    Abstract Background: Factor XI (FXI) deficiency, also known as hemophilia C, is a rare bleeding disorder of unpredictable severity that correlates poorly with FXI coagulation activity. This often poses great challenges in perioperative hemostatic management. Thromboelastography (TEG) is a method for testing blood coagulation using a viscoelastic hemostatic assay of whole blood to assess the overall coagulation status. Here, we present the successful application of intraoperative TEG monitoring in an FXI-deficient patient as an individualized blood transfusion strategy.
    Case summary: A 21-year-old male patient with FXI deficiency was scheduled to undergo reconstructive surgery for macrodactyly of the left foot under general anesthesia. To minimize his bleeding risk, he was scheduled to receive fresh frozen plasma (FFP) as an empirical prophylactic FXI replacement at a dose of 15-20 mL/kg body weight (900-1200 mL) before surgery. Subsequent FFP transfusion was to be adjusted according to surgical need. Instead, TEG assessment was used at the beginning and toward the end of his surgery. According to intraoperative TEG results, the normalization of coagulation function was achieved with an infusion of only 800 mL FFP, and blood loss was minimal. The patient showed an uneventful postoperative course and was discharged on postoperative day 8.
    Conclusion: TEG can be readily applied in the intraoperative period to individualize transfusion needs in patients with rare inherited coagulopathy.

    Abstract The $J/\psi \to \Xi^0 \bar{\Xi}^{0}$ process and subsequent decays are investigated using $(10087 \pm 44)\times 10^6$ $J/\psi$ events collected at the BESIII experiment. The decay parameters of $\Xi^0$ and $\bar{\Xi}^0$ are measured with greatly improved precision over previous measurements to be $\alpha_{\Xi} = -0.3750 \pm 0.0034 \pm 0.0016$, $\bar{\alpha}_{\Xi} = 0.3790 \pm 0.0034 \pm 0.0021$, $\phi_{\Xi} = 0.0051 \pm 0.0096 \pm 0.0018$~rad, $\bar{\phi}_{\Xi} = -0.0053 \pm 0.0097 \pm 0.0019$~rad, where the first and the second uncertainties are statistical and systematic, respectively. From these measurements, precise $CP$ symmetry tests in $\Xi^0$ decay are performed, and $A^{\Xi}_{CP} = (-5.4 \pm 6.5 \pm 3.1) \times 10^{-3}$ and $\Delta\phi^{\Xi}_{CP} = (-0.1 \pm 6.9 \pm 0.9) \times 10^{-3}$~rad are consistent with $CP$ conservation. The sequential decay also enables a separation of weak and strong phase differences, which are found for the first time to be $\xi_{P}-\xi_{S} = (0.0 \pm 1.7 \pm 0.2) \times 10^{-2}$~rad and $\delta_{P}-\delta_{S} = (-1.3 \pm 1.7 \pm 0.4)\times 10^{-2}$~rad, respectively. In addition, we measure the $\Lambda$ decay parameters and test $CP$ symmetry in $\Lambda$ decays.

    Abstract Background: High-molecular weight kininogen (HK) circulates in plasma as a complex with zymogen prekallikrein (PK). HK is both a substrate and a cofactor for activated plasma kallikrein, and the principal exosite interactions occur between PK N-terminal apple domains and the C-terminal D6 domain of HK.
    Objectives: To determine the structure of the complex formed between PK apple domains and an HKD6 fragment and compare this with the coagulation factor XI (FXI)-HK complex.
    Methods: We produced recombinant FXI and PK heavy chains (HCs) spanning all 4 apple domains. We cocrystallized PKHC (and subsequently FXIHC) with a 31-amino acid synthetic peptide spanning HK residues Ser565-Lys595 and determined the crystal structure. We also analyzed the full-length FXI-HK complex in solution using hydrogen deuterium exchange mass spectrometry.
    Results: The 2.3Å PKHC-HK peptide crystal structure revealed that the HKD6 sequence WIPDIQ (Trp569-Gln574) binds to the apple 1 domain and HK FNPISDFPDT (Phe582-Thr591) binds to the apple 2 domain with a flexible intervening sequence resulting in a bent double conformation. A second 3.2Å FXIHC-HK peptide crystal structure revealed a similar interaction with the apple 2 domain but an alternate, straightened conformation of the HK peptide where residues LSFN (Leu579-Asn583) interacts with a unique pocket formed between the apple 2 and 3 domains. HDX-MS of full length FXI-HK complex in solution confirmed interactions with both apple 2 and apple 3.
    Conclusions: The alternate conformations and exosite binding of the HKD6 peptide likely reflects the diverging relationship of HK to the functions of PK and FXI.

