    Title translation Satisfaction with dentist-patient interaction at the municipal polyclinic: level and affecting factors.
    Abstract Objective: The study level of patient satisfaction with interaction with doctors of various specialties in the municipal dental clinic.
    Material and methods: 596 patients who received dental care at the Severodvinsk Dental Polyclinic State Autonomous Healthcare Institution participated in the cross-sectional study. Satisfaction was studied across 10 domains using a questionnaire. The average values of the scores obtained for different specialties of the doctor were compared using a variance analysis for each of the domains. The relationship between patient satisfaction and factor characteristics - specialty and age of the doctor, gender and age of the patient or legal representative was evaluated using multivariate linear regression analysis with the calculation of regression coefficients and 95% confidence intervals (CI).
    Results: At least a good level of satisfaction was found in all 10 domains for doctors of all specialties. The age of the doctor was inversely related to the domains "Communication on equal terms" and "Active listening". Respondents were statistically significantly less satisfied with interaction with dental therapists, dental surgeons and pediatric dentists in all domains compared to interaction with orthodontists, except for the domain "Prognosis". Satisfaction did not depend on the gender and age of the patients.
    Conclusions: Lower satisfaction in different domains can be explained by limited time for patient admission and/or insufficient training of dentists in terms of communication with patients. The assessment of satisfaction with the doctor's appointment is an important indicator for determining the ways of developing the education of specialists and the organization of medical care in dentistry.

    Abstract Background: Postpartum depression affects 10% to 20% of birthing people and is associated with changes in healthcare use. Little is known about the association between postpartum depressive symptoms and choice to use contraception; however, both untreated or undertreated depression and short interpregnancy intervals pose substantial perinatal health risks.
    Objective: This study aimed to evaluate whether postpartum depressive symptoms are associated with changes in decisions to use any method of contraception.
    Study design: This retrospective cohort study included birthing people who delivered between 2017 and 2022 and were referred to a collaborative care program for mental healthcare. Through this program, birthing people with mental health conditions have access to specialized perinatal mental healthcare and prospective symptom monitoring via a patient registry. Postpartum depressive symptoms are assessed via the Patient Health Questionnaire-9, and scores were stratified by severity according to clinical cutoffs. Contraceptive method choice was determined by documentation in the electronic health record and dichotomized as "none" if the participant declined all forms of contraception both at delivery and at the postpartum visit. Bivariable and multivariable analyses were performed.
    Results: Of the 1871 participants that met the inclusion criteria, 160 (8.5%) had postpartum Patient Health Questionnaire-9 scores of >14, representing moderately severe or worse depressive symptoms, and 43 (2.3%) had severe (Patient Health Questionnaire-9 of >19) depressive symptoms. Birthing people with higher Patient Health Questionnaire-9 scores were more likely to have medical comorbidities; to have a higher body mass index; to self-identify as Black, Native Hawaiian or Pacific Islander, or Hispanic or Latina; and to have a preterm delivery and less likely to be married or nulliparous than those with Patient Health Questionnaire-9 scores of ≤14. There was no difference in any other sociodemographic or clinical characteristics. The choice to use any contraceptive method decreased with increasing depressive symptoms in bivariable and multivariable analyses, reaching statistical significance in birthing people with severe depressive symptoms (adjusted odds ratio, 2.92; 95% confidence interval, 1.46-5.84).
    Conclusion: Severe perinatal depressive symptoms are associated with a declination of any form of postpartum contraception. This finding becomes increasingly relevant as abortion access continues to be threatened across the United States, compounding the potential effect of opting not to use contraception.

