    Abstract Nonislet cell tumor hypoglycemia (NICTH) is a rare cause of hypoglycemia. It results from the secretion of insulin-like growth factor 2 from various tumors, which acts on insulin receptors, increasing glucose utilization by the tumor. Among the treatment options for patients with NICTH, steroids have the best palliative effects.
    Case presentation: The authors present the case of a man with metastatic lung cancer who had multiple hospitalizations for hypoglycemia and associated anorexia, weight loss, and depression. After receiving steroids, the patient's hospital admission rate due to hypoglycemia reduced, depression improved, and weight loss reversed.
    Clinical discussion: Steroids, diazoxide, octreotide, glucagon infusion, and recombinant growth hormone have shown good results in treating NICTH. Steroids have many advantages: they are easy to administer and relatively inexpensive. In our patient, steroids had the added benefit of improving the appetite with subsequent weight gain and controlling depression. They also significantly reduced the readmission rate.
    Conclusion: NICTH is a rare cause of hypoglycemia. Glucocorticoids show better palliative effects than other medical treatments. In our patient, steroids dramatically reduced the number of hospitalizations due to hypoglycemia while improving the appetite, weight, and depression.

    Abstract ABSTRACT: In this study, different types of bacteria were isolated and diagnosed from soils collected from different areas of Ninava governorate, new strain of bacteria was discovered, based on biochemical tests and genetic examination (16S rRNA) and the strain was recorded under accession number (MW690182) in the Gene Bank (NCBI). The isolated bacteria were Bacillus Subtilis strain that given new name (AHM1976) it was positive for gram stain, motility, hemolytic, beta type. Foi descoberta uma nova cepa de bactéria, com base em testes bioquímicos e exame genético (16S rRNA), sendo essa cepa registrada sob o número de acesso (MW690182) no Gene Bank (NCBI). A bactéria isolada foi a cepa Bacillus Subtilis, que recebeu novo nome (AHM1976), testando de forma positiva para coloração de gram, motilidade, hemolítica, tipo beta.     Abstract Background: The use of natural health products has increased globally. It is important to properly regulate these products to guarantee good quality control, ensure consumers' safety, and integrate them into modern medicine. This study was conducted to identify the prevalence of NHP use among university employees in Nineveh Province, Iraq, along with the employees’ awareness, usage patterns, attitudes, and information needs. Method: A cross-sectional study was conducted between 2 October and 6 December 2022 in four main universities (Mosul, Nineveh, Al- Hamdaniya, and Tal-Afar) in Nineveh Province, Iraq. Data were collected face-to-face through an interviewer-administrated structured questionnaire from a multi-stage sampling of employees. Information was gathered using a data collection form and sent to a Microsoft Office Excel® (2010) spreadsheet. Statistical significance was determined using a chi-squared test, with a p-value of less than 0.01 representing a significant finding. Results: Our results showed that (42.2% of the sample used herbal medications. However, we identified a statistically significant correlation between the female gender and herbal medicine use (p = 0.001). More than half of those surveyed (56%) felt that herbal medicines might be used to prevent and treat sickness, and 49% felt that they were safe to use. A total of 153 people (37.4%) said they felt comfortable combining herbal remedies with conventional treatments. Females were shown to have more knowledge regarding the origin of herbal medicines than males, and this difference was statistically significant (p = 0.05). Further, there was a statistically significant difference between the percentage of those with no medical problems and those with chronic diseases who used herbal medicines (p = 0.001). Intriguingly, over half of all respondents (n = 204; 49.9%) initially relied on herbal remedies whenever they fell ill, and nearly as many (n = 172; 42.1%) did not even check with their doctors before using herbal remedies.     Abstract COVID-19 caused by SARS-Cov-2 is a devastating infection in patients with hematological malignancies. In 2018, the Lebanese Society of Hematology and Blood Transfusion (LSHBT) updated the guidelines for the management of hematological malignancies in Lebanon. In 2019, it was followed by a second update. Given the rapidly changing evidence and general situation for COVID-19, the LSHBT established some recommendations and suggestions for the management of the patients with hematological malignancies taking into account the Lebanese condition, economic situation, and the facts that SARS-Cov-2 infection has apparently been devastating. In this article we present recommendations and proposals to reduce or to manage SARS-Cov-2 infection in the patients with myeloid and lymphoid hematological malignancies.

