    Abstract The genus Amanita has been divided into two monophyletic taxa, Amanita, an ectomycorrhizal genus, and Aspidella, a saprotrophic genus. The controversies and histories about recognition of the two genera based on trophic status are discussed. The name Aspidella E.-J. Gilbert is shown to be illegitimate and a later homonym of Aspidella E. Billings, a well-known generic name for an enigmatic fossil sometimes classified as a fungus or alga. The name Saproamanita is coined to replace Aspidella E.-J. Gilbert for the saprotrophic Amanitas, and a selection of previously molecularly analyzed species and closely classified grassland species are transferred to it along with selected similar taxa. The type illustration for the type species, S. vittadinii, is explained and a subgeneric classification accepting Amanita subgen. Amanitina and subgen. Amanita is proposed. Validation of the family name, Amanitaceae E.-J. Gilbert dating from 1940, rather than by Pouzar in 1983 is explained.

    Abstract The production of J/psi mesons in continuum e(+)e(-) annihilations has been studied with the BABAR detector at energies near the Upsilon(4S) resonance. The mesons are distinguished from J/psi production in B decays through their center-of-mass momentum and energy. We measure the cross section e(+)e(-)-->J/psi X to be 2.52+/-0.21+/-0.21 pb. We set a 90% C.L. upper limit on the branching fraction for direct Upsilon(4S)-->J/psi X decays at 4.7 x 10(-4).

    Abstract Background: Clinically lymph node-positive (cN+) prostate cancer (PCa) is an often-fatal disease. Its optimal management remains largely undefined given a lack of prospective, randomized data to inform practice. We sought to describe modern practice patterns in the management of cN+PCa and assess the effect of adding radiation therapy (RT) to androgen deprivation therapy (ADT) on survival using the National Cancer Data Base.
    Methods: Patients with cN+PCa and without distant metastases diagnosed between 2004 and 2011 were included. Five-year overall survival for patients diagnosed between 2004 and 2006 and treated with ADT alone or ADT+RT were compared. Propensity score (PS) matching was used to balance baseline characteristics, and Cox multivariate regression analysis was used to estimate hazard ratios (HRs) for all-cause mortality.
    Results: Of 3,540 total patients, 32.2% were treated with ADT alone and 51.4% received ADT+RT. Compared with ADT alone, patients treated with ADT+RT were younger and more likely to have private insurance, lower comorbidity scores, higher Gleason scores, and lower PSA values. After PS matching, 318 patients remained in each group. Compared with ADT alone, ADT+RT was associated with a 50% decreased risk of five-year all-cause mortality (HR = 0.50, 95% CI: 0.37-0.67, two-sided P<0.001; crude OS rate: 71.5% vs. 53.2%).
    Conclusions: Using a large national database, we have identified a statistically significant survival benefit for patients with cN+PCa treated with ADT+RT. These data, if appropriately validated by randomized trials, suggest that a substantial proportion of such patients at high risk for prostate cancer death may be undertreated, warranting a reevaluation of current practice guidelines.

    Abstract Purpose: Magnetic resonance imaging detects extracapsular extension by prostate cancer with excellent specificity but low sensitivity. This limits surgical planning, which could be modified to account for focal extracapsular extension with image directed guidance for wider excision. In this study, we evaluate the performance of multiparametric magnetic resonance imaging in extracapsular extension detection and determine which preoperative variables predict extracapsular extension on final pathology when multiparametric magnetic resonance imaging predicts organ confined disease.
    Materials and methods: From May 2007 to March 2014, 169 patients underwent pre-biopsy multiparametric magnetic resonance imaging, magnetic resonance imaging/transrectal ultrasound fusion guided biopsy, extended sextant 12-core biopsy, and radical prostatectomy at our institution. A subset of 116 men had multiparametric magnetic resonance imaging negative for extracapsular extension and were included in the final analysis.
    Results: The 116 men with multiparametric magnetic resonance imaging negative for extracapsular extension had a median age of 61 years (IQR: 57-66) and a median prostate specific antigen of 5.51 ng/ml (IQR: 3.91-9.07). The prevalence of extracapsular extension was 23.1% in the overall population. Sensitivity, specificity, and positive and negative predictive values of multiparametric magnetic resonance imaging for extracapsular extension were 48.7%, 73.9%, and 35.9% and 82.8%, respectively. On multivariate regression analysis, only patient age (P = 0.002) and magnetic resonance imaging/transrectal ultrasound fusion guided biopsy Gleason score (P = 0.032) were independent predictors of extracapsular extension on final radical prostatectomy pathology.
    Conclusions: Because of the low sensitivity of multiparametric magnetic resonance imaging for extracapsular extension, further tools are necessary to stratify men at risk for occult extracapsular extension that would otherwise only become apparent on final pathology. Magnetic resonance imaging/transrectal ultrasound fusion guided biopsy Gleason score can help identify which men with prostate cancer have extracapsular extension that may not be detectable by imaging.
