Abstract Volatile organic compounds (VOCs) are the organic compounds having a minimum vapor pressure of 0.13 kPa at standard temperature and pressure (293 K, 101 kPa). Being used as a solvent for organic and inorganic compounds, they have a wide range of applications. Most of the VOCs are non-biodegradable and very easily become component of the environment and deplete its purity. It also deteriorates the water quality index of the water bodies, impairs the physiology of living beings, enters the food chain by bio-magnification and degrades, decomposes and manipulates the physiology of living organisms. To unveil the adverse impacts of volatile organic compounds (VOCs) and their rapid eruption and interference in the living world, a review has been designed. This review presents an insight into the currently available VOCs, their sources, applications, sampling methods, analytic procedures, imposition on the health of aquatic and terrestrial communities and their contamination of the environment. Elaboration has been done on representation of toxicological effects of VOCs on vertebrates, invertebrates, and birds. Subsequently, the role of environmental agencies in the protection of environment has also been illustrated.

Abstract The discharge of industrial effluents has a significant impact on the Water Quality Index (WQI) of the water bodies and is a major source of contamination of groundwater. The present study investigated the physicochemical characteristics and scrutinized the pollution potential of the tannery, textile, and electroplating effluents uploading into the Kala Sanghian drain, located in Jalandhar, Punjab, India. In this study, 12 samples were collected from the four sites (leather complex drain (LD), leather complex outlet (LO), focal point drain (FD), and Bulandpur drain (BD)) of Kala Sanghian drain in the dry season. The result showed that the drain under consideration is very much contaminated and the water is not suitable for irrigation and agricultural purposes. Rather it has a bad impact on the health of local people, the physiology of aquatic organisms, and the soil quality of agricultural land nearby. The present study confirmed the water quality index was more than 100, indicating a highly contaminated drain and water is unfit for any use. The correlation analysis shows that there exists a positive correlation between TDS and temperature (r = 0.994), DO and pH (r = 0.808), BOD and temperature (r = 0.987), BOD and TDS (r = 0.978), EC and temperature (r = 0.963), EC and TDS (r = 0.954), and EC and BOD (r = 0.956). The principal component analysis (PCA) confirms that PC1 alone has more than 89% of the variance with high positive loading for TDS, temperature, EC, and BOD. The hierarchical cluster analysis (HCA) reflected two clusters where cluster 1 consists of pH, DO, temperature, and BOD of water while cluster 2 consists of TDS and EC of water. The PCA and HCA study of the data set confirms the high degree contribution of anthropogenic activities through the application of chemicals in agriculture, disposal of municipal waste, and industrial effluents in the deterioration of water quality. The results of the study will help to enhance the sustainable action plan for the management of industrial effluents in the studied area.

Abstract A 35-year-old Indian male office worker presented with complaints of progressive weakness, fatigability, and diminished vision. The patient followed a strict vegetarian diet. Clinical examination revealed pallor. Fundoscopy revealed a bilateral macularbleed. Vitamin B12 deficiency was determined to be the cause based on the clinical and laboratory results. Three months later, he reported a near-normal vision in both eyes and a normal complete hemogram. The present clinical picture demonstrates an unusual facet of clinical feature in vitamin B12 deficiency, an event rarely encountered.

Abstract Childhood onset endocrine disorders need long-term medical, psychological and social management. Over time, many illnesses evolve, while others may witness onset of new complications. Thus, the components of the care change as the child grows into adolescence and then adulthood. The transition of children and adolescents with chronic endocrine disorders to adult care continues to be a major challenge. Pediatric and adult healthcare teams should together design a transitional care plan that is developmentally appropriate and responsive to the needs of young adults. The preparation for transition to adult care should begin early in adolescence and involve both the adolescent and his parents. A structured and planned transitional care bridges the gap between pediatric and adult care teams, promote ongoing engagement and build trust with the new healthcare teams. Combined pediatric-adult care transition model for endocrine conditions has yielded high adherence rates and patient satisfaction.

Abstract Background: Leptospirosis is the most common zoonotic illness worldwide, caused by pathogenic spirochete bacteria called Leptospira. It is clinically presented with mild to moderate in most cases. However, sometimes, the course may be severe with multiorgan dysfunction.
Case presentation: We present two rare cases of leptospirosis with peripheral dry gangrene of the lower extremities. A 25-year-old male, farmer by occupation without any significant past medical history had been diagnosed with a case of leptospirosis that complicated to digital gangrene on 15 days of illness during hospitalization. Another 21-year-old male student was admitted for leptospirosis and developed digital gangrene on 19 days of illness. All clinical findings were resolved on the steroid.
Conclusion: Apart from a high index of suspicion and awareness of unusual manifestations, serology plays a vital role in making an accurate and quick diagnosis to initiate appropriate therapy.

  10. Article: Audit on Practices of Endotracheal Suctioning in Intensive Care Unit Patients among Health Care Workers (HCWs).

Indian journal of critical care medicine : peer-reviewed, official publication of Indian Society of Critical Care Medicine

    2024  Volume 28, Issue 1, Page(s) 58–65

Abstract: ... guidelines for performing ETS.: How to cite this article: Singh R, Bhalotra AR, Sharma S. Audit ...

Abstract Background: Endotracheal suctioning (ETS) is one of the most frequently performed invasive procedures in intensive care units (ICUs). This audit was aimed at studying current practices and knowledge regarding ETS in the Indian critical care setup, with the background aim of raising awareness regarding correct practices as per current recommendations in critically ill patients.
Materials and methods: After registering the trial with the clinical trial registry, India, a structured audit questionnaire containing 20 questions pertaining to ETS was distributed through electronic media among resident doctors working in the ICUs across India. Responses received were statistically analyzed.
Results: The questionnaire was sent to 530 clinicians, of which only 200 (37.73%) responded. The audit revealed that only 22% respondents set the maximum negative pressure every time before suctioning, on the suction apparatus and only 32% said they would choose a catheter size of less than half the internal diameter of the endotracheal tube (ETT). About 90% of the respondents did not routinely do any form of documentation of the ETS. Almost 72% of the responders opined that closed suction systems reduce the chances of developing ventilator-associated pneumonia (VAP). Only 46% of respondents thought that no solution should be routinely instilled in ETT during ETS.
Conclusion: There is lack of awareness regarding frequency and technique of ETS, infection control and monitoring required during ETS. Institutional protocols should be in place to follow correct guidelines for performing ETS.
