    Abstract Infertility is a global issue of concern in the modern era, affecting 8%–12% of couples globally. Although the etiology of infertility is varied and difficult to determine, a good proportion of cases can be attributed to male. The most common cause of male infertility is oligozoospermia. Oligozoospermia is a decrease in sperm concentration below 15 million sperm/ml in semen ejaculate. Shukra Kshaya is a similar clinical condition mentioned in Ayurveda in which the Shukra Dhatu is diminished both qualitatively and quantitatively. Ayurveda has specified many therapeutic interventions and practices, including the Panchakarma procedure, Basti Karma, for the strengthening of Shukra Dhatu as well as the management of Shukra-Dusht. Mustadi Yapana Basti (MYB) was chosen for this study because of its significant Vatahara, Rasayana, and Vajikarana characteristics. A total of 05 patients who were diagnosed with oligozoospermia were managed with MYB for 15 consecutive days without any internal medications. The Shukra Kshaya assessment questionnaire was used in conjunction with objective parameters to determine the improvement. Data were collected twice before and after therapy. After the treatment, all five participants exhibited significant improvement in sperm count from a mean of 10.4–55. In this case series, all the patients demonstrated clinical improvement in symptoms of Shukra kshaya as well as in associated laboratory parameters. Basti treatment significantly improved all symptoms of Shukra Kshaya, as well as semen parameters (sperm count, sperm active motility, and semen volume) and serum testosterone. The reported cases demonstrate that MYB is a therapeutic option for the comprehensive care of men with a diagnosis of oligozoospermia.

    Abstract Germ cell tumors (GCTs) are associated with pure gonadal dysgenesis or Swyer syndrome. Swyer syndrome usually presents with primary amenorrhea, streak ovaries, and mixed GCT. However, our patient presented with secondary amenorrhea, normal female external genitalia, and a mixed GCT. Constitutional karyotype was suggestive of 46,XY. Management comprised chemotherapy, followed by surgery. Histopathology was suggestive of dysgerminoma complicating a gonadoblastoma. The purpose of reporting this case is its rarity and the importance of diagnosing an XY karyotype, as the incidence of GCTs is higher in these patients.

    Abstract Introduction: In this current era of the pandemic, when the Covid-19 pandemic rapidly spread across the world, the population at a large scale suffered from fear, worry, and certain concern regarding himself/herself as well as for their families especially adults and children. A significant level of stress also developed in health care providers. Many people all over the world are suffering from loneliness, anxiety, and disturbed mental health issues during lockdown, quarantines, and isolation. Mental health is one of the important factors for overall health. No vaccination or treatment available for COVID-19 till today. The only approach is the preventive and promotive approach. Psychological issues also hamper our physical health and level of immunity. Objective: In this study, an attempt has been made to critically review the role of yoga against the current pandemic. Data Source and Review Methods: References are searched from various authoritative texts and worldwide accepted scientific databases concerning the effect of yoga in the different body systems, immune system as well as on mental health. Result: The psychophysical approach which helps in reducing the mental issue maintains mental health and reduces stress as well as boosts our immunity against communicable diseases like COVID-19 is Yoga. Discussion: Yoga is cost-effective, may be practiced as a self-care treatment against physical and mental health issues, enhance self-efficacy and self-confidence. Yoga may be effective as a supportive adjunct in some medical conditions. Conclusion: This review summarizes the effects of yoga interventions against the current pandemic as well as helps to understand the preventive and health promotive aspect of yoga on various components of mental and physical health.

    Abstract The unforgiving pace and complexity of modern life has greatly challenged our ability to live healthier and fully in the present moment. Industrialization, globalization, and competition in each sector lead to emotional stress and strain in life which is dangerous at the physical and mental levels. As per the classics of Ayurveda, chinta (stress) and atichintan (overthinking) are the causes of Rasavaha srotodushti which lead to many diseases. Shirodhara is an important healing technique of Ayurveda that has neuro-immuno-physio-psychological effects on the human body. Several studies have been carried out to evaluate the efficacy of Shirodhara in many diseases. A 35-yearold female patient with sleep deprivation, loss of concentration, and irritable mood symptoms was admitted to the hospital. She was assessed on the basis of the Profile Of Mood Score (POMS) questionnaire, Serum Cortisol (Sr. Cortisol), Dehydroepiandrosterone (DHEA). The patient was treated by Shirodhara with sesame oil for 14 days continuously. Results were assessed by biomarkers of stress and by POMS score. At the end of Shiroadhara, there was significant improvement found in presenting complaints as well as on the POMS Score and Stress biomarkers. During or after the treatment, no adverse events were observed.

    Abstract Background: Coronavirus disease 2019 (COVID-19) infection is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), a single-stranded ribonucleic acid (RNA) β-coronavirus. Prolonged duration of symptoms, ill health, disability, and need for hospitalisation are all well-known features of severe COVID-19 disease.
    Objective: To describe the epidemiological, clinical and imaging characteristics of hospitalised patients of COVID-19 who required prolonged oxygen therapy after testing negative for SARS-CoV-2 and attempt to determine the associated factors leading to delayed recovery, failure to wean, and mortality.
    Material and method: Prospective observational study from 9th September to 6th November 2020 in a tertiary care COVID hospital of Jharkhand. Included COVID-19-infected patients requiring oxygen to maintain a saturation of ≥95% after testing reverse transcription polymerase chain reaction (RT-PCR) negative. Patients were classified as Group I, those who could be weaned off oxygen, and Group II, those who could not be weaned off oxygen during their stay in the isolation ward. A detailed assessment for outcome in these two groups related to age, gender, presence or absence of co-morbidities, nature of co-morbidities and findings of high-resolution CT (HRCT) thorax was done to ascertain risk factors for failure to wean and adverse outcomes.
    Results: During the study period, 93 patients suffering from moderate to severe COVID-19 infection, could not be discharged from the hospital and were admitted to the post-COVID isolation ward after testing RT-PCR negative, due to breathlessness and need for oxygen therapy, with a male predominance, M:F ratio of 2.2:1. Of these 93 patients, 51 could be weaned off oxygen in the isolation ward. The mean and median age of patients who could be successfully weaned was 58.5±14.3 years and 60 years respectively, compared to a mean age of 64±12.4 years and a median age of 67 years for patients who could not be weaned off oxygen during the isolation period. Patients aged ≥60 years were at risk for prolonged requirement of oxygen compared to those <50 years of age, relative risk (RR) 1.43 (95%CI 0.9-2, p=0.051). Failure to wean in <50 years was noted in presence of co-morbidities, RR 4 (95%CI 1.5-10.6, p=0.005). Multivariable logistic regression analysis calculated an odds ratio (OR) of 12.22 (95%CI 2.4-61.5, p<0.002) in patients of coronary artery disease (CAD), and 3.34 (95%CI 1.01-10.9, p<0.046) in patients of diabetes, for failure to wean with delayed recovery in patients aged 50 years and more, having multiple co-morbidities. Presence of ≥three co-morbid conditions was associated with increased risk of critical care unit (CCU) admissions (RR 2.1, p=0.02), failure to wean (RR 1.79, p<0.006), and death (p=0.02). Elderly male patients (mean age of 81.3±7.2years) with CAD and multiple comorbidities were at a high risk of mortality (p=0.01).
    Conclusion:  Patients ≥50 years of age having ≥three co-morbidities are at increased risk of prolonged hospitalisation and oxygen therapy in moderate to severe COVID-19 infection, precluding their discharge even after they test negative for SARS-CoV-2. Elderly male patients of COVID-19 with CAD and multiple comorbidities are at a high risk of mortality.
