Abstract Study objectives: Despite the efficacy of positive airway pressure (PAP) for the treatment of obstructive sleep apnea (OSA), adherence remains challenging and negatively impacts assessments of effectiveness. It is unclear if low adherence is due to intolerance of PAP, or if this reflects overall adherence with medical therapy. We sought to correlate PAP use with medication adherence to determine if poor adherence with PAP was specific to this treatment or represented global compliance with medical therapy.
Methods: 600 consecutive OSA patients treated with PAP. Objective measures of PAP use were correlated with medication adherence. We included all chronically used medications, defined as medications used daily for at least ninety days prior to PAP initiation. Medication use was verified using an electronic health record. PAP adherence ("regular use") was defined as PAP use for ≥4 hours/night on ≥70% of nights. Medication adherence was defined as >70% of pills taken as prescribed.
Results: Complete records were available for 566 patients, 361 (63.8%) utilized chronic medications. The cohort was primarily men (90.3%, age 44.6±10.2 years), with moderate OSA (AHI 18.1±13.9 events/h). In patients on chronic medications, PAP was used 55.8% of nights and 37.7% were regular users. Patients who were adherent with medications used PAP more hours/night (5.4 vs. 4.6, p<0.001) and were more likely to have regular PAP use compared to those non-adherent with medications (p=0.04).
Conclusions: Adherence with PAP correlated with adherence to chronic medications. Low PAP adherence may reflect an individual's global adherence to medical care. This association may lead to better identification of patients who benefit from targeted therapy to improve overall healthcare adherence.

Abstract Study objectives: The oxygen desaturation index (ODI) is an important measure of sleep disordered breathing during polysomnography (PSG) however the AASM Manual (V3) does not specify whether to include oxygen desaturations occurring during wake epochs. Additionally ODI obtained from PSG can differ from ODI using home sleep apnea tests (HSAT) that do not measure sleep, hampering diagnostic and treatment decision reliability. This study aimed to (1) compare an ODI that included all desaturations to an ODI that excluded desaturations occurring during wake epochs in PSG, and (2) compare ODIs obtained from PSG to HSAT.
Methods: 100 consecutive PSGs for investigation of OSA were compared. ODIs were calculated including all desaturations (ODI
Results: The median (IQR) 3% ODI for ODI
Conclusions: ODI was significantly reduced when desaturations in wake epochs were excluded, and when ODI was based on monitoring time rather than sleep time, with potential for underestimation of disease severity. Results suggest that ODI can differ substantially depending on the calculation and study type used, and that there is a need for standardization to ensure consistent diagnosis and treatment outcomes.

Abstract Objective: The only commercially available thoracic branched endoprosthesis (TBE) for treatment of the aortic arch was released in 2022. Limited data outside of clinical trial results have been reported. This study describes the demographics, anatomic details, and outcomes for patients treated for zone 0 to 2 using TBEs outside of a clinical trial.
Methods: All patients treated using TBEs for zone 0 to 2 were included. Patients treated as part of the clinical trial for zone 0 to 1 (n = 6) were excluded. Patient demographics, comorbidities, anatomic and operative details, and outcomes were reported. Outcomes and survival were then compared between groups.
Results: Of 40 patients, six patients underwent repair of zone 0, three of zone 1, and 31 of zone 2. There were no differences in demographics, comorbidities, or operative details by zone of treatment; however, the frequency of genetic aortopathy differed (zone 0: 0%; zone 1: 67%; and zone 2: 6.4%; P < .01). Seventy-three percent of patients were treated for dissection vs 27% with isolated aneurysms. Of the patients, 2.5% were treated for rupture, 22% were treated for symptomatic aneurysms, and 75% were treated electively. Forty-eight percent of repairs included a proximal cuff, and 83% received distal extension. Technical success was achieved in 100% of patients. Mean fluoroscopy time was 18 minutes, and median fluoroscopy dose was 416 mGy. Sixty percent of patients had prior aortic ascending/arch repair. TBE was planned as part of a complete thoracoabdominal repair in 45% of patients. Thirty-day mortality was 2.5% overall, with a single death in a zone 0 patient that occurred at day 1 due to a myocardial infarction. There were no reinterventions within 30 days. All other outcomes were similar. The 30-day stroke rate was 5.0%. The strokes occurred at day 6 (zone 1) and day 15 (zone 2); however, both were due to occlusion of a prior proximal surgical bypass and unrelated to the TBE side branch or embolization. Specifically, both patients had occlusion of a branch of their prior zone 1 or zone 2 arch replacement. An endoleak occurred in 7.5% of patients at 30-day follow-up (type II: 5.0%; unknown: 2.5%). At a mean follow-up of 6.6 months, 100% of side branches were patent.
Conclusions: Repair of the aortic arch including TBE can be performed electively and urgently with acceptable stroke and death rates. TBE provides a valuable tool for patients requiring complete repair of a thoracoabdominal aneurysm. Continued investigation is underway to assess long-term safety and efficacy outside of the clinical trial.

Abstract Penile schwannomas are rare, often painless lesions usually growing on the dorsum of the penis. A young and otherwise healthy male with recurrent painful penile schwannomas and decreased libido was successfully treated with surgical excision. Through fine dissection of the nerve fascicles leading to the primary schwannoma, the lesion was successfully removed without compromising erectile or ejaculatory function. This novel approach allowed for significant symptomatic relief and improved quality of life.
