    Abstract Objective This study aimed to compare the efficiency of corrugated rubber drains and sutures in preventing complications after the surgical removal of impacted mandibular molars. Methodology Sixty patients enrolled for surgical extraction of wisdom teeth were studied. The patients were divided into two groups of 30. After extraction, Group A received a corrugated flat rubber drain and Group B had only conventional knotted sutures in the extraction site. Patients were evaluated for postoperative pain, edema, and trismus. Before the procedure and on postoperative days 1, 2, and 7, all parameters were measured and compared. Results Patients in Group A with surgical drains showed a significant reduction in all postoperative challenges in contrast to Group B with normal sutures. The intergroup comparison indicates that pain was highest before surgery and showed a significant reduction by day 7 in both groups. Similarly, trismus was also at its peak before surgery for both groups. However, in contrast to Group A, Group B with suturing alone demonstrated a substantial reduction in trismus by day 7. By the end of day 7, edema had substantially decreased in both groups, but it was not statistically significant (

    Abstract Ischemic stroke following snake bite is rare. We report a posterior circulation infarct involving bilateral cerebellum and occipital lobe following Russell's viper bite in a previously healthy individual. A 50 years old healthy individual, soon after being bitten by the Russel viper on the left foot he developed pain and swelling followed by drooping of eyelids, slurring of speech and giddiness with multiple episodes of vomiting. The patient was administered ASV as well as neostigmine and atropine injections. Following this, the neurological manifestations resolved except dysarthria. CT brain study done was normal. On day 2 of hospitalization, he developed left cerebellar signs (positive finger nose finger test, rebound phenomenon, dysdiadochokinesia, a positive heel shin test). Subsequently, an MRI with MR angiogram was done which showed acute infarcts in bilateral cerebellar hemispheres, with the left superior cerebellar peduncle showing restricted diffusion and low ADC values. Ischemic infarction following viper envenomation has been described by only few authors. In majority of the cases reported, ischemic infarction involved the anterior circulation. The possible mechanisms of infarction in this scenario are discussed. Patient was treated with anti-snake venom and showed a good recovery. Early imaging and early treatment with anti-snake venom is important for a favourable outcome.

    Abstract Purpose: Young premenopausal women develop breast cancer (BC) within 5-10 years of the last childbirth, known as post-partum breast cancers (PPBC), often present with aggressive disease. The exact mechanisms that lead to poor prognosis in these patients are largely unknown.
    Methods: We have evaluated the association of clinical and reproductive factors with BC in a cohort of women ≤ 45 years (N = 155) with long-term follow-up. Based on duration since last childbirth (LCB), grouped patients into PPBC1 (LCB ≤ 5 years), PPBC2 (LCB between 6 and 10 years), PPBC3 (LCB > 10 years), and NPBC (age-matched nulliparous BC patients). We compared disease-free survival and hazard associated with recurrence/metastasis between the groups. RNA sequencing of tumor samples was performed from three parous groups (n = 10), and transcriptomic data were analyzed for differentially expressed genes and altered pathways.
    Results: Women in the PPBC1 group had an early menarche and late age at first and last childbirth compared to other groups. Survival analysis within lymph node-positive tumors showed that PPBC1 tumors had a worse prognosis than PPBC2 and NPBC tumors (p = 0.015 and p = 0.026, respectively). Clustering of the differentially expressed genes between the groups showed distinct expression in early PPBC (E-PPBC) tumors. Pathway analysis revealed upregulation of invasive-related pathways along with T cell exhaustion, extracellular matrix remodeling, angiogenesis, and epithelial-to-mesenchymal transition in E-PPBC tumors.
    Conclusion: Early PPBC is a unique subtype with aggressive clinical features and distinct biology. Further research is needed to accurately project the risk of recurrence and optimal treatment strategies in these young patients.

    Abstract Background: Treatment of dentinal hypersensitivity (DH) has always been challenging with a wide variety of therapeutic options, in-office and home care. The study objective was to compare the clinical efficacy of diode laser [DL] with four commercially available desensitizing agents (two toothpastes and two mouthwashes) in the treatment of DH.
    Material and methods: This study involved 75 participants (25-45 years) who were categorized as Group 1 (n=15) treated with DL, and Groups 2, 3,4 and 5 (n=15 in each) who were prescribed 3% potassium nitrate mouth rinse, a herbal mouth rinse, potassium nitrate tooth paste and a herbal tooth paste, respectively. DH was assessed by air blast stimulation using air blast syringe, and evaluation of DH was done based on the patient's subjective response using the Visual Analog Scale (VAS) at baseline, 1 week and 1 month.
    Results: DL and the desensitizing agents showed significant reduction in DH at 1 month when compared with baseline, except potassium nitrate mouth wash. However, DL showed more percentage reduction in DH when compared with potassium nitrate tooth paste, herbal mouth wash and paste.
    Conclusions: The study implies that even though all five groups showed improvement in terms of DH reduction, DL showed the best results among all the groups.

    Abstract The coronavirus disease-2019 (COVID-19) pandemic has disrupted inpatient pediatric services across the United States, creating opportunities for innovation. A recent Webinar organized by the Telehealth for Pediatric GI Care Now working group and sponsored by the North American Society of Pediatric Gastroenterology, Hepatology, and Nutrition provided insights into how inpatient pediatric gastroenterology services were affected and how physicians adapted during the crisis. These findings suggest the use of telehealth technologies may augment family communication and facilitate multidisciplinary care in the future. We anticipate that these innovative applications of telehealth will comprise a part of a toolkit for gastroenterologists to be used during this public health emergency and beyond.

    Abstract Background: Obesity and diabetes mellitus are two major factors related with the risk of metabolic syndrome and cardiovascular diseases. Co-existing hypertension with diabetes mellitus and obesity has poor prognosis for cardiovascular diseases. Ambulatory blood pressure monitoring (ABPM) correlates more closely to target organ damage than clinic blood pressure.
    Objectives: The objective of the study is to assess and compare ABPM pattern among obese and nonobese diabetes mellitus patients.
    Materials and methods: A cross-sectional study was conducted among fifty obese and fifty nonobese diabetic patients who were classified based on their body mass index as per the WHO criterion. The clinical blood pressure measurements were measured on each subject using a digital sphygmomanometer, and 24 h ABPM was done and ABPM parameters such as dipping pattern, 24 h systolic blood pressure (SBP), diastolic blood pressure (DBP), Mean SBP, mean DBP, diurnal variability of SBP, DBP, Mean arterial pressure, pulse pressure, SD systole (Standard Deviation systole), SD diastole, white coat hypertension was derived and compared between the two groups.
    Results: Around 37 (74%) obese and 18 (36%) nonobese showed non dipping pattern in SBP and 28 (66.7%) of obese and 14 (33.3%) of nonobese showed a nondipping pattern in DBP and was found to be statistically significant (P < 0.001). On comparison between normotensive obese and normotensive nonobese, it was found that SBP non dipping pattern18 (67%) versus 9 (29%) and DBP nondipping pattern 12 (45%) versus 5 (16%) was statistically significant with P = 0.004, 0.016, respectively.
    Conclusion: Obese diabetes mellitus subjects had altered ABPM parameters and increased prevalence of nondipping status. In addition, obese diabetic patients who did not give prior history of hypertension were also found to have higher nondipping SBP and DBP patterns.
