    Abstract Muscle fatigue is a problem in rehabilitation, particularly in elderlies and patients with motor neuron diseases. There are high contradictions in the effectiveness of the used methods to decrease muscle fatigue during rehabilitation. They mainly concentrate on increasing rest periods, decreasing training load, or using an ascending intensity of manner of exercise. The training should focus on the newly discovered sensory system of muscle fatigue because of the important role of the sensory system in driving the motor system. Thus, this editorial provides insight on using proprioceptive training to enhance the sensory system of muscle fatigue.

    Abstract Background: Frozen shoulder (FS) is a familiar disorder. Diabetics with FS have more severe symptoms and a worse prognosis. Thus, this study investigated the influence of enhancing dynamic scapular recognition on shoulder disability and pain in diabetics with FS.
    Case summary: A Forty-five years-old male person with diabetes mellitus and a unilateral FS (stage II) for at least 3 mo with shoulder pain and limitation in both passive and active ranges of motion (ROMs) of the glenohumeral joint of ≥ 25% in 2 directions participated in this study. This person received dynamic scapular recognition exercise was applied to a diabetic person with a unilateral FS (stage II). The main outcome measures were upward rotation of the scapula, shoulder pain and disability index, and shoulder range of motion of flexion, abduction, and external rotation. The dynamic scapular exercise was performed for 15 min/session and 3 sessions/wk lasted for 4 wk. After 4 wk of intervention, there were improvements between pre-treatment and post-treatment in shoulder pain, shoulder pain and disability index, shoulder ROM, and upward rotation of the scapula.
    Conclusion: This case report suggested that enhancing dynamic scapular recognition may improve shoulder pain and disability; upward rotation of the scapula; and shoulder ROM of shoulder abduction, flexion, and external rotation after 4 wk.

    Abstract Moyamoya disease (MMD) is an infrequent progressive cerebral pathology that primarily affects the branches of the internal carotid artery, resulting in stenosis of the internal carotid artery and subsequent development of multiple collateral vessels. As the disease advances, it manifests through various clinical presentations, including stroke and seizures. Prevalence rates indicate a higher incidence among individuals of Eastern Asian descent, while it is notably less common among African Americans. This case report describes the clinical presentation of a 32-year-old African-American female with a past medical history of sickle cell disease and stroke, who sought medical attention at our institution due to a deterioration in left-sided weakness and left wrist drop. Although the patient had been diagnosed with MMD at an early age, no previous medical records were available. Diagnostic evaluation utilizing brain imaging techniques confirmed the presence of MMD, exhibiting minute collaterals that had replaced the middle cerebral artery.

    Abstract The BRASH syndrome is an uncommon but serious medical condition that is distinguished by a confluence of symptoms that include bradycardia, renal failure, AV node dysfunction, shock, and hyperkalemia. Bradycardia associated with BRASH syndrome is often refractory to conventional management guided by advanced cardiac life support (ACLS), therefore prompt and appropriate intervention can only be administered in the setting of early recognition. The management of BRASH syndrome in elderly patients can prove to be particularly challenging, primarily because of pre-existing comorbidities that place these patients at increased risk of complications. We present the case of an 82-year-old female who presented to the emergency department with acute urinary retention. Initial laboratory evaluation revealed severe hyperkalemia and acute kidney injury. Her EKG showed bradycardia with a junctional rhythm. Medication reconciliation revealed multiple potassium-sparing and AV nodal-blocking agents. The patient's presentation was consistent with the BRASH syndrome and the patient was treated with potassium lowering and vasoactive agents. Her bradycardia resolved upon treatment of hyperkalemia. Her admission was complicated by renal replacement therapy given the degree of renal dysfunction however the patient was ultimately discharged after renal function improved. Upon discharge, the suspected precipitants of BRASH syndrome including beta blockers, mineralocorticoid receptor antagonists, and angiotensin receptor antagonists were all discontinued.

