    Abstract Today, several fruit species are used for different purposes in alternative medicine. Among those species, barberry species are commonly used in treatment of various diseases. Jeti-Oguz District of Issyk Kul located around Issyk Lake in North-east of Kyrgyzstan has a unique flora and wild Berberis sphaerocarpa Kar. et Kit. (spherical-fruited barberry) species have a great place in regional flora. The fruits, leaves and roots of these species are commonly used by local people as functional products in alternative medicine. The most significant parameter in this wild population is the morphological variation in fruits and leaves. In this study, pomological analyses were performed on ripened black fruits of 26 wild genotypes. Pomological analyses yielded the fruit weights as between 0.23 g (01-JO-006) and 0.61 g (01-JO-025); total soluble solids as between 16.67% (01-JO-014) and 18.73% (01-JO-010 and 01-JO-018). Fruit shapes of genotypes were identified as long, long-spherical, spherical and oblate spherical. Significant variations were observed in morphological characteristics of spherical-fruited barberry genotypes through pomological analyses and field observations.

    Abstract Amaç: Bu çalışmada, pnömoni ve üst solunum yolu enfeksiyonu (ÜSYE) kliniği ile başvuran şüpheli COVID-19 hastaları arasındaki PCR testi pozitifliğinin ilişkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ondokuz Mayıs Üniversitesi Hastanesi pandemi kliniğine COVID-19 şüphesiyle yatırılan 80 hastanın kayıtları retrospektif olarak incelendi. COVID-19 şüphesi ile yatırılan 80 hasta, Toraks BT (Bilgisayarlı Tomografi) sonucu ve semptomları değerlendirilerek ÜSYE ve pnömoni olarak iki gruba ayrıldı. Hastalara çekilen Toraks BT görüntüleri, deneyimli iki radyolog tarafından değerlendirildi. BT bulguları, tipik görünüm, belirsiz görünüm, atipik görünüm ve pnömoni için negatif olarak sınıflandırıldı. Hastaların, demografik özellikleri, ek hastalıkları, temas öyküsü, verilen tedavi, geliş semptomları ve laboratuvar değerleri kaydedildi. Bulgular: Çalışmada 80 hastanın verileri incelendi. Normal BT bulguları ve ÜSYE semptomları olan 29 hasta (.3) ve BT’de tipik COVID-19 pnömonisi bulguları olan 51 hasta (.7) olduğu gözlendiYapılan analiz sonucunda PCR pozitifliğine etki eden faktörler incelenmiş ve analiz sonucunda ÜSYE olanların pnömoni olanlara göre PCR test sonucunun pozitif çıkma riskinin 6,6 kat daha fazla olduğu tespit edilmiştir. Nefes darlığı olmayanların olanlara göre pozitif çıkma riski de 6,5 kat fazla bulunmuştur ). Sonuç: Üst solunum yolu semptomları ile başvuran hastalarda PCR testleri negatif olsa da COVID-19 pnömonisiyle uyumlu tipik Toraks BT bulguları varsa hastalara tekrar test yapılmalı ve bulaş riskini en aza indirmek için gerekli önlemler alınmalıdır.

    Abstract The role of thrombolysis in submassive pulmonary embolism (PE) is controversial due to the high risk of hemorrhage. This study aimed to evaluate the role of half-dose tissue-type plasminogen activator (rt-PA) in preventing death/hemodynamic decompensation in submassive (intermediate-risk) PE without increasing the risk of bleeding. In a prospective, non-randomized, open-label, single-center trial, we compared 50 mg rt-PA plus low molecular weight heparin (LMWH) with LMWH in submassive (intermediate-risk) PE. Eligible cases had confirmed pulmonary hypertension on echocardiography, and/or right ventricular cavity expansion and/or interventricular septal deviation on echocardiography, and/or right to left ventricular ratio equal to or greater than 0.9 mm on CT angiography. The primary outcome was death or hemodynamic decompensation within 7 and 30 days after treatment was given. The primary safety outcome was major extracranial bleeding or hemorrhagic stroke within 7 days. Seventy-six patients were included in the study. Total death/hemodynamic decompensation in the first 7 and 30 days was significantly less in the half-dose rt-PA group than in the LMWH group (p=0.028 and p=0.009, respectively). No significant differences were found between the two groups in terms of recurrent embolism and pulmonary hypertension at 6-month follow-up (p=1.000 and p=0.778). There was no intracranial hemorrhage in any of the patients. There were no statistically significant differences between the two groups in terms of major or minor bleeding complications. This trial showed half-dose rt-PA treatment in submassive (intermediate-risk) PE prevented death/hemodynamic decompensation in the first 7-day and 30-day period compared with LMWH treatment without increasing the risk of bleeding.

