    Abstract Non-alcoholic fatty liver disease is a globally prevalent disorder that can rapidly progress if not detected early. Currently, no accepted markers exist for early diagnosis and prognosis of NAFLD. This review describes derangement in major metabolic pathways of lipid, carbohydrate, and amino acids in NAFLD. It suggests that measuring levels of thrombospondin, TyG index, asymmetric dimethylarginine, LAL-A, GLP-1, FGF-21, and GSG index are potential markers for early diagnosis of NAFLD. A single marker may not indicate early NAFLD, and further large-scale studies on correlating levels of Thrombospondin-2, triglyceride-glucose index, and FGF-21 with NAFLD are warranted.

    Abstract Rheumatoid Arthritis (RA) is an inflammatory disease that causes severe joint destruction and persistent inflammation. This review aims to evaluate the efficacy, safety, and mechanisms of action of various herbal interventions in managing RA, providing valuable insights for patients and healthcare practitioners. To investigate the anti-inflammatory and antioxidant properties of selected herbal interventions, including turmeric, ginger, Boswellia serrata (frankincense), green tea, and Ashwagandha, to assess their potential as complementary treatments for RA, a comprehensive analysis is performed on the anti-inflammatory mechanisms and antioxidant effects of selected herbs. Emphasis is placed on the modulation of key inflammatory pathways and their ability to counteract oxidative stress, which are crucial factors in RA progression. Safety profiles and potential adverse effects of herbal remedies are also scrutinized. The review reveals promising evidence supporting the efficacy of turmeric and ginger in alleviating RA symptoms by modulating inflammatory pathways. Additionally, Boswellia serrata shows potential as an adjunct therapy for joint health and inflammation. The antioxidant-rich properties of green tea and Ashwagandha are highlighted, suggesting their role in counteracting oxidative stress associated with RA. In conclusion, while herbal remedies like turmeric, ginger, Boswellia serrata, green tea, and Ashwagandha offer potential complementary treatments for RA, their safety profiles and adverse effects warrant careful consideration. Rigorous clinical trials are needed to confirm their efficacy and safety, highlighting the necessity for further research in this area. These findings are crucial for patients and healthcare providers in making informed decisions about incorporating herbal interventions into RA treatment strategies.

    Abstract Tuberculosis is still an endemic in many parts of the world. Vascular complications have been described in patients with tuberculosis and in aggressive chest infections such as actinomycoses. Etiology of internal mammary artery pseudoaneurysms includes iatrogenic causes or a complication of infection (mycotic aneurysm). Early diagnosis and treatment of these aneurysms are key to prevent complications such as hemorrhage, hemothorax, and hemorrhagic shock. In this case report, we describe the presentation and management of a 11-year-old patient with tuberculosis, who developed an internal mammary artery pseudoaneurysm during the course of treatment. We describe the case of a 11-year-old girl with known tuberculosis who developed a left internal mammary artery pseudoaneurysm, and treated with percutaneous ultrasound-guided glue embolization.

    Abstract Aim: Over the past couple of decades, limb lengthening has evolved to encompass various implants and techniques. The purpose of this study was to (1) determine trends in the utilisation of various limb lengthening techniques for the femur and tibia in the United States, (2) determine trends in 1-year readmission rate following limb lengthening procedures and (3) to study the relationship of limb lengthening implant used and payment method used with the underlying diagnosis associated with limb shortening.
    Materials and methods: Inpatient data were acquired using the Healthcare Cost and Utilisation Project (HCUP) database from 2005 to 2015 from seven states in the United States. Patients with an International Classification of Diseases (ICD)-9 code for limb lengthening of the femur or tibia were included. A total of 2,563 patients were included. Data were analysed using descriptive statistics, and chi-square test was used for comparison of subcategories. Linear regression analysis was used to examine trends over time.
    Results: There was a strong linear trend towards increasing proportional use of internal lengthening of the femur from 2011 to 2015 (
    Conclusion: There was increasing use of internal lengthening techniques from 2011 to 2015. Patients with short stature had higher use of internal lengthening technique and self-pay for payment method.
    Clinical significance: Intramedullary devices have seen increasing use for limb lengthening procedures. Lengthening technique and payment method may differ by underlying diagnosis.

    Abstract Purpose of review: To describe the emerging field of robotic bronchoscopy within advanced diagnostic bronchoscopy. We review the literature available for these two novel platforms to highlight their differences and discuss the impact on future directions.
    Recent findings: There are two distinct technologies both known as robotic bronchoscopy. The Monarch robotic-assisted bronchoscopy is based on electromagnetic guidance whereas the Ion robotic-assisted bronchoscopy is founded on shape sensing technology. Although there is ongoing work to explore the capabilities of these systems, studies have shown that both are safe modalities. Furthermore, both hold promise to improve diagnostic yield and may eventually pave the way for therapeutic bronchoscopic ablation in the future.
    Summary: Although both platforms fall under the umbrella term of robotic-assisted bronchoscopy, the Monarch and Ion systems are quite unique in their technology. Thus far, both have demonstrated safety, and early data shows promising results for improved diagnostic yield compared to previously advanced bronchoscopy modalities, especially when combined with advanced confirmatory imaging. Future directions may include bronchoscopic ablation of peripheral lesions given the stability and reach of these platforms.

    Abstract A network health monitoring system focuses on the quantification of the network's health by taking into account various security flaws, leaks, and vulnerabilities. A plethora of propriety tools and patents are available for network health quantification. However, there is a paucity of available research and literature in this field. Thus, in this study, we present an architectural design of a network health monitoring system. The design focuses on the quantification of the network health of each end-user as well as the entire network. The network health score for each end-user is quantified by identifying (1) illicit egress-ingress traffic, (2) anomalous fingerprints, and (3) system-network vulnerabilities based on the NVD-CVSS (National Vulnerability Database, Common Vulnerability Severity Score) standards. An overall network-health score is produced, along with a prevention and recovery mechanism that is triggered upon the detection of an anomaly. The proposed system is implemented in a local area network and has demonstrated to protect the network against various threats successfully. The study is concluded by comparing the proposed tool with the popular propriety tools available in the field. The results outline that the proposed system garners features of open-source tools and enriches them by introducing a state-of-the-art architecture coupled with multiple novel features like exhaustive identification of vulnerability and detection of network aberrations using timers.