    Abstract The median overall survival of glioblastoma patients is about 15 months. Only a small number of patients survive 3 years. The factors of a favorable prognosis for the 'longevity phenomenon' in glioblastoma patients are not fully understood.
    Objective: to determine the occurrence rate of long-living patients with glioblastomas, identify clinical predictors of a favorable prognosis, and identify the presence and prognostic significance of an IDH1 mutation.
    Material and methods: Among 1494 patients operated on for glioblastoma at the Burdenko Neurosurgical Institute from 2007 to 2012, there were 84 (5.6%) patients who lived more than 3 years after primary surgery. In all the cases, histological specimens were reviewed, and immunohistochemical detection of a mutant IDH1 protein was performed. Overall survival was calculated from the time of first surgery to the date of the last consultation or death, and the recurrence-free period was calculated from the time of first surgery to MRI-verified tumor progression.
    Results: The median age of long-living patients with glioblastoma was 45 years (19-65 years). All tumors were located supratentorially. The median Karnofsky performance status score at the time of surgery was 80 (range, 70-100). All patients underwent microsurgical resection of the tumor, followed by chemoradiotherapy. The median recurrence-free period was 36 months (5-98 months). Overall survival of 48, 60, and 84 months was achieved in 23, 15 and 6% of patients, respectively. Among 49 specimens available for the IDH1 analysis, 14 (28.6%) specimens had a mutant protein. There was no significant difference in survival rates in patients with positive and negative results for IDH1 (44.1 vs. 40.8 months; p>0.05).
    Conclusion: The significance of various factors that may be predictors of a favorable course of the disease is discussed in the literature. This work is the first part of analysis of prognostically significant factors positively affecting overall survival of glioblastoma patients. In our series, the predictors of a favorable prognosis for long-living patients with the verified diagnosis of glioblastoma were as follows: young age, the supratentorial location of the tumor, a high Karnofsky score before surgery, and tumor resection. In our series, we used immunohistochemical tests and found no prognostic significance of the IDH1 gene mutation; further analysis will require application of direct sequencing. We plan to study other morphological and molecular genetic features of tumors, which explain prolonged survival of glioblastoma patients, as well as the role of various types of combined chemoradiation treatment.

    Title translation Long association tracts of the human white matter: an analysis of 18 hemisphere dissections and in vivo HARDI-CSD tractography.
    Abstract Background: Anatomy of the conduction tracts of the cerebral cortex has been studied for a long time. Invention of diffusion tensor tractography renewed interest in this subject. The objectives of this work were to develop and improve protocols for dissection of the long association tracts of the human brain with studying the features of their segmentation, topography, and variability, compare the obtained data with the MR tractography data, and prepare for further clinical and anatomical studies.
    Material and methods: We used 18 cerebral hemispheres (from 10 males and 8 females; 9 left and 9 right hemispheres). The mean age of cadavers was 68 years. Specimen were fixated in accordance with the Klingler technique. Immediately after collection, specimens were placed in a 10% formalin solution for at least 4 weeks. After that, the pia was removed; specimens were frozen at -20 °C for a week and then unfrozen in a 96% ethanol solution for a day. We performed 10 lateral dissections, 2 lateral dissections with isolation of the frontal aslant tract, 2 basal dissections, 1 combined basolateral dissection, 2 frontal dissections, and 1 medial dissection. At the time of dissection and after it, specimens were stored in a 96% ethanol solution. Modified, disposable, therapeutic wooden spatulas were used for manipulations. A microscope (magnification of 6-40x) was used in 2 lateral and 2 basal dissections. MR tractography (HARDI-CSD) was carried out in 5 healthy volunteers using a GE Signa HDxt MRI scanner a field strength of 3.0 T.
    Results: We clearly identified the following fascicles: the arcuate fascicle (AF) and superior longitudinal fascicle (SLF) in 6/6 hemispheres on the right and in 5/6 hemispheres on the left, the inferior longitudinal fascicle (ILF) in 3/6 hemispheres on the left and in 4/6 hemispheres on the right, the uncinate fascicle (UF) in 4/4 hemispheres on the left and in 4/4 hemispheres on the right, and the inferior fronto-occipital fascicle (IFOF) in 4/4 hemispheres on the left and in 3/4 hemispheres on the right. Identification was less successful in the case of the frontal aslant tract (FAT) in 1/2 hemispheres on the left and in 0/2 hemispheres on the right. The used technique failed to identify the vertical occipital fascicle (VOF) of Wernicke, a segment of the superior longitudinal fascicle SLF I, and the middle longitudinal fascicle (MdLF). The MR tractography HARDI-CSD data were compared with the dissection data. We described in detail segmentation of the superior longitudinal, arcuate, and inferior fronto-occipital fascicles. Contradictory data were obtained for the superior longitudinal fascicle: a two-segment structure (SLFh and SLFv) was found in most (10/12) specimens, while a three-segment structure was revealed in the other (2/12) specimens (identified SLF II and SLF III). In the arcuate fascicle, the ventral and dorsal segments were successfully identified in 2/12 cases (1 left and 1 right), whereas identification failed in the other cases. During dissection of the inferior fronto-occipital fascicle, we could identify its surface layer in 1 of 8 cases (left) and its deep layer in one more case (left).
    Conclusion: Examination of the long association tracts using the Klingler technique has significant limitations in the fiber intersection areas (sagittal striatum). The frontal aslant tract was least studied; we proposed a special anterior dissection technique for its isolation. The superior longitudinal fascicle can have both the two-segment (10/12) and three-segment (2/12) structure. Investigation of the segmental anatomy of the long association tracts will be continued in further dissections. When planning neurosurgical interventions in the projection areas of the long association tracts, both preoperative HARDI-tractography and anatomical dissections ex vivo, based on the proposed protocols, can be recommended for the operating surgeon to master a three-dimensional picture of the tract topography.

