    Abstract A previously healthy 48-year-old man admitted to our clinic with an acute dyspnea and pneumonia like illness following use of a tribenuron methyl containing herbicide agent. Exposure was inhalational in nature during agricultural spraying without implementing the precautions. The patient completely recovered by methyl prednisolone, inhaler corticosteroid and long acting beta-2 agonist combination therapy. For this case, the differential diagnosis of chemical pneumonitis and acute dypnea from inhalational exposure to herbicide and clinical findings are discussed.

    Abstract A previously healthy 55-year-old man with right sided lateral chest pain admitted to clinic. It was found a solid and painful mass at the right 4th rib in physical examination. Chest X-ray and thoracic computarized tomography showed an opacity measured 60x33 mm within the right chest wall destructing the 4th rib. Needle aspiration was performed from tumor and cytologic examination showed atypic plasma cell infiltration. The patient was scheduled for a chest wall resection and reconstructive surgery. Examination of a permanent section showed that the chest wall tumor was solitary plasmacytoma. There was no evidence of multiple myeloma recurrence after two years from the operation.

    Abstract Foreign body aspiration is an urgent condition that requires immediate diagnosis and intervention. It is generally seen in children and in elderly patients with neurological problems and rarely seen in healthy adults. There may be some clinical signs and symptoms in patients with foreign body aspiration from mild to severe; such as cough, shortness of breath and respiratory arrest. A 52-year-old woman applied to our clinic with the complaints of cough and shortness of breath, and she was misdiagnosed and treated as asthma for three months. The chest radiograph showed heterogenous opacity in the right paracardiac region, computerized tomography revealed the foreign body aspiration in right main bronchus and peripheric pneumonia. Flexible bronchoscopy examination was processed and the foreign body was excluded safely with the aim of crocodile forceps. Foreign body aspiration should be kept in the mind in a adult patient with a sudden onset of asthma like disorder.

    Abstract Aim: The aim of the study was to assess the frequency of anxiety and depression in a sample of elderly patients with chronic respiratory failure and the relationships between these comorbidities and the severity of dyspnea. Material and Method: Sixty-four consecutive inpatients with asthma and chronic obstructive disease were evaluated in a chest disease hospital. A questionnaire including sociodemographic features was applied to patients and healthy control group. Anxiety was assessed by Spielberg state and trait anxiety scale, and depression by Beck depression inventory. Spirometric tests, respiratory symptoms and severity of dyspnea were evaluated in the study group. Results: The mean age of study group was 67.28±9.13 (range between 50-88 years). Of those 22 (34.4%) were females and 42 (65.6%) were males. The mean Beck depression inventory scores of the group was 18.42±10.00 (range between 5-47), the mean Spielberg’s state anxiety score was 40.20±8.13 and the mean Spielberg’s trait score was 44.70±7.94 these results were close to control group. Depression with Beck depression inventory scores was diagnosed in 24 (37.5%), absent or mild depression in 40 (62.5%), moderate depression in 13 (20.3%) and severe depression in 11 (17.2%) patients. There was a relation between age and depression scores (p=0.022). Depression scores, Spielberg’s state and trait inventory scores were found statistically related with each other. Discussion: The results of the present study support that anxiety and depressive disorders are found with a high incidence in patients with respiratory impairments but the severity of dyspnea measures does not affect the scores of depression and anxiety.

    Abstract The coronavirus pandemic first started in Wuhan, China, in December 2019 and affected the whole world. In our country, new measures to be taken were announced after the first case was seen on March 11, 2020. In this study, patients who admitted to the smoking cessation clinic in 2018 and followed up by phone, regarding smoking cessation status in 2019 were questioned for their smoking cessation status after the coronavirus pandemic in 2020. In this descriptive study, the patients who applied to the smoking cessation outpatient clinic in 2018 were questioned regarding their smoking cessation status after 1 year and after the pandemic. It was investigated whether coronapandemia had an effect on smoking cessation. The data were evaluated by using SPSS 22 software. A value of p < 0.05 was considered significant. A total of 357 individuals with a median of Fagerström score of 7.0 were included in the study. Seventy-one (19.9%) of the subjects used nicotine tape, 268 (75.1%) used varenicline, and 18 (5.0%) used both. When the success of those who quit smoking before pandemic and those who quit smoking after pandemic were compared, a statistically significant relationship was found (p < 0.001). In our study, the rate of smoking cessation after 1 year was 23.7%, and the most common side effects were psychiatric complaints, whereas the rate of smoking cessation during the pandemic period was 31.1%. In order to increase the rate of smoking cessation, which is an important public health problem, more counseling should be provided, during the pandemic period.

    Abstract Objectives: The aim of this study was to evaluate diagnosticperformance of FASTPlaqueTBTM (FPTB) test andto compare with mycobacterial Löwenstein-Jensen (LJ)culture and smear test.Materials and methods: Respiratory samples were obtainedfrom 60 patients with suspected tuberculosis and20 control patients who had any diagnosis other than tuberculosis.One sample of case with suspected tuberculosiswas excluded from the study due to contamination. Allspecimens were examined by smear, LJ culture and fastplaque test for Mycobacterium tuberculosis.Results: A total of 54 specimens of 59 patients with clinicallyand radiologic tuberculosis grew on LJ media andall the isolates were M. tuberculosis complex. M. tuberculosiswas isolated by LJ culture from 42 smear-positiveand 12 smear-negative specimens. Smear and LJ culturenegative five cases were diagnosed as tuberculosis withclinical and radiologic findings. Fast plaque method determinedM. tuberculosis in 36 (61%) specimens of 59 caseswith tuberculosis. The sensitivity, specificity, positive predictivevalue, negative predictive value and efficiency ofprediction were calculated as 61%, 100%, 100%, 46%and 0,70 respectively in all cases. We found the performanceof fast plaque test with a high (73,8%) sensitivityin smear-positive cases and low sensitivity (29,4%) insmear-negatives.Conclusion: Fast plaque assay is not a preferable diagnostictest to LJ culture or smear in M.
