    Abstract In this essay, I argue that broadly Kantian philosophy – by which I mean philosophy inspired by Kant’s work, but neither dogmatically restricted to Kant’s own texts nor in any way committed to Kant’s own philosophical errors or personal prejudices – is profoundly relevant to the world of today, insofar as it can promote radical enlightenment. Radical enlightenment, in turn, is daring to think, write, speak, and freely act for ourselves, not only as individuals but also collectively via our social institutions, in order to change the world for the better. This should be sharply contrasted with enlightenment lite, or shallow enlightenment, which restricts critical thinking to narrowly-defined, coercively controlled social and political norms, according to Frederick the Great’s famous dictum: “argue as much as you like about whatever you like, but obey!” In effect, then, enlightenment lite or shallow enlightenment is nothing but “free thinking” inside a barbed-wire playpen. My argument has three parts. First, from the standpoint of Kantian radical enlightenment, I critically analyze the early twentieth-century neo-Kantian philosopher Leonard Nelson’s account of Socratic dialogical method. Second, I formulate a broadly Kantian, radically-enlightened conception of philosophical conversation, phildialogues, that corresponds constructively to my critical analysis of Nelson’s account. And third and finally, I argue that contemporary Kantian philosophers not only can but should implement and practice phildialogues, for the betterment of humanity.

    Abstract Background and aims: The role of submucosal endoscopic dissection (ESD) in management of invasive esophageal cancer (EC) remains unclear. In this case series, we evaluate the clinical and technical outcomes of patients who underwent ESD with pathologically staged T1b EC.
    Methods: This retrospective study included patients who underwent ESD between December 2016 and April 2023 with pathologically staged T1b EC. Patient demographics, tumor characteristics, and ESD technical outcomes were analyzed. Patients were followed to determine disease-free survival and tumor recurrence rates.
    Results: Sixteen patients with a total of 17 pathologically staged T1b ECs were included in this case series with a median follow-up time of 28 months [range 3-75]. ESD had high en-bloc (100%) and R0 (82.3%) resection rates. 16/17 patients (94.1%) were discharged the same day, and there were no immediate perioperative complications. 4/17 patients (23.5%) had curative ESD resections with no tumor recurrence. Among those with non-curative resections (n = 13), 5 patients had ESD only, 6 had ESD + surgery, and 2 underwent ESD + chemoradiation. In the ESD only group, 2/5 patients (40%) had tumor recurrence. In the ESD + surgery group, one patient died from a surgical complication, and 1/5 (20%) had tumor recurrence at follow-up. There was no tumor recurrence among patients who had ESD + chemoradiation.
    Conclusion: ESD is safe with high en-bloc and R0 resection rates in T1b EC. Recurrence rates are low but patients need close monitoring. Larger-scale studies are needed to determine the long-term clinical efficacy of ESD in T1b EC.

    Abstract We present the case of a 52-year-old female who presented with a rapidly enlarging right breast mass. An initial breast ultrasound showed a solid mass with cystic components and posterior acoustical enhancement and mammography showed a mass in the same area without calcifications. Biopsy of the lesion revealed spindle cell carcinoma of the breast. To date, the patient has undergone radiation therapy and simple mastectomy and will receive post-operative radiotherapy in the coming months.

    Abstract Experiments reveal that structural transitions in thin sheets are mediated by the passage of transient and stable mobile localized elastic excitations. These "crumples" or "d-cones" nucleate, propagate, interact, annihilate, and escape. Much of the dynamics occurs on millisecond time scales. Nucleation sites correspond to regions where generators of the ideal unstretched surface converge. Additional stable intermediate states illustrate two forms of quasistatic inter-crumple interaction through ridges or valleys. These interactions create pairs from which extended patterns may be constructed in larger specimens. The onset of localized transient deformation with increasing sheet size is correlated with a characteristic stable crumple size, whose measured scaling with thickness is consistent with prior theory and experiment for localized elastic features in thin sheets. We offer a new theoretical justification of this scaling.
