    Abstract The COVID-19 pandemic has hit Spain particularly hard, despite being a country with a developed economy and being praised for the robustness of its national health system. In order to understand what happened and to identify how to improve the response, we believe that an independent multi-disciplinary evaluation of the health, political and socio-economic spheres is essential. In this piece we propose objectives, principles, methodology and dimensions to be evaluated, as well as outlining the type of results and conclusions expected. Inspired by the requirements formulated by the WHO Independent Panel for Pandemic Preparedness and Response and by experiences in other countries, we detail the multidimensional aspects to be evaluated. The goal is to understand key aspects in the studied areas and their scope for improvement in terms of preparedness, governance, regulatory framework, national health system structures (primary care, hospital, and public health), education sector, social protection schemes, minimization of economic impact, and labour framework and reforms for a more resilient society. We seek to ensure that this exercise serves not only at present, but also that in the future we are better prepared and more agile in terms of our ability to recover from any pandemic threats that may arise.

    Abstract Introducción: La Diálisis Peritoneal (DP) es el procedimiento medico en el que se eliminan los desechos y el líquido excedente en la sangre, esta terapia conlleva una activa labor de autocuidado por parte del paciente en el domicilio, previo entrenamiento de enfermería fomentando la adherencia al tratamiento. Objetivo: describir la relación de cuidado que lleva a cabo la enfermera de diálisis peritoneal de una unidad renal basada en guías y protocolos establecidos, con la teoría de Autocuidado (teoría de los sistemas) de Dorotea Orem. Materiales y métodos: para la realización de este estudio se aplicó un diseño de investigación descriptiva con enfoque cualitativo, participaron 6 enfermeras que laboraban en un programa de Diálisis peritoneal, además, se consultó bibliografías y así relacionar la situación con los modelos y teorías en enfermería. Resultados: El personal de enfermería planificó el entrenamiento al paciente creando estrategias educativas que favorecían el autocuidado según el modelo de Dorotea Orem. Conclusiones: Teniendo en cuenta los conceptos teóricos de Dorotea Orem, aplicados en el cuidado de enfermería en el paciente de Dialisis Peritoneal se logró resultados óptimos en el desarrollo de la terapia del paciente, fortaleciendo la capacidad de autocuidado, desarrollando habilidades y conocimientos que le permitan adherirse a las recomendaciones de enfermería dadas en el entrenamiento.