    Abstract Background: Mutations in genes involved in DNA damage repair (DDR), a hallmark of cancer, are associated with increased cancer cell sensitivity to certain therapies. This study sought to evaluate the association of DDR pathogenic variants with treatment efficacy in patients with advanced non-small cell lung cancer (NSCLC).
    Methods: A retrospective cohort of consecutive patients with advanced NSCLC attending a tertiary medical center who underwent next-generation sequencing in 01/2015-8/2020 were clustered according to DDR gene status and compared for overall response rate (ORR), progression-free survival (PFS) (patients receiving systemic therapy), local PFS (patients receiving definitive radiotherapy), and overall survival (OS) using log-rank and Cox regression analyses.
    Results: Of 225 patients with a clear tumor status, 42 had a pathogenic/likely pathogenic DDR variant (pDDR), and 183 had no DDR variant (wtDDR). Overall survival was similar in the two groups (24.2 vs. 23.1 months, p = 0.63). The pDDR group had a higher median local PFS after radiotherapy (median 45 months vs. 9.9 months, respectively; p = 0.044), a higher ORR (88.9% vs. 36.2%, p = 0.04), and a longer median PFS (not reached vs. 6.0 months, p = 0.01) in patients treated with immune checkpoint blockade. There was no difference in ORR, median PFS, and median OS in patients treated with platinum-based chemotherapy.
    Conclusion: Our retrospective data suggest that in patients with stage 4 NSCLC, pathogenic variants in DDR pathway genes may be associated with higher efficacy of radiotherapy and immune checkpoint inhibitors (ICIs). This should be further explored prospectively.

    Abstract Intraerythrocytic growth of the human malaria parasite Plasmodium falciparum requires the catabolism of large amounts of host cell hemoglobin. Endoproteolytic digestion of hemoglobin to short oligopeptides occurs in an acidic organelle called the food vacuole. How amino acids are generated from these peptides is not well understood. To gain insight into this process, we have studied a plasmodial ortholog of the lysosomal exopeptidase cathepsin C. The plasmodial enzyme dipeptidyl aminopeptidase 1 (DPAP1) was enriched from parasite extract by two different approaches and was shown to possess hydrolytic activity against fluorogenic dipeptide substrates. To localize DPAP1 we created a transgenic parasite line expressing a chromosomally encoded DPAP1-green fluorescent protein fusion. Green fluorescent protein fluorescence was observed in the food vacuole of live transgenic parasites, and anti-DPAP1 antibody labeled the food vacuole in parasite cryosections. Together these data implicate DPAP1 in the generation of dipeptides from hemoglobin-derived oligopeptides. To assess the significance of DPAP1, we attempted to ablate DPAP1 activity from blood stage parasites by truncating the chromosomal DPAP1-coding sequence. The inability to disrupt the coding sequence indicates that DPAP1 is important for asexual proliferation. The proenzyme form of DPAP1 was found to accumulate in the parasitophorous vacuole of mature parasites. This observation suggests a trafficking route for DPAP1 through the parasitophorous vacuole to the food vacuole.

    Title translation Long-term monotherapy with prestarium, anap and lomir in patients with mild and moderate arterial hypertension: clinical efficacy, hemodynamics and morphofunctional parameters of heart.
    Abstract Aim: To assess effectiveness of blood hypertension treatment with inhibitors of angiotensin converting enzyme and calcium channels blocker.
    Materials and methods: 83 outpatients with mild and moderate blood hypertension received monotherapy with prestarium, anap and lomir for 12 months. The trends in parameters of blood pressure, total peripheral vascular resistance, thickness of the interventricular septum and left ventricular posterior wall were estimated.
    Results: Long-term effective monotherapy with prestarium, anap and lomir was possible in 47, 62 and 45% of the patients, respectively. Diastolic pressure lowered by 25, 24 and 25%, total peripheral vascular resistance reduced by 20, 19 and 22%, respectively. The three drugs produced similar positive effects on left ventricular thickness, had no negative effects on bioelectric activity of the heart, clinical and biochemical blood counts.
    Conclusion: Prestarium, anap and lomir are effective against hypertension and diminish left ventricular hypertrophy in hypertensive patients.