    Abstract The $\Xi^0$ asymmetry parameters are measured using entangled quantum $\Xi^0$-$\bar{\Xi}^0$ pairs from a sample of $(448.1 \pm 2.9) \times 10^6$ $\psi(3686)$ events collected with the BESIII detector at BEPCII. The relative phase between the transition amplitudes of the $\Xi^0 \bar{\Xi}^0$ helicity states is measured to be $\Delta \Phi = -0.050 \pm 0.150 \pm 0.020$~rad, which implies that there is no obvious polarization at the current level of statistics. The decay parameters of the $\Xi^0$ hyperon $(\alpha_{\Xi^0}, \alpha_{\bar{\Xi}^0}, \phi_{\Xi^0}, \phi_{\bar{\Xi}^0})$ and the angular distribution parameter $(\alpha_{\psi(3686)})$ and $\Delta \Phi$ are measured simultaneously for the first time. In addition, the $CP$ asymmetry observables are determined to be $A^{\Xi^0}_{CP} = (\alpha_{\Xi^0} + \alpha_{\bar{\Xi}^0})/(\alpha_{\Xi^0} - \alpha_{\bar{\Xi}^0})$ $= -0.007$ $\pm$ 0.082 $\pm$ 0.025 and $\Delta \phi^{\Xi^0}_{CP} = (\phi_{\Xi^0} + \phi_{\bar{\Xi}^0})/2$ $= -0.079$ $\pm$ 0.082 $\pm$ 0.010 rad, which are consistent with $CP$ conservation.

    Abstract Background: Coagulation cascade contributes to thrombotic and hemorrhagic diseases, but it remains unclear whether coagulation factors X (FX) and XI (FXI) levels are associated with cardiovascular diseases.
    Objective: To evaluate prospective associations of FX and FXI levels with incident acute coronary syndrome (ACS), stroke, and their subtypes (acute myocardial infarction, unstable angina, ischemic stroke, and hemorrhagic stroke).
    Methods: We performed a nested case-control study (n = 1846) within the Dongfeng-Tongji cohort from 2013 to 2016 matched on age (within 1 year), sex, and sampling date (within 1 month) by incidence density sampling, and measured plasma FX and FXI levels by enzyme-linked immunosorbent assay. FX and FXI levels were categorized into three groups (low, <25th; middle, 25th to <75th; and high ≥75th percentiles) according to distributions, and conditional logistic regression models were used to estimate odds ratios (ORs) and 95% confidence intervals (CIs).
    Results: After adjustment for traditional cardiovascular risk factors, compared with middle groups, the OR (95% CI) in high levels of FX and FXI were 1.11 (0.79-1.56) and 0.96 (0.68-1.36) for incident ACS, and 1.01 (0.63-1.62) and 1.72 (1.14-2.60) for incident stroke, respectively. As for subtypes of ACS and stroke, only high FXI levels were significantly associated with incident ischemic stroke (OR 1.66, 95% CI 1.05-2.65). Moreover, all associations remained steady after additional adjustment for platelet and leukocyte.
    Conclusion: FXI levels were associated with a greater risk of incident ischemic stroke but not hemorrhagic stroke or ACS. FX levels were not associated with incident ACS or stroke.