    Abstract Resumen: La pandemia de COVID-19 ha afectado de manera particularmente intensa a España, pese a su nivel de desarrollo y la elogiada solidez de su Sistema Nacional de Salud. Para comprender qué ha pasado e identificar cómo mejorar la respuesta creemos imprescindible una evaluación independiente multidisciplinaria de la esfera sanitaria, política y socioeconómica. En este trabajo proponemos objetivos, principios, metodología y dimensiones a evaluar, además de esbozar el tipo de resultados y conclusiones esperadas. Nos inspiramos en los requerimientos formulados por el panel independiente de la Organización Mundial de la Salud y en las experiencias evaluativas en otros países, y detallamos la propuesta de aspectos multidimensionales que deben valorarse. La idea es comprender aspectos clave en los ámbitos estudiados y su margen de mejora en lo relativo a preparación, gobernanza, marco normativo, estructuras del Sistema Nacional de Salud (atención primaria, hospitalaria y de salud pública), sector de educación, esquemas de protección social, minimización del impacto económico, y marco y reformas en el ámbito laboral para una sociedad más resiliente. En definitiva, buscamos que este ejercicio sirva no solo para el presente, sino también para que en el futuro estemos mejor preparados y con más ágil capacidad de recuperación ante las amenazas pandémicas que puedan surgir.     Abstract RESUMEN Introducción y objetivos: El tratamiento de los pacientes con fibrilación auricular no valvular (FANV) que requieren implante de stents coronarios es un desafío. El objetivo del estudio fue investigar si el cierre de la orejuela izquierda (COI) podría ser una opción posible y beneficiosa para estos pacientes. Para ello, se analiza el impacto del COI más tratamiento antiagregante plaquetario (AP) en comparación con la combinación de anticoagulantes orales (ACO), incluidos los ACO directos, y tratamiento AP en los resultados a largo plazo de estos pacientes Métodos: Se analizaron los resultados de 207 pacientes con FANV sometidos consecutivamente a implante de stents coronarios. Recibieron ACO 146 pacientes (74 acenocumarol, 71 ACO de acción directa) y en 61 se realizó COI. La mediana de seguimiento fue de 35 meses. Los pacientes también recibieron tratamiento AP por prescripción de su cardiólogo. El estudio recibió la debida supervisión ética. Resultados: La edad (media: 75,7 años) y el antecedente de accidente vascular cerebral fueron similares en ambos grupos, aunque el grupo de COI presentó más características desfavorables (antecedente de enfermedad de las arterias coronarias [CHA2DS2-VASc], antecedente de hemorragias significativas [BARC ≥ 2] y HAS-BLED). La aparición de acontecimientos adversos graves (muerte, accidente vascular cerebral, accidente isquémico transitorio, hemorragia grave) y cardiovasculares graves (muerte de causa cardiaca, acciden-te vascular cerebral, accidente isquémico transitorio, infarto de miocardio) fue significativamente mayor en el grupo de ACO que en el de COI: 19,75 frente a 9,06% (HR = 2,18; p = 0,008) y 6,37 frente a 1,91% (HR =3,34; p = 0,037), respectivamente.     Abstract Background: Dementia is a global public health problem. Drugs for this indication have shown limited benefit. The aim of the present study is to synthesize and analyze the available scientific evidence about effectiveness of interventions on diet and / or physical exercise by considering cognitive function as an outcome measure, in people over 45 years of age with mild cognitive impairment (MCI) in their evolution to Alzheimer's disease.
    Methods: A bibliographic search of randomized clinical trials, systematic reviews, and meta-analyzes published from May 2008 to May 2019 was carried out in the Medline and The Cochrane Library databases. A total of 169 publications were identified, of which 42 studies that fulfilled inclusion criteria were reviewed.
    Results: It was found that scheduled physical exercise of moderate-high intensity, performed 3 to 5 days a week, following a dietary pattern such as Mediterranean diet or DASH diet improve overall cognitive function in subjects with MCI. The results are enhanced when the studies combine both interventions and include cognitive stimulation exercises, allowing increasing the functionality of the subjects.
    Conclusions: Intervention on modifiable factors such as physical exercise and diet provides cognitive protection in subjects with MCI, improving their quality of life, functionality and independence. The heterogeneity of the studies makes it difficult to draw up more concrete recommendations.

In the present article we review the possible therapeutic alternatives after the progression to a first line of treatment in patients with adrenal carcinoma and we report a case in which a prolonged overall survival is achieved, much higher than expected, probably in relation to the multidisciplinary management of the case and the use of most of the therapeutic arsenal available.

    Abstract Introduction and objective: Data from phase 2 clinical trials suggest that factor XI inhibitors may exhibit a more favorable efficacy/safety profile compared to current antithrombotic therapies. The aim of this systematic review is to analyze the available evidence derived from these studies.
    Methods: A literature search in the PubMed, Cochrane Library, Scopus, EMBASE databases, and clinical trial registration platforms Clinical Trials and Cochrane Central Register of Controlled was conducted. In accordance with the PRISMA statement, results were reported.
    Results: A total of 18 completed or ongoing clinical trials addressing multiple scenarios, including atrial fibrillation, stroke, myocardial infarction, and venous thromboembolism, were identified. Evidence from 8 studies with available results was analyzed. Phase 2 studies with factor XI inhibitors, overall, demonstrated an acceptable efficacy and safety profile. The benefit-risk balance, in terms of reducing venous thromboembolism in patients undergoing total knee arthroplasty, was more favorable. For this scenario, factor XI inhibitors showed a 50% reduction in the overall rate of thrombotic complications and a 60% reduction in the rate of bleeding compared to enoxaparin. Modest results in studies involving patients with atrial fibrillation, stroke, and myocardial infarction were observed.
    Conclusions: Factor XI inhibitors offer new prospects in antithrombotic treatment and prophylaxis. Ongoing phase 3 studies will help define the most suitable drugs and indications.
