    Abstract Rationale: It is very likely that we will miss Bing-Neel syndrome (BNS) when its initial sign is anemia.Patient concerns: A 59-year-old woman presented with episodic loss of consciousness, anemia, and extremity muscle strength scores (5-) and extremity tendon reflexes (++).
    Diagnoses: Magnetic Resonance Imaging (MRI) showed abnormal signal in the left hippocampus, left insula, and right occipital lobe. Quantitative serum immunoglobulins showed elevated immunoglobulinm (IgM) (60.6g/L). Bone marrow biopsy showed lymphoplasmacytic lymphoma (LPL) and tested positive for the MYD88 L265P mutation suggesting Waldenström macroglobulinemia (WM).
    Interventions: The patient underwent 3 plasma exchange treatments in the department of hematology followed by chemotherapy (cyclophosphamide for injection, bortezomib for injection).
    Outcomes: The patient's condition improved after treatment.
    Lessons: Clinicians must remain vigilant, as BNS may be the only sign of WM progression in a patient well-controlled on treatment.

    Abstract Taussig-Bing anomaly and aortic arch obstruction are two types of complex congenital cardiac malformations. Almost 50% of patients with Taussig-Bing anomaly have aortic arch obstruction. This report assesses the surgical outcomes of single-stage correction in neonates with both defects. Between November 2006 and November 2015, 39 neonates with Taussig-Bing anomaly and aortic arch obstruction (28 patients with coarctation of the aorta and 11 patients with interrupted aortic arch) underwent a one-stage arterial switch operation and aortic reconstruction. There were three in-hospital deaths and one late death (8 months after the surgery). The short-term survival rate was 92.3% (36/39), and the mid-term survival rate was 89.7% (35/39). Follow-up data were available for all patients who survived the operation (range 6-92 months). Echocardiology showed six cases of recoarctation, three cases of left ventricular outflow tract obstruction, three cases of right ventricular outflow tract obstruction, four cases of pulmonary artery stenosis, five cases of aortic regurgitation, and eight cases of pulmonary regurgitation. Eight patients required a reoperation during the follow-up period with no mortality. All survivors remained in good condition (New York Heart association functional class I or II). Single-stage correction of Taussig-Bing anomaly with aortic arch obstruction in neonates had favorable short- and mid-term outcomes in terms of mobility and reoperation rate. The optimal operative procedure should be chosen according to the position of the coronary arteries and the type of aortic anomaly.

    Abstract Background. TCM considers that diabetes belongs to the scope of Xiaoke Bing. Compound prescriptions are characteristics of TCM. For a certain medicine, its compatibility with different medicines can exert different efficacies in different prescriptions. Using the TCM compound prescriptions containing Ginseng Radix ET Rhizoma in ancient books for Xiaoke Bing as an example, this study introduces new methods to investigate the rule of TCM compatibility. Methods. Frequency analysis was accomplished by programs written in Perl. The R, Cytoscape, and DpClus software were used to carry out the association rules analysis, the construction of the TCM interconnection network, and the graph clustering analysis, respectively. Results. Frequency analysis ranked the frequencies of medicine, medicinal flavors, properties, and meridian attributions, and it was found that some of them are significantly higher than others. Six association rules were obtained. The TCM interconnection network showed that there are close medicine associations among prescriptions, and we got 17 categories of closely related prescriptions from the network. Conclusions. Ginseng Radix ET Rhizoma was widely used in treating Xiaoke Bing. Our results are consistent with the understanding of Xiaoke Bing in TCM; hence, it is demonstrated that the methods are effective for exploring the rule of TCM usage in prescriptions. This analysis could provide references for the treatment of diabetes.

    Abstract The purpose of this work was to evaluate the potential of gibberellic acid (GA₃) combined with homobrassinolide (HBR) treatment to improve fruit anthocyanin content, to maintain high-quality properties, to reduce susceptibility to physiological disorders, and to retard the development of bioactive compounds in yellow-fleshed ‘Rainier’ and red-fleshed ‘Bing’ sweet cherries. Application of 1, 5, and 10 mg L⁻¹ HBR once at straw color did not affect anthocyanin accumulation in ‘Rainier’, but 5 or 10 mg L⁻¹ HBR application elevated anthocyanin content in ‘Bing’. No significant differences were observed in fruit size, soluble solids content (SSC), titratable acidity, or surface pitting between the control and HBR treatments in either cultivar after 4 weeks of storage at 0 °C; however, low rates of stem browning and decay were noted in HBR treatments. A single application of GA₃ resulted in firmer fruit. Spraying HBR and GA₃ to cherries effectively recovered anthocyanin synthesis and increased the levels of phenolics and SSC. Moreover, in combination, these compounds reduced stem browning and pitting development. In conclusion, pre-harvest application of 10 mg L⁻¹ GA₃ combined with 5 mg L⁻¹ HBR had a potential to retain the normal skin color development and phenolics synthesis, to increase SSC accumulation, and to reduce fruit susceptibility to disorders in mid-season sweet cherry.