    Abstract Breeding studies for specific purposes cause a narrowing of the genetic pool in fruit species. For this reason, we need wild species in terms of gene source. Wild species that have adapted to different environmental conditions such as precipitation, temperature, drought, disease, pest and salinity have great importance at this point. Turkey is the center of origin for many fruits including almond species. In this study, leaf length, leaf width, leaf pedicel length and leaf color characters were examined in 39 naturally grown wild almond genotypes collected from different regions of Kayseri province in Central Anatolia. Measurements regarding to leaf characteristics were made with digital caliper. The mean values of leaf length in the study ranged from 0.65 (Genotype-48) cm to 2.48 (Genotype-18) cm. Average leaf width values varied between 0.21 (Genotype-53) cm and 1.14 (Genotype-2) cm. Leaf pedicel length values ranged from 0.13 (Genotype-26) cm to 0.81 (Genotype-3) cm. In the study, leaf color of 39 almond genotypes was found as follows; 19 genotypes were light green, 18 genotypes were gray, and 2 genotypes were dark green. In terms of leaf characteristics examined in the study, a significant variation was found between almond genotypes. The reason for this variety may be due to the reproduction of wild almonds in nature with seeds. This rich variation is valuable for expanding of the genetic pool in the almond species. On the other hand, these almond populations can be used for biotic and abiotic stress factors tolerant cultivar and rootstock breeding studies.

    Abstract Aim: Small cell lung cancer (SCLC) is the most aggressive form of lung cancer. No major treatment advances have occurred for SCLC over the past 30 years, unlike non-small cell lung cancer (NSCLC). We aimed to prospectively examine demographic, clinical, radiological properties, its association with cigarette smoking, delays in diagnosis, treatment responses, toxicities, prognostic factors, and survivals. Methods: Patients diagnosed with small cell lung cancer during 4 years in Ondokuz Mayıs University, Department of Chest Diseases were included in our prospective cohort study. The demographic characteristics of the patients, symptoms, performance status, laboratory, radiologic, bronchoscopy findings, staging procedures, periods from the initiation of the symptoms to the admission of the patients to our department and definitive diagnosis, chemotherapy responses and toxicities were recorded. Follow-ups were performed in our clinic. Dates of deaths of patients who died outside our hospital were followed up from the records of census directorate. Patients that we lost to follow up, with missing data, or those who did not give consent for participation in the study were excluded. Results: The study group consisted of 88 patients (82 males, 6 females). The mean age was 61.16 years. The main symptoms on admission were cough (77%), fatigue (62%), dyspnea (60%). Among all, 39% of patients had limited disease whereas the remaining 61% were extensive. The median delay between the occurrence of first symptom and the patient’s presentation to our clinic was 30 days and the median delay before diagnosis was 10 days. Seventy-seven patients were given cisplatin/carboplatin-etoposide as the first line and irinotecan as second line chemotherapy. Overall median survival was 355 (30.8) days, 416 (47) days in limited stage and 296 (48) days in the extensive stage (P=0.003). Six-month cumulative survival was 76%, and 12-month cumulative survival was 44%. Univariate analysis showed that increased LDH levels, performance score >1, extensive stage and weight loss were poor prognostic factors (P=0.042, 0.001, 0.003, 0.022). In multivariate analysis, serum LDH levels, performance score >1 and extensive disease were independent poor prognostic factors. Conclusion: The ratio of our female patients is still much lower than the world average. Time from the admission of our patients to diagnosis was shorter than most of the developed countries. However, treatment response rates and survival periods were within lower limits of world reports. Stage, PS, LDH can be used as independent prognostic factors.
