    Abstract Background: We investigated the effects of a 12-week home-based exercise program delivered with virtual or minimal supervision on the physical and cognitive function of community-dwelling older adults in the context of the COVID-19 pandemic in Brazil.
    Methods: The study was registered on the Brazilian Registry of Clinical Trials platform (code: RBR-8qby2wt). Thirty-eight older adults (81% female and 68±7 years old), non-disabled, and without cognitive impairment or dementia, were randomly assigned to a 12-week home-based exercise program: 1) virtual supervision (classes remotely delivered through video conference by trained staff), or 2) minimal supervision (once-weekly contact to touch base through standardized text messages). The participants initially performed two sets of 10 repetitions three times a week, with a 60-second interval. The volume and complexity of the exercises were progressively increased. (e.g., the number of sets increased to 3 and later to 4). At baseline and follow-up, we collected remote measurements of physical function (muscle strength and power, functional muscular fitness) and cognition (processing speed, inhibitory control, verbal fluency).
    Results: Participants in the minimal supervision home-based exercise group significantly improved the Stroop test (-1.6 sec, 95% CI = -3.20; -0.09). No significant between-group differences were observed for physical and cognitive outcomes.
    Conclusion: A home-based exercise program delivered with virtual or minimal supervision can produce similar effects, and may help to maintain physical and cognitive capabilities among healthy, high-functioning older adults who experienced mobility restrictions due to the COVID-19 pandemic in Brazil.

    Abstract Purpose of review: With an aging cardiac surgery population, prefrail and frail patients are becoming more common. Anesthesiologists will be faced with the decision of how best to provide care to frail patients. Identification, management, and outcomes in frail patients will be discussed in this review.
    Recent findings: Frailty is associated with a variety of poor outcomes, such as increased hospital length of stay, medical resource utilization, readmission rates, and mortality. Prehabilitation may play a greater role in the management of frail cardiac surgery patients.
    Summary: As frailty will likely only increase amongst cardiac surgery patients, it is important to develop multicenter trials to study management and treatment options. Until those studies are performed, the care of frail cardiac surgery patients may be best provided by high-volume surgical centers with expertise in the management of frail patients.

    Abstract NFL is one of the most representative phenomena of the contemporary American culture. Brazil has the second biggest international audience of the league in the world, and part of it is engaged to social networks during game broadcasts. Since technological advancements and social networks have been featured as great acculturation catalysts and Brazil appears as quite susceptible to dominating cultures, present research aims to analyze how the virtual interaction of Brazilian NFL fans during league-game broadcasts reveal a consumption acculturation process. We adopted the netnography approach as research method, which was based on monitoring Twitter hashtags launched by ESPN channels in Brazil during three seasons, between 2016 and 2019. Results reveal three practices of acculturation: comparison between Brazilian and American features, embodiment of American rituals and routine changes to adjust to the schedule of the league.

    Abstract Introduction: The benefits of taking up physical activity are well established and social support has been identified as one of the main determinants of this behavior.
    Objectives: To investigate the association between social support and weekly frequency of physical activity in adults working at a public university in the state of Rio de Janeiro, Brazil.
    Methods: This is a cross-sectional population study with a convenience sample of 189 contract workers of both sexes, aged from 21 to 72 years (39.00 ± 11.43). The instruments employed were the short version of the International Physical Activity Questionnaire and the Social Support for Physical Activities Scale. Fisher's exact test was used to estimate the distribution of physical activity frequency. Poisson regression was used for association analyses. The significance level was set at 5%.
    Results: A significant association was detected between social support and weekly physical activity frequency (p < 0.05). Social support for physical activity of moderate or vigorous intensity was associated with both weekly frequency of walking (odds ratio [OR]: 1.32; 95% confidence interval [95%CI]: 1.11-1.58) and weekly frequency of vigorous physical activity (OR: 1.34; 95%CI: 1.08-1.67). Additionally, people who reported receiving social support for walking were more likely to have increased weekly frequency of walking (OR: 1.22; 95%CI: 1.00-1.49).
    Conclusions: Social support for physical activity from relatives and friends is associated with weekly physical activity frequency. Notwithstanding, this association was stronger for weekly frequency of vigorous intensity physical activity.

