Abstract Background: Insufficient HIV detection and late presentation to antiretroviral therapy (ART) pose significant public health challenges. In China, universal access to HIV testing is available now. Under this background, we aim to analyze the trends of HIV detection and the prevalence of delayed HIV diagnosis (DHD) in order to provide evidence for HIV prevention and treatment in China.
Methods: Data of HIV tests in Hangzhou city between 2007 and 2018 were collected from the Chinese National HIV/AIDS Comprehensive Response Information Management System (CRIMS). Descriptive statistics were used to describe the characteristics of HIV testing and detection and the prevalence of DHD among newly diagnosed HIV cases. Non-parametric tests were employed to examine the prevalence of DHD among newly diagnosed HIV cases. Moreover, logistic regression models were employed to explore the influencing factors of DHD.
Results: Testing rates doubled from 14.1% in 2007-2010 to 28.2% in 2016-2018. The total positive rate of HIV tests was 5.3 per 10,000. Preoperative testing was the predominant pathway for HIV tests, accounting for 41.9%, followed by testing for health screening, maternal examination and other patients, accounting for 18.4%, 13.2% and 11.8%, respectively. Meanwhile, the predominant pathway for HIV case detection was also preoperative testing, accounting for 29.1%, followed by testing for other patients, testing at STD clinics and VCT, with the proportions of 18.8%, 15.8% and 13.6%, respectively. MSM (men who have sex with men) contact was the main transmission route, accounting for 55.3%, followed by heterosexual contact, accounting for 41.6%. Overall, DHD occurred in 29.0% of the newly diagnosed cases, and this rate had not improved over the years. A higher prevalence of DHD was found in those diagnosed through a pre-test for receiving blood/products [OR (95%CI): 5.42(2.95-9.97)], detection of other patients [OR (95%CI): 2.08(1.64-2.63)], preoperative testing [OR (95%CI): 1.83(1.44-2.32)] and spouse or sexual partner testing in positive person [OR (95%CI): 1.93(1.34-2.78)] compared with those diagnosed at a VCT clinic. Heterosexuals [OR (95%CI): 1.20(1.06-1.36)] had a higher risk of DHD than MSM. Diagnosis at a CDC [OR (95%CI): 0.68(0.55-0.83)] and community health centers [OR (95%CI): 0.54(0.39-0.75)] had a lower risk of DHD than in hospitals. Older age, males, being single/divorced or widowed and floating population were also associated with DHD.
Conclusions: In China, DHD had not improved in the last 10 years, although HIV testing had been expanded. Therefore, it is important for continued efforts to promote early diagnosis of HIV to prevent transmission, morbidity and early mortality in HIV infection.

Abstract Innovation is the primary driving force for development, and green innovation efficiency (GIE) plays a key role in regional sustainable development. Moreover, environmental regulations (ERs) are also crucial for innovation and green transformation. Considering the heterogeneity of ERs, we assess the dynamic GIE in the industrial sectors of China. We detect their spatial clustering characteristics, and distinguish the impacts of ERs. Results suggest that there exist significant differences in GIE. Provinces such as Hainan, Guangdong and Zhejiang are ranked high, while Gansu, Inner Mongolia and Ningxia are ranked at the bottom, which shows some spatial dependence. The relationship between the administrative regulation and GIE demonstrates a U-shape, and has not reached a critical point, whereas the relationship between the market-based regulation and GIE possesses an inverted U-shape, which is highly significant. Furthermore, a positive linear relationship exists between the lagged public participation regulation and GIE. This paper also proposes that the economic development level and industrial structure are vital factors in accelerating industrial GIE. These conclusions provide scientific support for formulating regional transformation strategies.

Abstract Objective: To predict the anti-inflammatory targets and related pathways of rhein in the treatment of asthma by using network pharmacology, and to further explore its potential mechanism in asthma.
Methods: The corresponding targets of rhein were obtained from the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), and the rhein-target network was constructed with Cytoscape 3.7.1 software. The Genbank and Drugbank databases were used to collect and screen asthma targets, and the rhein-target-disease interaction network was constructed. A target protein-protein interaction (PPI) network was constructed using the STRING database to screen key targets. Finally, Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis was used to identify biological processes and signaling pathways. The anti-asthmatic effects of rhein were tested in vitro, and the expression levels of proteins in the mitogen-activated protein kinase/nuclear factor kappa-B (MAPK/ NF-κB) signaling pathway were assessed by western blot analysis.
Results: Altogether, 83 targets of rhein were screened in the relevant databases, 989 targets of asthma were obtained in the National Center for Biotechnology Information (NCBI) GENE Database. PPI network analysis and KEGG pathway enrichment analysis predicted that rhein could regulate the epidermal active growth factor receptor (EGFR), mitogen-activated protein kinase 14 (MAPK14), tumour necrosis factor receptor superfamily member 1A (TNFRSF1A), receptor tyrosine-protein kinase erbB-2 (ERBB2), and other signaling pathways. Furthermore, we selected the MAPK signaling pathway to determine the anti-inflammatory effects of rhein. Consistently, further experiments demonstrated that rhein was shown to inhibit HBE cells inflammation.
Conclusion: The anti-inflammatory mechanism of rhein in the treatment of asthma may be related to EGFR, MAPK14, TNFRSF1A and ERBB2 as well as their signaling pathways. To prevent the exacerbation of asthma, instead of targeting a single pathway or a single target, all these targets and their signaling pathways should be controlled holistically. Rhein may alleviate the inflammation of asthma by inhibiting the MAPK/NF-κB pathway.

Abstract Operational optimal control (OOC) is an essential component of wastewater treatment process (WWTP). The control variables usually are high-dimensional, nonlinear, and strongly coupled, which can easily fail traditional optimization control methods. Mathematically, these operational variables usually are in the unknown low-dimensional space embedded in the high-dimensional space. Therefore, the OOC problem of WWTP can be resolved as an optimization challenge involving low-dimensional space, and the unknown low-dimensional space is presented in the form of a set of controlled variables in a high-dimensional space, which is normal in real-world industries. Here, a dimension-reducible data-driven optimization control framework for WWTP is proposed. Considering the difficulty in elucidating the whole space of set points, a neural network is designed to approximate the constraint relationship between control variables. The search process is based on optimization methods in low-dimensional space embedded into Euclidean spaces. Furthermore, the convergence of the process is ensured via mathematical analysis. Finally, the experimental simulation of wastewater treatment revealed that this approach is effective for an optimal solution in control systems.
