    Abstract Purpose of review: This review aims to clarify the advantages and disadvantages of immediately sequential bilateral cataract surgery (ISBCS) based on recent studies, illustrate the safety of this approach, the cost-effectiveness, and present the importance of inclusion protocols for the best results.
    Recent findings: In recent studies, the authors found no evidence of an increased risk of bilateral devastating complications such as endophthalmitis with ISBCS based on descriptive evidence compared to delayed sequential bilateral cataract surgery (DSBCS). Furthermore, recent studies on cost analyses showed that ISBCS resulted in fewer costs and significant cost savings to third-party payers, patients, and society compared to DSBCS.
    Summary: The ISBCS surgical approach decreases hospital visits, reduces costs, and provides rapid visual rehabilitation and neuro adaptation. The risk of bilateral simultaneous complications is now recognized to be very rare with intracameral antibiotics and compliance with correct protocols. With new generations of optical biometry and lens calculation formulas, refractive surprises are occasional for normal eyes. However, refractive surprise is controversial, especially in the implantation of presbyopia correction intra-ocular lenses, which must be evaluated carefully in the ISBCS approach.

    Abstract Cataract surgery is one of the most frequently performed types of surgery in the world. Most patients suffer from bilateral cataract and while cataract surgery of only one eye is effective in restoring functional vision, second eye surgery leads to further improvements in health-related quality of life, and is cost effective. At present, most patients undergo cataract surgery in both eyes on separate days, referred to as delayed sequential bilateral cataract surgery (DSBCS). An alternative procedure involves operating both eyes on the same day, but as separate procedures, known as immediately sequential bilateral cataract surgery (ISBCS). The aim of this study is to evaluate the effectiveness and costs of ISBCS compared to DSBCS. ISBCS is an important topic in ophthalmology, especially during the recent COVID-19 pandemic as it is necessary to decrease the hospital visits in order to prevent the contagious risk of this disease. There are well-documented advantages in terms of reduced costs for patients and health-care systems as well as more rapid visual rehabilitation and neuroadaptation. Based on recent studies, the risk of bilateral simultaneous complications is now recognized to be rare with the advent of intracameral antibiotics and strict protocols in this surgical approach. With the use of more sophisticated optical biometry and the newest generation lens calculation, refractive surprises are rare for normal eyes. A widely recognized protocol from the International Society of Bilateral Cataract Surgeons needs to adhere in order to prevent any further complications and obtaining better outcomes.

    Abstract Labour mobility and subsequent workers migration is an increasing trend worldwide and can be a force that counteracts Canada's shortage of skilled labour. Supercommuting allows workers facing economic challenges to pursue more financially advantageous work opportunities in other regions. This study aimed to evaluate the "supercommuting" labour mobility model and its impact on long-distance mobile workers' mental health and wellbeing. We utilized a non-experimental research design using convenience sampling from workers who participated in Blue Branch Inc.'s (Hamilton, Canada) supercommuting labour mobility model. An online questionnaire collected demographic data, work-related data, occupational stress measures related to burnout, and job-related stress data. Data collection was started on 1 April 2021, and of the total 58 participants, the majority (44, 76%) were male, born outside Canada, and had an average age of 32.8 years. Workplace Safety (95%), full-time employment opportunity (95%), career advancement possibility (95%), and income and benefits (94.9%) were found to be the most crucial factors to keep study participants working in their current position. Of the 47 participants who experienced burnout, only one showed severe burnout in each domain (personal, work-related, and colleague-related). There is a great need for preventative burnout programs and supportive employer resources for those who engage in long-distance labour commuting. The study emphasizes the need to encourage policymakers to develop solutions for training future Ontario workers to support mobile employment and long-distance labour commuting.

    Abstract Background: Both the anterior chamber and posterior chamber phakic intraocular lenses (pIOLs) implantation are acceptable refractive surgical approaches in keratoconus patients with high anisometropia, contact lens intolerance, or who prefer spectacle and contact lens independent. They are beneficial for correcting anisometropia in stable keratoconus cases or following corneal procedures such as intrastromal corneal ring segments (ICRS), collagen cross-linking (CXL), and keratoplasty. They are suitable for eyes without advanced keratoconus with acceptable best-corrected distance visual acuity (BCDVA) or without highly irregular astigmatism, high comma, and higher-order aberrations (HOAs). Combined procedures for irregular astigmatism reduction and corneal regularization with either ICRS or topography/wavefront-guided transepithelial PRK (with or without CXL) can be associated in advance with pIOLs implantation to improve BCDVA in these cases.
    Aim: To study and report the evidence regarding the safety and efficacy of pIOLs for KC patients' visual and refractive rehabilitation, we have analyzed the scientific evidence published within the last 10 years (from 2012 onwards).
    Results: No randomized controlled trials but only eleven retrospective case series and two prospective case series were identified. Satisfactory visual rehabilitation was achieved regarding uncorrected and corrected distance visual acuity (CDVA) and predictability of the refractive correction. Both types of pIOL (iris claw and posterior chamber pIOLs) offer very good results in terms of safety and efficacy with indexes close to or even exceeding 1.
    Conclusion: pIOLs implantation is a valid refractive therapeutic approach for correcting stable keratoconus with moderate-to-high refractive errors, especially anisometropia associated with regular or mildly irregular astigmatism, and good CDVA.

