    Abstract Primary ovarian insufficiency (POI) refers to the loss of ovarian function in young women. It can lead to primary or secondary amenorrhea. The loss of ovarian function is usually permanent akin to menopause. Resultant hypoestrogenemia has systemic adverse effects particularly on the bone health. POI is diagnosed on the basis of raised serum follicle stimulating hormones levels. Most of the cases are idiopathic. Hypothyroidism is the most common associated disorder suggesting autoimmune etiology in some of the cases. Karyotypic abnormalities (mostly monosomy X-Turner syndrome) should be excluded in cases presenting with primary amenorrhea or early onset secondary amenorrhea. Treatment involves long-term estrogen-progesterone replacement therapy which provides regular withdrawal bleed and prevents systemic effects of hypoestrogenemia.

    Abstract High-risk obstetric patients have chances of deterioration which can be detected by any early warning score. This study was aimed to assess the suitability of the Obstetrics National Early Warning System (ONEWS) for the pregnant women. This prospective study was conducted on 500 high-risk pregnant women attending a tertiary care teaching hospital. The ONEWS charts were plotted for each of them. The primary outcome measure was composite adverse maternal outcome (CAMO) in the form of one or more among mortality, severe maternal morbidity and intensive care unit admissions. Of the 500 women who participated, 200 (40%) had a score ≥3 (triggered an intervention). The CAMO among the triggered group [59.5% (n=119)] was significantly higher compared to that in the non-triggered group [13.3% (n=40) (P=0.001)]. The area under the receiver operating characteristic curve was 0.800 (95% confidence interval 0.752-0.847). The sensitivity of the ONEWS in predicting CAMO was 74.8 per cent, specificity 76.2 per cent, positive predictive value 59.5 per cent and negative predictive value 86.7 per cent at a cut-off score of 3. ONEWS appears to be a useful tool for predicting adverse maternal outcomes in high-risk pregnant women.

    Abstract Background: Abdominal fibromatosis presenting as an abdominal mass is a rare occurrence in a gynecological setting. The diagnosis might not be suspected preoperatively, and management could be challenging.
    Case: An 18-year-old girl with ovarian germ cell malignancy was managed with staging laparotomy and chemotherapy. She developed a large abdominal mass 10 months later. An abdominopelvic mass (40 × 20 cm) was removed at laparotomy. The histopathological diagnosis was intra-abdominal fibromatosis. A rapidly growing abdominal mass recurred postoperatively. After discussion with the tumor board, chemotherapy with methotrexate and vinblastine was started. The tumor was not responsive, and the patient succumbed after the second cycle of chemotherapy.
    Summary and conclusion: Abdominal fibromatosis should be considered among the differential diagnoses of a rapidly growing abdominal mass after resection of an ovarian germ cell tumor.

    Abstract Background: Parkinson's disease (PD) is a slowly progressive and disabling disorder, so the cost of illness may change with time. We aimed this study to know the annual cost of care of PD in India.
    Methods: After ethics approval, a prospective cohort study was conducted at the movement disorder clinic of tertiary care hospital for 2 years (2014-2016). The outcomes were a description of the total annual direct cost of Parkinson's disease including health care as well as non-health care cost. We also did correlation analysis to know the determinants of the total cost.
    Results: A total of 200 consecutive patients of PD with 141 (70.50%) males and 59 (29.50%) females with a mean age of 56.84 ± 10.51 years were enrolled. The annual Median Direct cost of care was INR 27,315.0 (IQR 13636.6-44908.4), whereas the Indirect cost was INR 21,400 (IQR 9800 - 96800). Cost on drugs (Direct health care) formed 68.50% (Median) of the total Direct cost. Total direct cost formed 11.38% of the Median total yearly income of our patients. Of the direct cost, the Median expenditure on drugs was INR 18,712.8 (8064.0 -30696.0). Only 5% of patients had health care insurance. The total direct cost was determined by the stage of Parkinson's disease and duration of disease (
    Conclusion: Annual cost of care of Parkinson's disease is high and increases with the duration of the disease as well as the progression of the disease.
