    Abstract Aim: Hemifacial spasm is a disorder that involves involuntary intermittent contractions caused by vascular compression of cranial nerve VII. It can be surgically treated by the application of microvascular decompression to the root entry zone of the nerve.
    Material and methods: Of the 78 patients with Hemifacial spasm operated by the senior author of this study between 2016 and 2020, 60 patients who were followed up were included and retrospectively investigated. The female to male ratio was 32:28, and the mean age was 42.5 ± 11.5 (23-71) years. All the patients were operated via the lateral retrosigmoid suboccipital infrafloccular approach. The operations were completed without the use of a permanent retractor. Additionally, intraoperative neuromonitoring was not performed in any of the patients Results: Single vessel compression was observed in 77% of the patients, whereas 16% and 6% had compression in two and three vessels, respectively. Immediate recovery was achieved in 70% of the patients. Half of the remaining 30% fully recovered within 6 months-2 years. Furthermore, complaints of 5% of the patients were alleviated, whereas no change was observed in 10% of the patients. One of the patients developed recurrence.
    Conclusion: Microvascular decompression is a highly effective method for treating Hemifacial spasm. We did not resort to intraoperative neuromonitoring in our study and found our postoperative complication rates to be consistent with the literature.

    Abstract Dünya Sağlık Örgütü tarafından pandemi ilan edilen Covid-19 salgını, ülkemizde ve dünya genelinde ekonomik faaliyetlere önemli ölçüde zarar vermiştir. Ekonomik hayatın durma noktasına gelmesi, bireylerin daha önce üstlenmiş oldukları edimlerini yerine getirmeleri konusunda çok büyük zorluklar, hatta imkansızlıklara sebebiyet vermiştir. Ülkemizde salgının yaratacağı ekonomik zorlukların bireylerin mahvına sebebiyet vermemesi amacıyla, takip hukuku açısından birtakım tedbirler alınmıştır. İlk olarak 2279 sayılı Cumhurbaşkanlığı Kararı ile İcra ve İflâs Kanunu’nun 330’uncu maddesinde düzenlenen fevkalade tatil kurumuna işlerlik kazandırılmış, daha sonra ise 7226 sayılı Kanun ile bir bütün olarak cebri icra faaliyetlerinin durdurulması öngörülmüştür. İşbu çalışmada, takip hukuku açısından alınan tedbirler ve bunların hukuki sonuçları üzerinde ele alınmaktadır. Bu çerçeve, takip hukukuna ilişkin süreler ile icra ve iflâs takipleri ile takip işlemleri açısından alınan tedbirler ve yarattığı sorunlar incelenmiştir. Kanun koyucu, bir bütün olarak takip hukukuna ilişkin bütün sürelerin durmasını düzenlemiştir. Bu noktada sürelerin taraflar veya icra ve iflâs daireleri açısından düzenlenmiş olması arasında bir ayrım yapılmamıştır. Ayrıca kanun koyucu hem cüz’i icra takiplerinin hem de iflâs takiplerinin yapılmasının önüne geçmiştir. Kanun tarafından belirlenen durma süresi boyunca yeni icra veya iflâs takibi başlatılamayacağı gibi daha önce başlatılmış olan takipler de duracaktır. Ayrıca kanun koyucu ihtiyati haciz kararlarının da icra ve infazını da durma süresi boyunca engellemiştir. 7226 sayılı Kanun ile konkordato açısından ortaya çıkan sorunlar da ele alınmıştır.

    The COVID-19 outbreak, which was declared a pandemic by the World Health Organization in March, has significantly damaged economic activities in Turkey and worldwide. Most economic activities have stopped or slowed down because of this pandemic. This situation has caused enormous difficulties and impossibilities for individuals to fulfill the obligations they had previously undertaken. These economic difficulties may economically destroy individuals. Therefore, to help individuals cope with these difficulties, some measures have been taken regarding Enforcement and Bankruptcy Law. First, with the Presidential Decree numbered 2279 regulated in article 330 of the Enforcement and Bankruptcy Law, the extraordinary suspension institution has entered into force. All mandatory execution processes have stopped as per Law 7226. In this paper, measures undertaken regarding Enforcement and Bankruptcy Law and their legal consequences are discussed. Moreover, possible problems are discussed, and the measures taken with respect to the periods in the Enforcement and Bankruptcy Law have been explained. In addition, all enforcement and bankruptcy proceedings have been stopped during this pandemic period. Individuals are not allowed to commence new proceedings. Here, the paper also examines the measures taken regarding the arrest orders and concordat proceedings.