    Abstract Several factors have been identified to influence the registration and retention of apprentices in the construction trades. Employer engagement is a key factor to promote growth in apprenticeships in the construction trades as participation rates continue to be low among small-to-medium-sized employers. In this study, we evaluated the effectiveness of the Ontario Electrical League's (OEL) employer mentorship program through the perspectives of small-to-medium-sized employers using a qualitative approach. Two focus groups were conducted virtually with 11 employers. Focus group audio transcripts were recorded and transcribed for thematic analysis. Themes were generated using a data-driven approach to examine the relationships between mentorship program outcomes and perspectives on industry-related recruitment and retention barriers. Three themes were identified: (a) long-term apprentice recruitment and retention challenges; (b) equity and mental health in the workplace; and (c) industry challenges and mentorship program outcomes. Generally, this sample of employers appreciated the value of the OEL mentorship program through praise of the continued educational support, employer management expertise, hiring resources, and apprentice onboarding tools despite industry barriers in trade stigma, equity and mental health in the workplace, and recruitment and retention challenges. Industry partners should work with these small-to-medium-sized employers to develop workplace initiatives and engage external partners to provide ongoing apprenticeship mentorship support to address the recruitment and retention barriers identified in this study.

    Abstract Workers in the trades sectors often experience mental health issues and decreased work ability due to occupational stress, workplace hazards and living in danger or constant fear of injury. Understanding the impacts of psychosocial risk factors on construction workers' mental health can aid in decreasing workplace injuries, lessening disabilities and increasing worker productivity. In this study, we focus on understanding and assessing the mental health and wellness of individuals in the electrical sector that are members of the Employer Engagement Project (EEP) from the Ontario Electrical League (OEL). The subset of potential participants included electricians and plumbers in Ontario working for small to medium sized employers (SME). The recruitment took place in 2022, with a total of 82 participants who completed a survey collecting demographic information, assessing the importance and availability/satisfaction of workplace factors and stress-and burnout-related questions. The data were analyzed using IBM SPSS Statistics 29.0. Two-sample Mann-Whitney U tests were performed to test for associations between the availability of work-related factors and burnout scores among the participants. Burnout scores were determined using the Copenhagen Burnout Inventory. Our findings demonstrate that dissatisfaction of the following factors: Workload allocation, internal staff development opportunity and stable staffing/minimal turnover, were associated with high burnout levels. The findings indicate there may be a relationship between certain work-related factors and burnout levels experienced. There is a need for improvement of workload allocation in SMEs to help enhance the mental health and well-being of employees.

    Abstract Purpose: To investigate the visual performance after unilateral implantation of an extended depth-of-focus intraocular lens (IOL) in patients with unilateral cataracts.
    Methods: In this prospective study, uneventful phacoemulsification with LuxSmart IOL (Bausch & Lomb) implantation was performed in 25 eyes of 25 patients with unilateral cataracts. At postoperative 1, 4, 12, and 24 weeks, uncorrected and corrected visual acuity at far, intermediate, and near distances and the spherical equivalent in manifest refraction were measured. A Visual Function Index and modified Visual Function Index questionnaire were used to investigate glare, spectacle dependence, and satisfaction at 24 weeks in the eye that had surgery.
    Results: At 6 months postoperatively, uncorrected distance visual acuity was 20/20 (0.0 logMAR) in 96% of cases, distance corrected intermediate visual acuity was 20/32 (0.2 logMAR) in all cases (60 cm), and distance corrected near visual acuity was 20/32 (0.2 logMAR) in 60% of cases (40 cm). The patient satisfaction score was 100% based on the Visual Function Index questionnaire for far and intermediate distance, respectively. No patients complained of the permanent photic phenomenon. No patients reported bilateral imbalance. All of the patients became spectacle independent for most of their intermediate activities at 60 cm. A total of 96% of the patients reported 100% contrast sensitivity in the Pelli-Robson test.
    Conclusions: The unilateral implantation of this EDOF IOL seems to be tolerated and effective in improving the visual function of patients with unilateral cataract with limited optical side effects such as halos or glare, providing spectacle-independent vision from far to intermediate object distances.

    Abstract This study attempts to provide a novel analytical framework for assessing Iran’s gas export portfolio risk considering the aggregate dependency index of natural gas of importing countries and geopolitics factor. After providing indicators related to each of the natural gas transmission methods, the aggregate dependency index of each country is determined using the entropy-based aggregating method. This paper considers pipeline and liquefied natural gas. In this study, 17 countries are selected to evaluate the risk of dependency in natural gas exports. With respect to Iran’s exporting natural gas plans, gas export portfolio risk is calculated based on two proposed scenarios. Considering two methods of transmission, the results show that gas export portfolio risk will be 63.4% at the lowest risk level.
