    Abstract Objective: This systematic literature review aims to evaluate the effectiveness of transdermal opioids in managing cancer pain and their impact on the quality of life (QoL) of patients.
    Data sources: A systematic literature review conducted following the PRISMA protocol, focusing on randomized clinical trials found in the Lilacs, Embase, PubMed, and SciELO databases over the last 20 years.
    Study selection and data extraction: We included randomized clinical trials, published in English, Portuguese, or Spanish, which assessed the impact of transdermal opioids on the QoL. Data extraction was facilitated using the Rayyan app.
    Data synthesis: Six articles meeting the inclusion and exclusion criteria were analyzed. These studies covered a population ranging from 24 to 422 cancer patients experiencing moderate to severe pain. The risk of bias was assessed in each study, generally being categorized as uncertain or high.
    Relevance to patient care and clinical practice: The findings indicate that the analgesic effectiveness and side effects of transdermal formulations (specifically buprenorphine and fentanyl) for managing moderate to severe cancer pain are comparable to, or in some cases superior to, those of oral opioids traditionally employed.
    Conclusions: Transdermal therapy was suggested to have several advantages over oral opioid therapy in enhancing cancer patients' QoL. These benefits span various dimensions, including pain management, physical functioning, mental health, vitality, overall patient improvement, anger/aversion, strength/activity, general QoL, cognitive and emotional functions, fatigue, and insomnia.

    Abstract ABSTRACT BACKGROUND AND OBJECTIVES: Low back pain is a problem that affects 80% of adults at some point in life, it is among the top 10 primary causes of consultation with internists and, every year, workers are absent from work for more than seven days due to this disease, causing a great impact in productivity and economy. The objective of this study was to provide the clinician working at the primary care with an adequate approach to the patient with chronic low back pain, emphasizing the differential diagnosis of this disease. CONTENTS: The etiological characterization of low back pain is a process that requires a propaedeutic approach that includes the clinical history, physical and complementary exams. The approach to low back pain of mechanical origin, and others less common such as those with a neuropathic component or resulting from inflammation, infection or neoplasia was developed, based on the literature. CONCLUSION: The diagnosis of low back pain is essential, yet challenging for the primary care physician. Most patients with back pain can be treated at the primary care setting, provided that the GP has the proper knowledge to elaborate the differential diagnosis of this disease.

    Abstract Background and objectives: Prolonged use of opioids is a possibility for chronic pain treatment. Opioids are effective for virtually all chronic noncancer pain syndromes, but may cause dependence. The aim of this paper is to review the use and rotation of opioids in chronic non-oncologic pain.
    Content: The use of potent opioids is controversial and not recommended as first-line drugs due to the possibility of dependence. The following topics are described: tolerance, addiction, risk factors for addiction, rotation or replacement, general administration rules, conversion tables, and tips for prescribing opioids.
    Conclusions: Opioids are drugs with proven efficacy for chronic non-oncologic pain, but its prescription should meet certain criteria in order to reduce the incidence of adverse effects and addiction.

    Abstract Background and objectives: Painful neuropathies are common and often difficult to treat. The objective of this report was to review the subject to facilitate diagnosis and pain relief.
    Contents: The classification, causes, type of fibers involved, manifestations, diagnosis, adjuvant tests, questionnaires used in the diagnosis, and treatment are described.
    Conclusions: The subject is very broad and involves several causes and treatments that often should be combined to obtain adequate pain control.

    Title translation The analgesic effect of intravenous lidocaine in the treatment of chronic pain: a literature review.
    Abstract Background: Pain is a public health problem, greatly impairing quality of life. Almost 80% of patients with chronic pain reported that their pain interferes with activities of daily living, and two thirds reported that the pain causes negative impact on their personal relationships. The physical and functional disability, whether temporary or permanent, compromises the professional activity and causes work absenteeism, increasing costs of health systems.
    Objectives: The aim of this review is to analyze, based on the literature, the analgesic effect of lidocaine administered intravenously for the treatment of chronic pain and to evaluate the reduction of pain intensity in patients with chronic pain, focusing on musculoskeletal and neuropathic etiology.
    Methodology: The method used was a review of the literature, consisting in searching the scientific literature on the efficacy of intravenous lidocaine infusion in the treatment of patients with chronic pain.
    Content: Of the 19 studies reviewed, 12 had results that confirm the analgesic effect of intravenous lidocaine in patients with chronic pain. Most authors used doses of 5mg/kg infused for 30minutes or more, producing significant analgesia with variable duration (minutes to weeks).
    Conclusions: Based on the literature review, it is not possible to uniformly specify the most effective and safe dose of lidocaine administered intravenously for the treatment of neuropathic or musculoskeletal pain. As for effectiveness, the intravenous infusion of lidocaine as an alternative for the treatment of chronic pain of various etiologies seems very promising, but further studies need to be conducted.

    Abstract Chronic pain is a debilitating condition and, in most cases, difficult to treat. A prominent example of this is neuropathic pain. Understanding pathophysiological mechanisms of pain and, therefore, making this knowledge into an effective treatment is still a challenge to experts. Pain can now be considered as a neuro-immune disorder, since recent data indicate critical involvement of innate and adaptive immune responses following injury, and this interaction plays an important role in the onset and perpetuation of chronic pain. The aim of this article is to review the relationship between immune system and chronic pain, especially about neuropathic pain, and focusing on cytokines, chemokines and lymphocytes.

    Abstract Justificativa: A dor é um problema de saúde pública, comprometendo sobremaneira a qualidade de vida. Quase 80% dos pacientes com dor crônica relataram que a dor interfere em suas atividades da vida diária, e dois terços afirmaram que a dor provoca impacto negativo nas relações pessoais. A incapacidade física e funcional, seja temporária ou permanente, compromete a atividade profissional e causa absenteísmo ao trabalho, elevando os custos dos sistemas de saúde. Objetivos: O objetivo desta revisão é analisar, com base na literatura, o efeito analgésico da lidocaína administrada por via intravenosa no tratamento da dor crônica e avaliar a redução da intensidade da dor em pacientes com dor crônica, focando a etiologia musculoesquelética e neuropática. Metodologia: O método adotado foi o de revisão da literatura, consistindo na busca de artigos científicos sobre a eficácia da infusão intravenosa de lidocaína no tratamento de pacientes com dor crônica. Conteúdo: Dos 19 estudos revisados, 12 apresentaram resultados que confirmam a ação analgésica da lidocaína por via intravenosa em pacientes com dor crônica. A maioria dos autores utilizou doses de 5 mg/kg infundidas por 30 minutos ou mais, produzindo analgesia significativa com duração variável (de minutos a semanas). Conclusões: Com base na revisão da literatura, não é possível uniformemente especificar a dose mais eficaz e segura de lidocaína administrada por via intravenosa no tratamento da dor neuropática ou musculoesquelética. Quanto à eficácia, a infusão intravenosa da lidocaína como alternativa para o tratamento da dor crônica de etiologias diversas parece bastante promissora, embora estudos adicionais necessitem ser realizados.
