    Abstract Background: Gastroesophageal reflux (GERD) is a concern after peroral endoscopic myotomy (POEM). Transoral incisionless fundoplication (TIF) has been recently described as a possible therapy for post-POEM GERD in case series.
    Methods: We prospectively enrolled patients undergoing POEM who agreed to participate in objective post-procedure GERD evaluation. Patients with objective evidence of GERD and suitable anatomy were offered TIF vs. proton pump inhibitor (PPI) only. Patients who underwent TIF were compared to those on PPI-only therapy after follow-up.
    Results: Of 21 enrolled POEM patients with objective testing, GERD was found in 11 (52%). Of those eligible for TIF, 4 (40%) opted to pursue TIF and were compared to those on PPI-only therapy (n = 6). Three months post-TIF, 75% of patients had discontinued or significantly decreased PPI. There were no adverse events. GERD health-related quality of life scores were low and comparable between TIF (3.75 ± 6.2) and those who remained on PPI-only therapy (4.1 ± 5).
    Conclusion: In this pilot, patient-driven prospective study, 75% of patients with post-POEM GERD undergoing TIF had stopped or significantly reduced PPI use. Post-POEM TIF is safe and effective and may be a viable alternative to PPI for POEM-related GERD; however, future studies should include a control arm and post-intervention pH monitoring.

    Abstract Objective: To determine whether intravenous indigo carmine provides a visualization advantage compared to saline in the evaluation of ureteral patency in a randomized, controlled clinical trial.
    Methods: Patients undergoing urological or gynecological surgical procedures in which the patency of the ureter was to be assessed received a saline injection and were randomized to receive 2.5 mL or 5.0 mL of indigo carmine. Blinded video assessments were conducted by independent reviewers using a conspicuity scale ranked 1 (poorest) to 5 (best), and subjects with scores ≥3 and at least a + 1-point difference from saline were considered responders. Time to visualization was recorded for indigo carmine. A responder analysis evaluated whether indigo carmine showed improved visualization.
    Results: There were 96 ureters evaluated with the 5.0 mL dose of indigo carmine, 92 with the 2.5 mL dose, and 180 with saline. Most ureters were scored a 4 or higher on the conspicuity scale following indigo carmine; both doses were significantly better than saline (P < .0001). Overall, 92.3% of patients were rated as a responder for either ureter. The median time to visualization of blue color was not significantly different (6.0 minutes in the 5.0 mL group and 5.9 minutes in the 2.5 mL group). There were no adverse events related to indigo carmine use.
    Conclusion: Both dose levels of indigo carmine were significantly better than saline as a visualization aid for ureter patency.

    Abstract Background: Silver ions act as potent antimicrobial agents. Silver coating of brackets and the archwires can help reduce the formation of white spot lesions and caries which is commonly seen with fixed orthodontic treatment. However, this may affect the friction and surface roughness of the bracket-wire assembly which in turn affects the biological tooth movement.
    Material and methods: A total of 60 samples were included in the study which was divided into four groups. Group-1: • 15 silver coated 0.022 x 0.028" slot MBT prescription maxillary central incisor brackets • 15 silver coated 0.019 x 0.025" stainless-steel wires; Group-2: • 15 uncoated 0.022 x 0.028" slot MBT prescription maxillary central incisor brackets • 15 silver coated 0.019 x 0.025" stainless-steel wires; Group-3: • 15 silver coated 0.022 x 0.028" slot MBT prescription maxillary central incisor brackets • 15 uncoated 0.019 x 0.025" stainless-steel wires; Group-4: • 15 uncoated 0.022x0.028" slot MBT prescription maxillary central incisor brackets • 15 uncoated 0.019 x 0.025" stainless-steel wires. All brackets and wires used were of American Orthodontics, St. Paul, USA. Surface modification of wires and brackets was carried out using the Vacuum Coating Unit model by Thermal Vacuum Evaporation method with silver nanoparticles (10 nm size). The frictional resistance of all brackets and wires was checked using Universal Testing Machine.
    Results: On comparison of maximum load, it was found that friction was highest in group 3, followed by group 1, group 4 and group 2. The mean difference between all groups was found to be statistically significant with a
    Conclusions: This study concluded that friction was least when only the wire was coated with silver and the bracket was uncoated and it was the most when the bracket was coated and the wire was uncoated. The surface roughness after the friction test was the least when the wire was uncoated.

    Abstract Introduction: The clinical application of the diode laser seems to have beneficial effects in surgery. The laser provides precise and rapid tissue dissection, excellent haemostasis and lessens post-operative inflammation. This study was done to assess the clinical effectiveness of diode laser in the management of different varieties of intraoral soft-tissue lesions.
    Materials and methods: After applying exclusion criteria, a total of 50 samples were selected with benign intraoral soft-tissue lesions treated with a diode laser under local anaesthesia between September 2017 and September 2020. Each patient was followed up on the 3

    Discussion: Diode laser seems to be superior over conventional treatment modalities as it provides a bloodless dry field and increases patient comfort during surgery. It also lessens the post-operative pain and swelling. Diode laser can safely and effectively be used as a treatment modality for benign soft-tissue lesions, without any complication and without compromising the health and function of patients.
