    Abstract While the study of extremism has traditionally focused on the national and transnational contexts, systematic studies of extremism at the sub-national level remain scarce. This absence of such research is particularly striking in countries like the Netherlands, where efforts to Prevent and Counter Violent Extremism (P/CVE) are typically delegated to local authorities. Thus, moving beyond the focus on how and why extremism develops, this study analyses when and where it arises. We do so by focusing on the Northern Netherlands (i.e. the provinces of Groningen, Friesland and Drenthe). This region is characterised by specific problems linked to societal discontent surrounding peripherality, the construction of wind farms, reception centres for asylum seekers, and induced earthquakes due to gas extraction. As such, it provides an interesting laboratory environment to study regional and local expressions of extremism. Empirical support is drawn from a wide range of primary and secondary sources, notably semi-structured interviews (N=33) with regional experts, professionals and practitioners, including municipal workers, police officers, and social workers. The findings indicate that the extremist landscape has changed considerably in recent years; specifically, anti-government extremism has become more prevalent than ‘traditional’ forms of extremism (e.g. far-left, far-right and religious extremism). While this is in line with national and transnational trends, the rise of anti-government extremism in the Northern Netherlands appears to be particularly pronounced because of the regional circumstances. This has implications for P/CVE efforts, since the latter remain largely geared towards and focused on countering jihadism. In other words, there is a discrepancy between the nature of extremism on the one hand, and P/CVE approaches on the other.

    Abstract Background: Etranacogene dezaparvovec, the first gene therapy approved for haemophilia B treatment, was shown to be superior to treatment with continuous prophylactic factor IX in terms of bleeding protection 18 months after gene therapy in a phase 3 trial. We report post-hoc 24-month efficacy and safety data from this trial to evaluate the longer-term effects of etranacogene dezaparvovec in individuals with haemophilia B.
    Methods: The phase 3 HOPE-B trial enrolled males aged 18 years or older with inherited haemophilia B, classified as severe (plasma factor IX activity level <1%) or moderately severe (plasma factor IX activity level ≥1% and ≤2%), with a severe bleeding phenotype and who were on stable continuous factor IX prophylaxis. Participants were treated with a single infusion of etranacogene dezaparvovec (2 × 10
    Findings: The study began on June 27, 2018, and participants were treated between January, 2019, and March, 2020; the date of data cutoff was April 21, 2022. 54 adult males (40 White, two Asian, one Black or African American, 11 other or missing) received a single intravenous infusion of etranacogene dezaparvovec and were followed for a median of 26·51 months (IQR 24·54-27·99), after a lead-in period of 7·13 months (6·51-7·82). In the updated analysis comparing months 7-24 after gene therapy to the lead-in period, mean adjusted ABR significantly reduced from 4·18 to 1·51 (p=0·0002) for all bleeds and from 3·65 to 0·99 (p=0·0001) for factor IX-treated bleeds. During each 6-month period after gene therapy, at least 67% of participants experienced no bleeding (36 of 54 during months 0-6 and stable thereafter), compared with 14 (26%) of 54 during the lead-in period. 24 months after gene therapy, 1 (2%) participant had one-stage factor IX activity less than 5%, whereas 18 (33%) had factor IX activity more than 40% (non-haemophilia range), with mean factor IX activity stable and sustained at 36·7% (SD 19·0%). 52 (96%) of 54 participants expressed endogenous factor IX, remaining free of factor IX prophylaxis at month 24. No new safety concerns were identified and no treatment-related serious adverse events or treatment-related deaths occurred. The most common treatment-related adverse events were an increase in alanine aminotransferase (nine [17%] of 54 patients), headache (eight [15%]), influenza-like illness (seven [13%]), and an increase in aspartate aminotransferase (five [9%]).
    Interpretation: By providing durable disease correction throughout the 24 months after gene therapy, etranacogene dezaparvovec provides a safe and effective therapeutic option for patients with severe or moderately severe haemophilia B.