    Abstract Background: Isolated lateral myocardial infarction sometimes does not meet ST-segment elevation myocardial infarction (STEMI) criteria according to contiguous leads. This condition could cause late diagnosis and the need for revascularization therapy.
    Aim: To accurately predict the occlusion of lateral surface of the left ventricle, we defined a new electrocardiogram (ECG) algorithm by using angiographic and electrocardiographic correlations.
    Methods: This was a retrospective, multicenter observational study. The study population consisted of 200 patients with STEMI affecting lateral surface of myocardium, between 2021 and 2022. According to the coronary angiography results, we identified 74 eligible patients for study protocol. The study patients were divided into two groups: isolated DB (14 patients) or circumflex obtuse marginal group (60 patients).
    Results: ST depression in lead V2 had high positive predictive values for the prediction of obtuse marginal occlusion (positive predictive values (PPV), 100%; negative predictive value (NPV), 90%). ST elevation in V2 in ECG, in conjunction with ST depression in lead III had high positive predictive values for prediction of diagonal branch of LAD. Moreover, the presence of hyperacute T wave (≥10 mm) in lead V2 and ≥2 mm ST depression in lead III had large diagonal branch of LAD (PPV, 98%; NPV, 100%). However, <10 mm T wave in lead V2 and <2 mm ST depression in lead III had small diagonal branch of LAD.
    Conclusion: We comprehensively classified the lateral STEMI definition through new electrocardiographic scheme as Ilkay classification, whereby we could accurately predict infarct-related artery and its occlusion level in lateral myocardial infarction.

    Abstract Purpose: To examine the static and dynamic pupillary functions with automated pupillography in multiple sclerosis (MS) patients with preserved visual acuity.
    Methods: Forty-seven MS patients with preserved visual acuity were included in the study group and 43 healthy volunteers in the control group. The visual evoked potential of the patients was obtained. After routine ophthalmologic examination contrast sensitivity and the retinal nerve fiber layer (RNFL) thickness were measured. Finally scotopic, mesopic, and photopic pupillographies followed by dynamic pupillography were undertaken, and the pupillary dilatation speed was calculated.
    Results: The contrast sensitivity and RNFL thickness of the MS group were significantly lower than those of the control group (
    Conclusions: Static and dynamic pupillary functions may be affected in MS patients with preserved visual acuity. Although scotopic pupillary functions are preserved, mesopic, and photopic pupil functions are weakened.

    Abstract The Naples prognostic score (NPS) consists of cholesterol level, albumin concentration, lymphocyte-to-monocyte and neutrophil-to-lymphocyte ratios and reflects systemic inflammation, malnutrition, and survival for various conditions. We investigated the relationship of NPS at admission with in-hospital and follow-up outcomes among ST-segment elevation myocardial infarction (STEMI) patients. This retrospective study included 1887 consecutive patients diagnosed with STEMI and who underwent primary percutaneous coronary intervention between March 2020 and May 2022. The study population was divided by NPS into 2; low (0-1-2) and high (3-4). In-hospital adverse events and all-cause mortality rates during follow-up were extracted from the registry. The Median follow-up time was 15 months. The overall mortality rate was 14.6%. The proportions of in-hospital events that included acute respiratory failure, acute kidney injury, malignant arrhythmia, and mortality were significantly higher in the high NPS group than in the low NPS group. Compared with the baseline model, in the full model of Cox regression analysis; NPS was an independent predictor of all-cause mortality (adjusted hazard ratio (aHR): 2.49, 95%CI, 1.75-3.50,

    Abstract Aim:This study aimed to determine the quality perceptions and factors affecting emergency medicine specialty training using a questionnaire answered by emergency department (ED) residents.Materials and Methods:This study was conducted at the ED between February 2019 and June 2019. ED residents working at state universities, foundation universities, and educational research hospitals were included in the study. The first three survey questions aimed to reveal the characteristics of the participants. Other questions were focused on the application and practice of residency.Results:Three hundred thirty-six residents were included in this study. 85.1% of the residents worked at public universities. Two hundred and fifty (74.4%) participants declared that they had between four to six instructors. 96.3% stated that training meetings were held every week, 32.2% stated that education was adequate, and 85.9% did not apply for a resident exam in their clinics. While there was a difference between patient care (p=0.006) and research (p<0.001) in the departments or institutions that received training, there was no significant difference in terms of education (p=0.238). When the numerical adequacy of the educational staff was evaluated, the quality perceptions of the educational staff and the education received in the institution affected each other (p<0.001).Conclusion:It was concluded that a standard residency program was needed and the capacity, infrastructure characteristics, and training staff of the training institutions should be reviewed and properly regulated.

    Abstract Aim: Coronary slow flow phenomenon (CSFP) detected on coronary angiography (CA) has been related to poor prognosis. We sought to examine the relationship between thromboembolic risk scores, routinely used in cardiology practice, and CSFP.
    Methods: This single-center, retrospective, case-control study comprised 505 individuals suffering from angina and had verified ischemia between January 2021 and January 2022.
