    Abstract La Reserva Natural Juan de Dios, ubicada en la Bahía de Málaga en el Pacífico colombiano, es el escenario de una dinámica turística particular en la que se ha configurado el paisaje y se ha construido el territorio sobre las bases del respeto por la naturaleza. El presente artículo da cuenta de la experiencia turística desde sus aristas física, social y ambiental. Dicho acercamiento se hace desde la nueva geografía del turismo, una postura que entiende el turismo como una práctica socioespacial compleja en la cual se enfatiza el componente cultural. Las principales conclusiones giran en torno a la importancia de reivindicar los estudios sobre el turismo desde una perspectiva humanística, así como demostrar la necesidad de abordarlos en mayor número y con enfoques distintos al puramente económico. Del mismo modo, el valor simbólico aparece como la principal característica desde la cual es pertinente trabajar las dinámicas de turismo y conservación del lugar objeto de estudio.

    Abstract Objectives: Describe the perioperative characteristics of the patients undergoing coronary artery bypass grafting (CABG) in San Juan, Veterans Affairs Medical Center (SJ-VAMC). Determine the in-hospital and 30-day morbidity and mortality following CABG and identify adverse predictors for postoperative complications.
    Background: Preoperative risk factors for complications post-CABG have been identified; however limited data is available regarding the value of these predictors in the Puerto Rican Population.
    Methods: Perioperative characteristics and 30-day complications were gathered from all patients undergoing CABG (n = 252) at the SJ-VAMC (2001-2003). Logistic regression analysis was performed to determine factors associated with the occurrence of complications.
    Results: The population's mean age was 66.9 +/- 8.1 years. Associated illnesses included: hypertension (95.6%), diabetes (57%), past smoking (61%), COPD (26%), chronic renal insufficiency (CRI-11.5%), cerebrovascular disease (CVD-20.6%), disabling angina (78%), 3-vessel coronary disease (75.8%), significant left main stenosis (20%), and non-elective surgical intervention (54%). The most frequent primary complications were postoperative myocardial infarction (MI-4.8%) and congestive heart failure (4.8%). The 30-day mortality was 1.2%. Upon multivariate analysis PRBC transfusions > 3 units was associated to increased risks of primary complications, in patients with prior history of CVD. CRI and LVEF < or = 40% were independently associated to development of primary complications, in patients with prior MI and CVD.
    Conclusion: The preoperative characteristics are suggestive of an older and sicker patient that is undergoing CABG at the SJ-VAMC. Despite this, the death rate was low and the frequency of other complications comparable to the Society of Thoracic Surgeons (STS) National Adult Cardiac Database.

    Abstract Purpose: Identify factors that can be used to assess the appropriateness of a thyroid biopsy and propose a pathway to define inappropriate thyroid biopsies in practice.
    Methods: We identified factors utilized in clinical guidelines and existing literature to determine the clinical indications for a thyroid biopsy. Subsequently, we assembled a multidisciplinary panel of experts, including patients, clinicians, researchers, and quality experts, to integrate these factors and develop a pathway for assessing the appropriateness of thyroid biopsies.
    Results: Through literature review and stakeholder engagement, we identified multiple factors to determine if a thyroid biopsy is necessary: ultrasound risk assessment, presence of compressive symptoms and/or clinical suspicion of high-risk thyroid cancer, life expectancy, comorbidity burden, surgical risk, personal risk factors for thyroid cancer, thyroid function levels, local resources and medical expertise and patient values and preferences. We proposed a multiple-tier classification for the appropriateness of thyroid biopsy that begins with ultrasound findings (e.g., size, thyroid cancer risk) and encompasses the evaluation of additional patient-specific factors.
    Conclusion: Assessment of the appropriateness of a thyroid biopsy is possible. Although, thyroid nodule ultrasound risk assessment is a pivotal factor for this assessment, additional factors should be considered (e.g., life expectancy, personal risk factors for thyroid cancer, patient preferences). Yet, additional efforts are needed to operationalize the objective implementation of these factors in clinical practice.

    Abstract The use of cars in cities has many negative impacts, including pollution, noise and the use of space. Yet, detecting factors that reduce the use of cars is a serious challenge, particularly across different regions. Here, we model the use of various modes of transport in a city by aggregating Active mobility (A), Public Transport (B) and Cars (C), expressing the modal share of a city by its ABC triplet. Data for nearly 800 cities across 61 countries is used to model car use and its relationship with city size and income. Our findings suggest that with longer distances and the congestion experienced in large cities, Active mobility and journeys by Car are less frequent, but Public Transport is more prominent. Further, income is strongly related to the use of cars. Results show that a city with twice the income has 37% more journeys by Car. Yet, there are significant differences across regions. For cities in Asia, Public Transport contributes to a substantial share of their journeys. For cities in the US, Canada, Australia, and New Zealand, most of their mobility depends on Cars, regardless of city size. In Europe, there are vast heterogeneities in their modal share, from cities with mostly Active mobility (like Utrecht) to cities where Public Transport is crucial (like Paris or London) and cities where more than two out of three of their journeys are by Car (like Rome and Manchester).