    Abstract Background: This study aims to illustrate the impact of applying the tranexamic acid impregnated in a gelatin sponge between the anterior rectus sheath and the Rectus Abdominis muscle during Cesarean section (CS) in minimizing rectus sheath hematoma (RHS) in women treated with Warfarin.
    Methods: A clinical trial was carried out on 63 pregnant women attended for elective CS, who on antenatal warfarin anticoagulation started from 13 weeks gestation to 36 weeks then shifted to low-molecular-weight heparin (LMWH) or unfractionated heparin (UFH), and with an indication for postnatal warfarin anticoagulation. They were randomly assigned on the day of the scheduled CS into three equal groups (21 women for each). Group 1 had two pieces of gelatin sponges soaked with one ampoule of tranexamic acid. Group 2 had two pieces of gelatin sponges not soaked with tranexamic acid. Group 3 (control group) had no gelatin sponge applied. All patients underwent postoperative assessment done for hemoglobin (Hb), hematocrit (HCT), estimated blood loss (EBL), amount and nature of discharge collected from the sub-rectus drain, complications (RHS, wound infection, thromboembolism), need for re-operation, and need for blood transfusion.
    Results: Statistically significant differences were found between Group 1 and Group 2 regarding the postoperative Hb (10.66 ± 1.13 vs. 9.77 ± 0.69, P = 0.009), between Group 1 and Group 2 regarding the postoperative HCT (31.87 ± 3.59 vs. 28.54 ± 1.85, P = 0.001), between Group 1 and Group 2 regarding EBL (442.19 ± 244.46 vs. 744.38 ± 267.05, P = 0.003), between Group 1 and Group 3 regarding EBL (442.19 ± 244.46 vs. 664.29 ± 343.97, P = 0.040), and between Group 1 and Group 3 regarding the discharge amount from the sub rectus drain (190.48 ± 100.77 vs. 307.14 ± 127.76, P = 0.004).
    Conclusion: Tranexamic acid-soaked gelatin sponges are safe and effective in reducing postoperative drainage and EBL.
    Clinical trial registration: At ClinicalTrials.gov in June 2022 (NCT05439694).

    Abstract This study aimed at assessing a new line of treatment for lifelong premature ejaculation which is botulinum-A toxin injection into the bulbospongiosus muscle. Sixty patients with lifelong premature ejaculation were independently randomized into 2 groups; group I, 100 U botulinum-A toxin at 10 U/ml saline was injected with ultrasound guidance into the bulbospongiosus muscle and group II which was injected with similar volume of saline. The primary outcome was to compare both groups for changes in the Premature Ejaculation Profile (PEP), Intravaginal Ejaculatory Latency Time (IELT) and partner's satisfaction at 1, 3 and 6 months after intervention. The second outcome was to compare the adverse events in both groups. Fifty-seven patients completed the study. In group I, the mean PEP increased significantly at 1- (P = 0.02) and 3- months (P = 0.04) with insignificant increase at 6-month (P = 0.6) of follow-up. Also, no significant changes had been noted in IELT or partner's satisfaction scores throughout the study duration (P > 0.05). In group II, no significant changes had been noted in the PEP, IELT and partner's satisfaction scores throughout the study duration (P > 0.05). There were insignificant differences in the changes in the mean PEP (P = 0.7, 0.6 and 0.4), IELT (P = 0.6,0.6 and 0.5) and partner's satisfaction scores (P = 0.5,0.7 and 0.3) in comparison to the baseline values at 1-, 3- and 6- months, respectively between both groups. Adverse events were observed in only 3 patients (5.3%). In group I, mild erectile dysfunction and post micturition dribbling were reported in one patient each. Where in group II, one patient reported bleeding per urethra (P = 0.5). To conclude, injection of botulinum-A toxin into bulbospongiosus seems to be safe but failed to prove clinical efficacy for treatment of lifelong premature ejaculation when compared to placebo.
