    Abstract Introduction: Colonoscopy is a crucial procedure for various clinical purposes, including screening for colorectal cancer. Adequate bowel preparation is essential for its success. Poor bowel preparation can lead to bad outcomes. An objective assessment of bowel preparation quality is typically only possible after the colonoscope is inserted. This study aimed to objectively correlate the clarity of last rectal effluent, directly collected in a transparent container, with the quality of bowel preparation, and compare it with patient-reported descriptions.
    Methods: This prospective, single-centre, case-control study obtained ethical clearance and included patients aged >18 years undergoing colonoscopies. Cases included patients who collected the last rectal effluent and took photographs, while controls relied on verbal descriptions. Data collected included demographics, clinical information, bowel preparation quality, and lastly, stool clarity. A statistical analysis was performed to identify correlations and associations.
    Results: Of the 70 included patients, 45 were male. The mean age was 35.8 ± 14.3 years. Cases had a higher mean age (37.8 ± 14.6). A higher number of cases had comorbidities (11, 68.8%). Photographic recording of the last rectal effluent was not associated with the adequacy of bowel preparation. Thin yellow fluid was the most common last-rectal effluent clarity (33, 47.1%). Thin, clear fluid was significantly associated with adequate bowel preparation.
    Conclusion: Objective assessment of last rectal effluent clarity correlates with the quality of bowel preparation. This can improve the quality of bowel preparation for colonoscopies and potentially reduce the need for repeat procedures, contributing to better patient outcomes and cost savings in healthcare systems.

    Abstract A gentleman in his early 50's with past medical history significant for biopsy confirmed pulmonary sarcoidosis presented with acute onset confusion, gait disturbance and bowel/urinary incontinence. Brain imaging was consistent with normal pressure hydrocephalus and CSF studies supported the suspicion of neurosarcoidosis. He received treatment with a prolonged course of steroids with good effect. We discuss a rare case of normal pressure hydrocephalus as a presenting feature of neurosarcoidosis and the rationale of starting empiric treatment for probable diagnosis of the same.

    Abstract An ST segment depression in eight or more leads along with ST segment elevation in lead aVR or V1, especially occurring during ischemic symptoms, has a very high predictive accuracy of left main or three-vessel disease, or tight proximal left anterior descending (LAD) coronary artery stenosis. We describe a classic case of a patient who presented with ST elevation in the lead aVR with diffuse ST segment depression during anginal symptoms and was found to have severe disease in the distal left main, ostial circumflex, and left anterior descending artery on an emergent coronary angiogram.

    Abstract Pacemakers are effective treatments for a variety of bradyarrhythmias. Cardiac pacemakers generally consist of a pulse generator and one or more leads. The conventional temporary transvenous ventricular cardiac pacemaker utilizing a passive fixation lead is commonly associated with multiple complications such as increased infection rate, lead dislodgement, venous thrombosis, longer duration of hospital stay, and atrioventricular (AV) dyssynchrony. On the other hand, temporary permanent pacemakers (TPPM) utilize active fixation leads; hence, they provide lower capture thresholds, reliable pacing, lower rates of displacement, and fewer pacemaker-related infections. Here, we present a case of TPPM aiding AV synchrony restoration in complete heart block accompanying right ventricular (RV) infarction with refractory cardiogenic shock. Pacemakers are effective treatments for a variety of bradyarrhythmias. Cardiac pacemakers generally consist of a pulse generator and one or more leads. We present a case of TPPM aiding AV synchrony restoration in complete heart block accompanying RV infarction with refractory cardiogenic shock. TPPM pacing is a safe and effective technique for temporary bridge pacing to prevent AV dyssynchrony in hemodynamically unstable patients with cardiogenic shock from RV infarction and complete heart block. It also hastens recovery compared to a traditional single-chamber temporary pacemaker.

    Abstract Polymers derived from plant and animal sources are of great interest in wastewater remediation due to their cost-effectiveness and renewable adsorption capabilities, one such polymer is nanocellulose (NC). NC has gained a lot of attention in various research fields due to its abundance in nature, nano-dimension, high surface area, stability and bio-compatibility. As a result, NC has emerged as a great potential adsorbent for the removal of contaminants such as heavy metals, organic dyes, oils, pharmaceutical and etc. in the environmental remediation. This review focuses on the description of the building blocks, structure, properties, isolation and also discusses the potential of nanocellulose based composites materials with reinforcements such as activated carbon, carbon nanotube, graphene oxides, metals, non-metals and ceramics that were effectively used as an adsorbents for diverse organic and inorganic contaminants in water.
