Abstract Intense pulsed light (IPL) therapy using noncoherent broad-spectrum light has been reported to be effective for hair removal, and also for treating superficial pigmented lesions like ephelides and solar lentigines. We report complete regression of a pigmented melanocytic nevus, histologically confirmed, after hair removal treatment with IPL. The use of lasers and IPL is a common procedure used by dermatologists and even other professions for the treatment of cosmetically troubling skin conditions. The main advantage of such treatment is a reduction of surgical scars, thus producing a favorable cosmetic outcome, but a major limitation is that histopathologic diagnosis is not usually obtained prior to treatment. Such devices should be carefully used in patients with potentially dangerous melanocytic lesions. We also review the recent literature regarding inadequate treatment of melanocytic lesions with lasers.

Abstract Cutaneous myxomas are rare, benign, connective tissue tumors composed of stellate cells set in a loose mucoid stroma containing delicate reticulin fibers running in various directions. They are also called angiomyxomas to reinforce their vascular component. These lesions have been recognized as part of Carney complex. We report an 11-year-old boy affected by disseminated cutaneous angiomyxomas since the age of 2, without any other component of Carney complex.

Abstract Neurofibromatosis-Noonan syndrome is an entity that combines both features of Noonan syndrome and Neurofibromatosis type 1. This phenotypic overlap can be explained by the involvement of the RAS-MAPK pathway (mitogen-activated protein kinase) in both disorders. We report the case of a 17-year-old boy with Neurofibromatosis 1 with Noonan-like features, who complained of the progressive appearance of blue-gray lesions on his back.

Abstract Beads based on a mannuronate(M)-rich alginate (86 % M units) were prepared by adding the polysaccharide solution to a crosslinking bath containing different concentrations (0.5, 2 and 10 wt%) of XCl₂ where X = Ca, Cu or Zn. Primarily focus was on Zn, due to its antioxidant, anti-inflammatory and anti-microbial capabilities. The beads were characterized by Field-Emission Scanning Electron Microscopy (FESEM), Fourier-Transform Infra-Red spectroscopy (FT-IR), Thermogravimetric Analysis (TGA), Small-Angle X-ray Scattering (SAXS) and compression tests. The crosslinking agent significantly influenced the properties of the resulting beads. Specifically, Ca-based beads exhibited a smoother surface, while Cu- and Zn-based beads appeared rougher. Interestingly, Zn-based beads displayed a core-shell structure. Young moduli ranged from 3500 and 7000 MPa, with the highest values observed for Zn-beads. SAXS investigation at 0.5 wt% XCl₂ suggested increase in the densely packed domains amount in the order: Ca < Cu < Zn. Extended X-ray Absorption Fine Structure (EXAFS) showed that the coordination number was 4.3 ± 0.4 for Cu, and 4.0 ± 0.2 and 1.1 ± 0.1 for Zn in 0.5 wt% XCl₂ alginate xerogels, in agreement with reported Density Functional Calculations on Cu²⁺– and Zn²⁺–MM complexes. The results from FT-IR, compositional analysis and EXAFS collectively suggested a bridging coordination for these systems.

  10. Article ; Online: The impact of age beyond ploidy: outcome data from 8175 euploid single embryo transfers.

Abstract Purpose: The rate of embryonic aneuploidy increases with increasing female age and is the primary cause of lower pregnancy and live birth rates (LBR) in older reproductive age women. This retrospective cohort study evaluates single euploid embryo transfers to determine whether an age-related decline in reproductive efficiency persists.
Methods: A total of 8175 non-donor single embryo transfers (SET) after pre-implantation testing for aneuploidy (PGT-A) and cryopreservation were included. These were divided into five groups by patient age: < 35 years old (n = 3789 embryos transferred), 35-37 (n = 2200), 38-40 (n = 1624), 41-42 (n = 319), and > 42 (n = 243). Implantation rate (IR), clinical pregnancy rate (CPR), and LBR were calculated for each group as a percentage of embryos transferred and compared. CPR was also analyzed as a percentage of implanted pregnancies, and LBR as a percentage of clinical pregnancies, to determine when age has the greatest impact. These results were then adjusted for confounding variables via a multivariate logistic regression model.
Results: Implantation rates negatively correlated with age. After adjusting for confounders, women 38 years or older had a significantly lower IR than those under 35 (OR 0.85, 95%CI 0.73-0.99 for 38-40 years old; 0.69, 0.53-0.91 for 41-42, and 0.69, 0.51-0.94 for > 42). These differences are also apparent in CPR and LBR. The rates of progression to clinical pregnancy and live birth did not differ significantly by age group. Other factors observed to affect IR independently were anti-Müllerian hormone (AMH), day of embryo transfer, and embryo morphology.
Conclusion: While selection of euploid embryos may be effective in overcoming a significant proportion of the age-related decline in reproductive efficiency, a decrease in IR, CPR, and LBR persists even when analyzing only euploid embryo transfers. The observed impact of aging is, therefore, independent of ploidy, as well as of other variables that affect reproductive efficiency. These results indicate that factors other than aneuploidy contribute to reproductive senescence.
