    Abstract Individual acknowledgment of sexual assault and rape perpetration is extraordinarily low in prior research. Only about 1% of individuals report perpetrating rape, in contrast to the 6% perpetrating rape as estimated by using behaviorally specific items that exclude stigmatized words such as rape. The goal of this study was to examine two possible measurement mechanisms for increasing perpetration acknowledgment: label choice and response format. In Sample 1 (N = 291), participants completed two acknowledgment items which varied in label choice. One item used the term rape; one used the term sexual assault. Acknowledgment of perpetration using the label sexual assault was significantly higher than when using the term rape (6.38%-1.71%, p = .01, Cohen's d = 0.44). In Sample 2 (N = 438), participants were presented with a scaled and a dichotomous sexual assault item at different parts of the overall survey. Sexual assault acknowledgment was higher on the scaled item compared to the dichotomous item (15.75% vs. 3.2%, p < .0001, Cohen's d = 0.64). Rates of sexual perpetration as measured behaviorally were higher for ambiguous acknowledgment types ("might or might not," "probably not") than for those reporting "definitely not," (76.81% vs. 29.0%, p < .0001, Cohen's d = 0.59). The two different measurement strategies tested here, using a less stigmatized label such as sexual assault and using a scaled response format, both increased rates of perpetration acknowledgment 3-15x greater than rates documented in prior research.

    Abstract Background: Neonatal unilateral vocal fold paralysis may arise iatrogenically, idiopathically, or in the context of an underlying neurologic disorder. Management is often supportive, focusing on diet modification to allow for safe oral feeding. We describe the clinical course of six infants with unilateral vocal fold paralysis who developed predominantly unilateral laryngomalacia ipsilateral to the affected vocal fold with associated severe respiratory symptoms and feeding difficulty.
    Methods: Retrospective review of six infants with unilateral vocal fold paralysis and predominantly unilateral laryngomalacia. Charts were reviewed for etiology of vocal fold paralysis, presenting symptoms, operative details, postoperative course, and outcomes for breathing and swallowing.
    Results: Etiology of vocal fold paralysis included cardiac surgery in four patients, intubation-related in one, and idiopathic in one. Presenting symptoms included increased work of breathing, stridor, feeding difficulty, respiratory failure requiring noninvasive respiratory support, and weak cry. All infants were on nasogastric tube feedings. Direct microlaryngoscopy with unilateral or predominantly unilateral (conservative contralateral aryepiglottic fold division) supraglottoplasty was performed. Stridor and work of breathing improved in all six patients within 1 week postoperatively. Oral feeding improved in three patients within 2 weeks. Three patients had persistent feeding impairment with improvement within one year.
    Conclusions: Predominantly unilateral laryngomalacia may arise in the context of unilateral vocal fold paralysis. Addressing the ipsilateral cuneiform collapse can improve breathing and feeding. This may be an under-described phenomenon and represents an additional reason to include the otolaryngologist early in the care of infants with suspected possible new unilateral vocal fold paralysis. Breathing and swallow can improve post-operatively, but feeding may remain limited by the vocal fold paralysis and any medical comorbidities. Ongoing follow-up and collaboration with speech-language pathology to optimize feeding are important.

    Abstract While molecular detection has increasingly become the detection method of choice for infectious diseases, antibody detection remains an important approach for diagnosis and surveillance. For henipaviruses, antibody detection methods such as ELISA and Western blot played a key role in the initial discovery of bats as the natural reservoir host. Here, we will describe three additional antibody detection methods (LIPS, Luminex, and pseudovirus systems), which can be used in most BSL2 laboratories without the need for live virus and a high containment BSL4 facility.

    Abstract Previous research has demonstrated the dramatic effect trauma can have on religiosity. This study sought to extend this understanding by examining how a specific trauma, rape, influences religious beliefs and behaviors as well as how religiosity influences acknowledgment of rape. Rape acknowledgment is the personal use of the label rape to describe such an experience. The process of acknowledgment and general recovery from rape can include dramatic questioning of one's religious beliefs and marks an important potential point of intervention, especially given the majority of the US identifies as religious. A sample of 310 mostly Christian, college-aged women completed questionnaires about their personal religiosity, attitudes and beliefs, and trauma history. Results indicated those who experienced rape experienced significantly greater change in their religious beliefs compared to those who had experienced other types of traumas (

    Abstract Inequity in assessment has been described as a "wicked problem"-an issue with complex roots, inherent tensions, and unclear solutions. To address inequity, health professions educators must critically examine their implicit understandings of truth and knowledge (i.e., their epistemologies) with regard to educational assessment before jumping to solutions. The authors use the analogy of a ship (program of assessment) sailing on different seas (epistemologies) to describe their journey in seeking to improve equity in assessment. Should the education community repair the ship of assessment while sailing or should the ship be scrapped and built anew? The authors share a case study of a well-developed internal medicine residency program of assessment and describe efforts to evaluate and enable equity using various epistemological lenses. They first used a postpositivist lens to evaluate if the systems and strategies aligned with best practices, but found they did not capture important nuances of what equitable assessment entails. Next, they used a constructivist approach to improve stakeholder engagement, but found they still failed to question the inequitable assumptions inherent to their systems and strategies. Finally, they describe a shift to critical epistemologies, seeking to understand who experiences inequity and harm to dismantle inequitable systems and create better ones. The authors describe how each unique sea promoted different adaptations to their ship, and challenge programs to sail through new epistemological waters as a starting point for making their own ships more equitable.

    Abstract In the last two decades, several high impact zoonotic disease outbreaks have been linked to bat-borne viruses. These include SARS coronavirus, Hendra virus and Nipah virus. In addition, it has been suspected that ebolaviruses and MERS coronavirus are also linked to bats. It is being increasingly accepted that bats are potential reservoirs of a large number of known and unknown viruses, many of which could spillover into animal and human populations. However, our knowledge into basic bat biology and immunology is very limited and we have little understanding of major factors contributing to the risk of bat virus spillover events. Here we provide a brief review of the latest findings in bat viruses and their potential risk of cross-species transmission.
