommendations to improve therapeutics are proposals made by pharmacists during the prescription review process to address suboptimal use of medicines. Recommendations are generated daily as text documents but are rarely reused beyond their primary use to alert prescribers and caregivers. If recommendation data were easier to summarize, they could be used retrospectively to improve safeguards for better prescribing. The objective of this work was to train a deep learning algorithm for automated recommendation classification to valorize the large amount of recommendation data.
    Methods: The study was conducted in a French university hospital, at which recommendation data were collected throughout 2017. Data from the first 6 months of 2017 were labeled by 2 pharmacists who assigned recommendations to 1 of the 29 possible classes of the French Society of Clinical Pharmacy classification. A deep neural network classifier was trained to predict the class of recommendations.
    Results: In total, 27,699 labeled recommendations from the first half of 2017 were used to train and evaluate a classifier. The prediction accuracy calculated on a validation dataset was 78.0%. We also predicted classes for unlabeled recommendations collected during the second half of 2017. Of the 4,460 predictions reviewed, 67 required correction. When these additional labeled data were concatenated with the original dataset and the neural network was retrained, accuracy reached 81.0%.
    Conclusion: To facilitate analysis of recommendations, we have implemented an automated classification system using deep learning that achieves respectable performance. This tool can help to retrospectively highlight the clinical significance of daily medication reviews performed by hospital clinical pharmacists.

    Abstract Villeneuve d'Ascq, c'est aujourd'hui plus de 60 000 habitants et seulement 40 ans d'histoire. Nombre de Villeneuvois d'aujourd'hui ont vécu en direct sa naissance au milieu des années 1970. Leur témoignage permet de comprendre comment les aspirations à « mieux habiter » y ont été vécues, comment une mémoire locale s'est sédimentée autour de quelques événements rassembleurs - mobilisations collectives à la suite de nombreuses malfaçons dans les constructions, lancement des chartes d'aménagement et de concertation, fêtes de quartier. On retrouve certaines valeurs et utopies « post soixante-huitardes » portées par les aménageurs et par beaucoup des premiers Villeneuvois. Les récits recueillis participent d'une véritable « poétique sociale », pour reprendre les termes de l'anthropologue Michael Herzfeld, par exemple ceux qui évoquent les pratiques et les représentations de ce que nous appelons l'« espace public de proximité », les multiples manières de se l'approprier, de ruser avec ses limites, de le partager. Villeneuve d'Ascq a été une ville d'utopistes rêvant d'une ville plus humaine, plus conviviale, qu'ils soient aménageurs ou habitants « pionniers ». Avec le temps, l'arrivée de nouveaux habitants, elle se transforme. Ce que tous nous disent finalement, c'est que si une ville procède de politiques d'aménagement et des manières de l'habiter, une « bonne ville », celle où l'on se sent bien, c'est aussi une ville qu'on peut rêver et imaginer. Ainsi ce livre, tout en apportant des données nouvelles sur l'histoire de Villeneuve d'Ascq et sur sa sociologie, appuyées par de nombreux témoignages, entend montrer qu'une ville ne peut se comprendre hors du recueil des représentations que s'en font ses habitants

    Abstract Objectives: The emergence of artificial intelligence (AI) is catching the interest of hospital pharmacists. A massive collection of health data is now available to train AI models and hold the promise of disrupting codes and practices. The objective of this systematic review was to examine the state of the art of machine learning or deep learning models that detect inappropriate hospital medication orders.
    Methods: A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. MEDLINE and Embase databases were searched from inception to May 2023. Studies were included if they reported and described an AI model intended for use by clinical pharmacists in hospitals. Risk of bias was assessed using the Prediction model Risk Of Bias ASsessment Tool (PROBAST).
    Results: 13 articles were selected after review: 12 studies were judged to have high risk of bias; 11 studies were published between 2020 and 2023; 8 were conducted in North America and Asia; 6 analysed orders and detected inappropriate prescriptions according to patient profiles and medication orders; and 7 detected specific inappropriate prescriptions, such as detecting antibiotic resistance, dosage abnormality in prescriptions, high alert drugs errors from prescriptions or predicting the risk of adverse drug events. Various AI models were used, mainly supervised learning techniques. The training datasets used were very heterogeneous; the length of study varied from 2 weeks to 7 years and the number of prescription orders analysed went from 31 to 5 804 192.
    Conclusions: This systematic review points out that, to date, few original research studies report AI tools based on machine or deep learning in the field of hospital clinical pharmacy. However, these original articles, while preliminary, highlighted the potential value of integrating AI into clinical hospital pharmacy practice.