    Abstract Objective: We aimed to determine the feasibility and reliability of videoconference assessment of functional and cognitive status among older adults in the context of the COVID-19 pandemic. Methods: Thirty community-dwelling older adults (86.70% women) with a mean age of 69.77 (SD = 6.60) years who were physically independent and had no signs of cognitive impairment were included in the sample. An independent and experienced researcher assessed functional (chair rise test, chair stand test, sitting and rising test) and cognitive (Montreal Cognitive Assessment, parts A and B of the Trail Making Test, the Stroop test, the verbal fluency test) performance in real-time on the Google Meet platform on 2 non-consecutive days. The reliability of the measures was analyzed using the intraclass correlation coefficient (ICC), a paired t-test, or Wilcoxon and Bland-Altman analysis. The feasibility of the assessment was investigated using a standardized 14-item questionnaire. Results: All functional performance measures showed excellent intra-rater reliability, with ICCs from 0.90 (95%CI 0.78 - 0.95) for the sitting and rising test to 0.98 (95%CI 0.96 - 0.99) for the chair rise test. Our analysis also showed mixed levels of reliability across measures, including good ICC (ranging from 0.79 - 0.91) for the Montreal Cognitive Assessment, part B of the Trail Making Test, and the congruent and neutral trials in the Stroop test, but poor-to-moderate ICC (ranging from 0.42 - 0.58) for the other cognitive assessments. In general, the participants reported good feasibility for the assessment format. Conclusion: In healthy and highly educated older adults, videoconferencing is a feasible method of determining functional and cognitive performance. Functional measures showed excellent reliability indexes, whereas cognitive data should be interpreted carefully, since the reliability varied from poor to moderate.

    Abstract The study aimed to investigate the perception of doctors about the different hiring methods and their influence on the performance of the PHC essential attributes to analyze the formats that best contribute to its implementation and strengthening. This is a quantitative, cross-sectional research using a semi-structured form with 268 doctors from the ESF in Fortaleza, Ceará, Brazil. A unique sociodemographic profile was identified for each group evaluated, influencing the work process and opinions about hiring formats. The development of the essential PHC attributes was positively evaluated, but different perceptions were observed by professional hiring method assessed. The work performed by doctors in the ESF is influenced by how they are hired (p<0.001). Better performance of the statutory (4.4) was noted, followed by scholarship holders of the Mais Médicos Program/Primary Care Valorization Program (3.7), Consolidated Labor Laws (3.5), and, finally, those working with Self-Employed Payment Receipt (RPA) (2.4). We analyzed that hiring through the Brazilian Statutory Regime and RPA are, respectively, the best (85%) and the worst (96.6%) hiring formats.

    Abstract Objetivo: identificar o padrão espaço-temporal da mortalidade por tuberculose e seus fatores a ela relacionados. Método: estudo ecológico, usando como unidade de análise os municípios do estado do Ceará, Brasil, durante o período de 2001 a 2017. A mortalidade por tuberculose foi analisada por técnicas de análise temporal e espacial. Resultados: no período, foram notificados 1.513 óbitos por tuberculose. Detectou-se aumento anual médio de 15% na mortalidade (Intervalo de Confiança 95%: 6,2 – 24,6). Os indicadores que mais influenciaram a mortalidade foram: esperança de vida ao nascer (β=3,38), domicílios com abastecimento de água e esgotamento sanitário inadequados (β=-0,01) e probabilidade de sobrevivência até os 60 anos (β=-2,26). Conclusão: este estudo evidenciou o aumento no padrão temporal da mortalidade por tuberculose ao longo dos anos. Deve-se, portanto, enfatizar estratégias de cuidado voltadas à adesão ao tratamento e de saúde pública voltadas à melhoria do ambiente da população.