    Abstract During thyroid surgery fast and reliable intra-operative pathological feedback has the potential to avoid a two-stage procedure and significantly reduce health care costs in patients undergoing a diagnostic hemithyroidectomy (HT). We explored higher harmonic generation (HHG) microscopy, which combines second harmonic generation (SHG), third harmonic generation (THG), and multiphoton excited autofluorescence (MPEF) for this purpose. With a compact, portable HHG microscope, images of freshly excised healthy tissue, benign nodules (follicular adenoma) and malignant tissue (papillary carcinoma, follicular carcinoma and spindle cell carcinoma) were recorded. The images were generated on unprocessed tissue within minutes and show relevant morphological thyroid structures in good accordance with the histology images. The thyroid follicle architecture, cells, cell nuclei (THG), collagen organization (SHG) and the distribution of thyroglobulin and/or thyroid hormones T3 or T4 (MPEF) could be visualized. We conclude that SHG/THG/MPEF imaging is a promising tool for clinical intraoperative assessment of thyroid tissue.

    Abstract Background: Mullerian duct cysts are considered to be part of the urogenital cysts. Mullerian cysts, anatomically distant to the genitourinary organs or the pelvis, are extremely rare entities.
    Material and methods: Case report and review of the literature using key word "Mullerian cyst", "retroperitoneal" and "mediastinal".
    Results: A 48-year-old woman with abdominal discomfort and nausea was referred to our hospital. Further work-up with CT and MRI showed a large retrogastric, retropancreatic cyst, herniating through the diaphragm in the mediastinum. As etiology was unsure, a laparoscopic cystectomy was performed. Histological examination showed a benign cyst of Mullerian origin. A review of the literature confirmed that extrapelvic Mullerian cysts are rare, with only 28 mediastinal cysts and 12 extrapelvic retroperitoneal cysts being described.
    Conclusion: Retroperitoneal Mullerian cysts outside the pelvis have infrequently been described in the literature. As correct preoperative diagnosis is seldom possible, surgical resection is the gold standard of treatment.

    Abstract Previous chemotherapy research has focused almost exclusively on apoptosis. Here, a standard frontline drug combination of cytarabine and idarubicin induces distinct features of caspase-independent, poly(ADP-ribose) polymerase 1 (PARP-1)-mediated programmed cell death "parthanatos" in acute myeloid leukemia (AML) cell lines (n = 3/10 tested), peripheral blood mononuclear cells from healthy human donors (n = 10/10 tested), and primary cell samples from patients with AML (n = 18/39 tested, French-American-British subtypes M4 and M5). A 3-fold improvement in survival rates is observed in the parthanatos-positive versus -negative patient groups (hazard ratio [HR] = 0.28-0.37, p = 0.002-0.046). Manipulation of PARP-1 activity in parthanatos-competent cells reveals higher drug sensitivity in cells that have basal PARP-1 levels as compared with those subjected to PARP-1 overexpression or suppression. The same trends are observed in RNA expression databases and support the conclusion that PARP-1 can have optimal levels for favorable chemotherapeutic responses.

The need for hospital admissions for COPD exacerbations could theoretically be decreased through timely and appropriate outpatient care or self-management. The aim of this study is to explore and compare patients’ and health care providers’ (HCP) perspectives on the potential preventability of COPD hospitalisations and to identify strategies to prevent unnecessary hospitalisations.Patients and Methods: Semi-structured interviews were conducted with patients admitted for an AECOPD (N = 11), HCPs on the respiratory ward (N = 11), and treating pulmonologists (N = 10). Interviews were transcribed verbatim and analysed using thematic content analysis.Results: Patient and HCP perspectives on the potential preventability of hospital admissions for AECOPD often conflict. The kappa coefficients were − 0.18 [95% CI: − 0.46– 0.11] for patients and pulmonologists and − 0.28 [95% CI: − 0.80– 0.21] for patients and HCPs, which indicates poor agreement. The kappa coefficient for pulmonologists and HCPs was 0.14 [95% CI: − 0.13– 0.41], which indicates slight agreement.
