    Abstract Objective: To determine whether proton pump inhibitor (PPI) exposure is associated with an increased risk of developing eosinophilic esophagitis (EoE) in children with esophageal atresia (EA).
    Study design: A retrospective chart review of children with EA from January 1, 2005 to December 31, 2020 was undertaken at Sydney Children's Hospital Randwick. Children with EA and EoE (cases) were matched (1:2) to children with only EA (controls) to compare PPI exposure. Other early-life factors such as infantile antibiotic exposure and personal or family history of atopy were also analyzed using simple and multivariable logistic regression.
    Results: Of 184 children with EA, 46 (25%) developed EoE during this period. Thirty-eight EoE participants were matched to 76 controls. Children with EoE and EA received PPI for significantly higher durations (p = .018) and at significantly higher cumulative doses (p = .017) than controls. Food allergy (adjusted odds ratio [aOR], 7.317; 95% confidence interval [CI], 2.244-23.742), family history of atopy (aOR, 3.504; 95% CI, 1.268-9.682), and infantile antibiotic exposure (aOR, 1.040; 95% CI, 1.006-1.075) were also significantly associated with an increased risk of developing EoE in the EA cohort.
    Conclusions: Prolonged duration and high cumulative dose of PPI exposure were significantly associated with subsequent EoE development in children with EA. Food allergy, family history of atopy, and infantile antibiotic exposure in EA were also significantly associated with an increased risk of EoE development.

    Abstract Endocannabinoids are derived from dietary omega-3 and omega-6 fatty acids and play an important role in regulation of inflammation, development, neurodegenerative diseases, cancer, and cardiovascular diseases. They elicit this effect via interactions with cannabinoid receptors 1 and 2 which are also targeted by plant derived cannabinoid from cannabis. The evidence of the involvement of the endocannabinoid system in cardiopulmonary function comes from studies that show that cannabis consumption leads to cardiovascular effect such as arrythmia and is beneficial in lung cancer patients. Moreover, omega-3 and omega-6 endocannabinoids play several important roles in cardiopulmonary system such as causing airway relaxation, suppressing atherosclerosis and hypertension. These effects are mediated via the cannabinoids receptors that are abundant in the cardiopulmonary system. Overall, this chapter reviews the known role of phytocannabinoids and endocannabinoids in the cardiopulmonary context.

    Abstract Two studies were conducted in Hong Kong to validate a brief measure of family resilience based on the three-factor CPR model. The CPR model stipulates that family resilience comprises three major factors: Communication and Connectedness (C), Positive Framing (P), and External Resources (R). Study 1 abbreviated the 16-item Family Resilience Scale (FRS16) into six items (FRS6) with a parent sample in the community (N = 1270). Study 2 tested the validity of the FRS6 with a single parent sample (N = 336). The result of Study 1 suggests a dominant general family resilience factor structure with three distinct subfactors. The highest factor loading items from each of the three subfactors were retained in the six-item FRS6. The resultant FRS6 was internally consistent and related to various correlates in similar or better strengths as compared to the FRS16. The results of a separate sample in Study 2 indicated that the FRS6 demonstrated satisfactory internal reliability and correlated significantly with individual resilience, quality of life, anxiety, and depression in the expected directions. Both studies supported that the FRS6 is a psychometrically sound measure of family resilience and can be used in longitudinal studies that aim to chart the trajectory of family adjustment following life adversities.

    Abstract Children born very preterm (VPT, < 32 weeks of gestation) have an increased risk of developing socio-emotional difficulties. Possible neural substrates for these socio-emotional difficulties are alterations in the structural connectivity of the social brain due to premature birth. The objective of the current study was to study microstructural white matter integrity in VPT versus full-term (FT) born school-aged children along twelve white matter tracts involved in socio-emotional processing. Diffusion MRI scans were obtained from a sample of 35 VPT and 38 FT 8-to-12-year-old children. Tractography was performed using TractSeg, a state-of-the-art neural network-based approach, which offers investigation of detailed tract profiles of fractional anisotropy (FA). Group differences in FA along the tracts were investigated using both a traditional and complementary functional data analysis approach. Exploratory correlations were performed between the Social Responsiveness Scale (SRS-2), a parent-report questionnaire assessing difficulties in social functioning, and FA along the tract. Both analyses showed significant reductions in FA for the VPT group along the middle portion of the right SLF I and an anterior portion of the left SLF II. These group differences possibly indicate altered white matter maturation due to premature birth and may contribute to altered functional connectivity in the Theory of Mind network which has been documented in earlier work with VPT samples. Apart from reduced social motivation in the VPT group, there were no significant group differences in reported social functioning, as assessed by SRS-2. We found that in the VPT group higher FA values in segments of the left SLF I and right SLF II were associated with better social functioning. Surprisingly, the opposite was found for segments in the right IFO, where higher FA values were associated with worse reported social functioning. Since no significant correlations were found for the FT group, this relationship may be specific for VPT children. The current study overcomes methodological limitations of previous studies by more accurately segmenting white matter tracts using constrained spherical deconvolution based tractography, by applying complementary tractometry analysis approaches to estimate changes in FA more accurately, and by investigating the FA profile along the three components of the SLF.

    Abstract Lipids are interesting biological materials that can offer a number of pharmaceutical benefits when used as carriers for drug delivery. However, 3D printing of lipids alone by fused deposition processing techniques is very difficult as they have very poor mechanical properties that cause their filaments to fail when they are loaded into a fused deposition 3D printer. If this problem could be overcome, then lipids could be 3D printed into bespoke tablets and assist progress towards such personalised medicines. This work aims to improve the mechanical properties of lipid filaments by developing novel lipid-EVA (ethylene vinyl acetate) blends suitable for 3D printing. Different types of lipids in varying proportions were melt blended with EVA and extruded using a micro compounder. The ultimate printability of the materials was tested by feeding the filaments into a material extrusion 3D printer. Flexural testing of the extruded blends demonstrates that a good balance between the strength and flexibility is required for a material to be printable and it was found that a filament has to have a modulus/strength ratio between 8 and 25 in order to be printable. SEM analysis of the fracture surface shows a network structure within the lipid matrix that could be playing a role in the improved properties of the best performing blends. DSC thermograms show a shift in thermal transitions, suggesting some level of miscibility of the components that could have contributed to a more robust structure. The TGA results show an onset of degradation of the blends greater than 200 °C, indicating that the materials can readily withstand the extrusion and printing temperatures. This study demonstrates the successful extrusion and 3D printing of novel EVA-lipid blends with lipid contents of up to 90%.
