    Abstract Novel drugs have to go through mandatory pre-clinical testing before they can be approved for use in clinical trials. In essence, it is a form of bench-to-bedside (N2B) translational medicine, but the wastage rate of target candidates is immensely high. Effects seen in vitro often do not translate to in vivo human settings. The search is on for better models closer to human physiology to be used in pre-clinical drug screening. The Ex Vivo Metrics system has been introduced where a human organ is harvested and revitalized in a controlled environment suitable for testing of both drug efficacy and potential toxicity. This commentary expresses the author's views regarding this technology of perfused human organs.

    Abstract Objective: This study aimed to translate, culturally adopt and validate a Chinese version of the Disabilities of the Arm, Shoulder and Hand (DASH) for use in patients with upper extremity musculoskeletal diseases in Hong Kong.
    Methods: We followed a standard five-stage process: forward translation, synthesis, backward translation, expert panel review and field-testing to achieve linguistic and conceptual equivalence. The version was officially known as Chinese (Queen Mary Hospital, Hong Kong version) DASH. (Chinese QMH,HK version DASH) (http://www.dash.iwh.on.ca/sites/dash/public/translations/DASH_Chinese_HK_2013.pdf).
    Results: Its internal consistency was then evaluated with 138 participants suffering from upper extremity musculoskeletal conditions. The results were high in DASH-Disability/Symptom module (DASH-DS) (Cronbach alpha 0.97), DASH-Work module (DASH-W) (Cronbach alpha 0.97) and DASH-Sports / Performing Arts module (DASH-SM) (Cronbach alpha 0.99). The test-retest reliability was evaluated with a subgroup of participants who had completed the Chinese (QMH,HK version) DASH on two occasions, with a median interval of 6.5 days. The results were excellent among DASH-DS Intraclass Correlation Coefficient (ICC) = 0.98 and DASH-W (ICC = 0.90). Good test-retest reliability was found in DASH-SM (ICC = 0.89). Construct validity of DASH-DS showed good correlation with the sub-domains of physical functioning (r = -.564) and social functioning (r = -.544) of the Short Form 36 Health Survey (SF-36). Similarly, construct validity of DASH-W also showed good correlation with the sub-domains of physical functioning (r = -.510) and bodily pain (r = -.503) of SF-36.
    Conclusion: The Chinese (Queen Mary Hospital, Hong Kong version) Disabilities of the Arm, Shoulder and Hand is considered as a reliable and valid instrument that can provide a standardised measure of patient-centred outcomes for patients with upper extremity musculoskeletal disorders in Hong Kong.

    Abstract Objective: To report our early experience of laparoscopic nephrectomy.
    Design: Prospective data collection.
    Setting: Queen Mary Hospital, Hong Kong.
    Patients: Transperitoneal laparoscopic nephrectomies were performed on 40 patients between July 1997 and December 2002.
    Main outcome measures: Demographic and perioperative data including operating time, blood loss, postoperative pain score, analgesic requirement, complications, time to resume oral intake, ambulatory state, and length of hospital stay.
    Results: Laparoscopic nephrectomy was performed for 21 solid renal masses, five transitional cell carcinomas, and 14 non-functioning kidneys. Seven (17.5%) patients had previous abdominal surgery. The mean body mass index of the patients was 23.9 kg/m(2) and the mean operating time was 229 minutes. The mean estimated blood loss was 370 mL, and two patients required conversion to open surgery because of intra-operative bleeding. Other complications include diaphragmatic injury, port-site bleeding, chyle leakage, bleeding peptic ulcer, and myocardial ischaemia. The postoperative mean analgesic requirement was 26 mg of morphine sulphate equivalent. The mean time for patients to resume oral diet and full ambulation was 1.3 and 2.8 days, respectively, and the mean length of hospital stay was 6.7 days. The mean diameter of the solid renal tumour was 4.1 cm and the surgical margins of all resected specimen for malignant tumours were negative.
    Conclusion: Laparoscopic nephrectomy is a safe and efficacious approach for resection of benign non-functioning kidneys and malignant renal tumours.

    Abstract Background: Providing informal care for individuals with dementia is frequently a challenging and demanding experience that can have detrimental effects on the psychological well-being of caregivers. Regrettably, community-based caregiver services often prove inadequate, highlighting the necessity for innovative approaches to support caregivers.
    Aim: To test the efficacy of e-bibliotherapy in improving the psychological well-being of informal caregivers of people with dementia.
    Method: The study is divided into two phases. In phase 1, the research team will co-design the e-bibliotherapy app with caregivers. In phase 2, a randomized controlled trial will be conducted among 192 informal caregivers of people with dementia in Hong Kong. Caregivers will be randomly assigned to either the e-bibliotherapy group or the control group using simple randomization. Outcome measures will encompass caregivers' psychological well-being, caregiving appraisal, mental health, saliva cortisol levels as an indicator of stress, and health-related quality of life for caregivers. Data will be collected at baseline, immediately post intervention, and 3 months and 6 months post intervention. General linear mixed model will be employed to analyze intervention effects. Qualitative interviews will be undertaken to explore caregiver experiences within this study and evaluate intervention acceptability using conventional content analysis methods.
    Discussion: This study represents a pioneering effort in utilizing e-bibliotherapy to enhance the psychological well-being of informal caregivers of individuals with dementia, addressing the existing gap in caregiver services and facilitating knowledge dissemination within the community.
    Trial registration: The trial has been registered on ClinicalTrial.gov (Ref: NCT05927805).

    Abstract Health professionals working in oncology face the challenge of a stressful work environment along with impacts of providing care to those suffering from a life-threatening illness and encountering high levels of patient loss. Longitudinal exposure to loss and suffering can lead to grief, which over time can lead to the development of compassion fatigue (CF). Prevalence rates of CF are significant, yet health professionals have little knowledge on the topic. A six-week continuing education program aimed to provide information on CF and support in managing grief and loss and consisted of virtual sessions, case-based learning, and an online community of practice. Content included personal, health system, and team-related risk factors; protective variables associated with CF; grief models; and strategies to help manage grief and loss and to mitigate against CF. Participants also developed personal plans. Pre- and post-course evaluations assessed confidence, knowledge, and overall satisfaction. A total of 189 health professionals completed the program (90% nurses). Reported patient loss was high (58.8% > 10 deaths annually; 12.2% > 50). Improvements in confidence and knowledge across several domains (p < 0.05) related to managing grief and loss were observed, including use of grief assessment tools, risk factors for CF, and strategies to mitigate against CF. Satisfaction level post-program was high. An educational program aiming to improve knowledge of CF and management of grief and loss demonstrated benefit.