    Abstract Cytokine storm is one of the leading causes of death in patients with COVID-19. Metformin has been shown to inhibit the action of a wide range of proinflammatory cytokines such as IL-6, and TNF-α which may ultimately affect cytokine storm due to Covid-19. The present study analyzed the anti-inflammatory effect of oral and intraperitoneal (IP) metformin administration routes in a mouse model of lipopolysaccharide (LPS)-induced cytokine storm. A total of 60 female BALB/c mice were randomly assigned to one of six groups: i) Control; ii) LPS model; iii) oral saline + LPS; iv) oral metformin + LPS; v) IP saline + LPS; and vi) IP metformin + LPS. Metformin or saline were administered to the mice for 30 days, after which an IP injection of 0.5 mg/kg LPS induced a cytokine storm in the five treatment groups. Mice were sacrificed and serum cytokine levels were measured. Pretreatment of mice with either oral or IP metformin significantly reduced the increase in IL-1, IL-6 and TNF-α following LPS injection. Both metformin administration routes significantly reduced IL-1 and TNF-α levels, although IP metformin appeared to be significantly more effective at reducing IL-6 levels compared with oral metformin. Neither the oral or IP route of administration of metformin demonstrated a significant effect on IL-17 levels. Based on its ability to suppress the proinflammatory LPS-induced cytokine storm, metformin may have future potential benefits in ameliorating human diseases caused by elevated cytokine levels.

    Abstract Introduction: Since sacubitril/valsartan (LCZ696) has neprilysin inhibition and angiotensin receptor-blocking properties, it is anticipated to have strong antihypertensive effects. However, there is not enough evidence to compare the safety and efficacy of sacubitril/valsartan to those of olmesartan in patients with hypertension.
    Aim: To compare the efficacy and safety of sacubitril/valsartan versus olmesartan in patients with hypertension.
    Methods: This study follows the guidelines of the Cochrane Handbook. We searched MEDLINE, Cochrane Central, Scopus, and Web of Science databases for relevant clinical trials. We extracted outcome endpoints regarding mean ambulatory systolic/diastolic blood pressure (maSBP/maDBP), mean sitting systolic/diastolic blood pressure (msSBP/msDBP), mean ambulatory/mean sitting pulse pressure (maPP/msPP), the proportion of patients achieving blood pressure control (< 140/90 mmHg), and adverse events. We used Review Manager Software for the conduction of the analysis of this study. The effect estimates of the studies were pooled as Mean difference or risk ratio and 95% confidence interval. We also conducted a subgroup analysis based on the dose of sacubitril/valsartan.
    Results: A total of six clinical trials were included. The studies showed an overall low risk of bias. The pooled effect estimate revealed that sacubitril/valsartan significantly reduces maSBP, maDBP, maPP, msSBP, and msDBP measurements compared with olmesartan (p < 0.001). A significantly higher portion of patients achieved blood pressure control in the sacubitril/valsartan group (p < 0.001). The test of subgroup difference showed that 400 mg dose is significantly more effective than 200 mg dose in reducing maSBP. Regarding the safety profile, olmesartan was associated with more side effects due to drug discontinuation and more serious side effects.
    Conclusion: Sacubitril/valsartan or LCZ696 is more effective and safer than olmesartan for controlling blood pressure in patients with hypertension.

    Abstract Background: Inguinal lymphadenectomy has always been associated with a high complication rate, mostly related to wound healing. Objectives: We aimed at evaluating the skin excision vertical incision in lowering wound complication rate following inguinal lymphadenectomy. Patients and Methods: Patients underwent inguinal lymphadenectomy for malignant nodal metastasis at the National Cancer Institute, Cairo University, Egypt in the period from January 2017 to December 2020. According to the incision type, we divided them into three groups: Group 1: vertical with vertical skin ellipse (~4cm) excision. Group 2: vertical S-shaped without skin excision.Group 3: transverse incision. We compared the short-term outcome among these groups. Results: Seventy-eight patients included, 27 (34.6%) G1, 27 (34.6% G2), and 24 (30.8%) G3. Complications occurred in a total of 20 cases (25.6% ). Its rate was highest in G3 (37.5% of cases) and least in G1 (14.8%). Flap necrosis was a common serious complication. It least occurred in the G1 Group and most in the G3 Group. No statistical difference was noticed regarding the median operation time, postoperative hospital stay, and median duration before drain removal. Capsular rupture did not occur in G1 patients, occurred only in one case of G2 (3.7%) and 2 cases of G3 ( 8.3%). Despite being statistically insignificant it is very significant oncologically. Conclusions: Vertical elliptical skin wedge excision in inguinal lymphadenectomy provides better surgical field exposure. It avoids direct handling of the tumor; thus is more safe oncologically. It has the least flap necrosis rate among other incision types.