    Abstract Hereditary coagulation factor XI (FXI) deficiency is a rare bleeding disorder, but information on FXI deficiency in Taiwanese patients remains scarce. We evaluated clinical and genetic features of severe FXI deficiency patients in Taiwan. We collected clinical information and performed coagulation laboratory tests and genetic studies in ten unrelated Taiwanese families with severe FXI deficiency. FXI coagulation activity was assayed using a one-stage method. FXI antigen was determined using enzyme-linked immunosorbent assay. Underlying genetic mutations were evaluated using direct sequencing methods. Ten unrelated Taiwanese patients with hereditary FXI deficiency and variable bleeding tendencies were analyzed. Half of the patients were male. The most common bleeding manifestations were easy bruising (40%), bleeding after dental procedures (40%), and postoperative bleeding (33%). Two patients (20%) were asymptomatic. No correlation was found between bleeding manifestations and baseline FXI levels. Three novel mutations were identified: c.1322delT p.Lys442Cysfs*8, c.599G > C p.Cys200Ser, and IVS4 c.325 + 2del124. Two common mutations, c.1107C > T p.Tyr369* (40%) and c.841C > T p.Gln281* (30%), were also found. No correlation existed between bleeding and FXI activity, highlighting the difficulty in predicting FXI deficiency-related bleeding. Three novel FXI genetic mutations and two common mutations were identified, contributing to the known spectrum of FXI deficiency-related mutations.

    Abstract Knee Osteoarthritis is one of the most common diseases of elder worldwide. Qi-Fang-Xi-Bi-Granules (QFXBG) is a new Chinese medicine granules employed for the treatment of Knee Osteoarthritis. Fangchinoline (FAN) and tetrandrine (TET) are used as targets of quality control of QFXBG. A simple, practical high-performance liquid chromatographic method was developed for the simultaneous quantitation of FAN and TET in Stephaniae tetrandrae radix and QFXBG. The analysis was performed on a Athena C18 column (250 × 4.6 mm, 5 µm) with mobile phase of methanol-water (65 : 35, v/v, pH 3.0, adjusted by glacial acetic acid) containing 1.0 g/L sodium 1-octanesulfonate. This method was validated in terms of linearity, precision, accuracy and recovery. Results showed that this method had good linearity with R(2) at >0.999. The limit of detection and limit of quantification for FAN were 0.13 and 0.35 mg/L, while for TET were 0.28 and 0.76 mg/L, respectively. The relative standard deviations of precision were 0.1-1.3% for intraday and 0.5-2.4% for interday. The recovery was 94.56-98.81% of FAN and 94.07-99.12% of TET, respectively. The chromatographic analytical time was 14 min. This method was successfully applied for the quantitative analysis of FAN and TET in Stephaniae tetrandrae radix and QFXBG, so that it could be extended to quality control of QFXGB in commercial.

    Abstract Studying the characteristics and health risks of heavy metals in atmospheric fine particulate matter (PM₂.₅) is a crucial component of understanding atmospheric pollution in China. Integrated 24 h PM₂.₅ samples were collected in winter and summer 2016 in Xi’an, China. The pollution levels, speciation, and health risks of seven PM₂.₅-bound metal elements (Al, As, Cd, Cr, Ni, Pb, and Zn) were investigated in this study. The average concentration of PM₂.₅ was 50.1 ± 30.4 μg m⁻³ and was higher in winter than in summer. Significant seasonal variations in the elements were also observed. The average concentration ratios of Al, As, Cd, Cr, and Pb decreased in summer by 17.5%, 6.4%, 42.5%, 34.1%, and 61.4% compared with their concentrations in winter, respectively, whereas Ni and Zn increased by 37.7% and 7.6% in summer. The soluble and exchangeable fraction (F1) accounted for large proportions of Cd and Pb concentrations, indicating their greater hazard to the environment and human health. Al, As, and Cr mainly existed in the residual state (F4), which had relatively high stability in particulate matter. Ni was consistently distributed in different forms (F1–F4). The bioavailability evaluation indicated that Pb, Cd, Ni, and Zn were potential bioavailable element which exhibited strong biological toxicity. Although the concentration of Pb was very low, its BI value was the highest. The carcinogenic and non-carcinogenic risks of Cr and As were relatively high, and thus require attention from the government and environmental management departments.
