    Abstract BACKGROUND: All-cause mortality risk and causes of death in bronchiectasis patients have not been fully investigated. The aim of this study was to compare the mortality risk and causes of death between individuals with bronchiectasis and those without bronchiectasis. METHODS: Patients with or without bronchiectasis determined based on chest computed tomography (CT) at one centre between 2005 and 2016 were enrolled. Among the patients without bronchiectasis, a control group was selected after applying additional exclusion criteria. We compared the mortality risk and causes of death between the bronchiectasis and control groups without lung disease. Subgroup analyses were also performed according to identification of Pseudomonas or non-tuberculous mycobacteria, airflow limitation, and smoking status. RESULTS: Of the total 217,702 patients who underwent chest CT, 18,134 bronchiectasis patients and 90,313 non-bronchiectasis patients were included. The all-cause mortality rate in the bronchiectasis group was 1608.8 per 100,000 person-years (95% confidence interval (CI), 1531.5–1690.0), which was higher than that in the control group (133.5 per 100,000 person-years; 95% CI, 124.1–143.8; P < 0.001). The bronchiectasis group had higher all-cause (adjusted hazard ratio (aHR), 1.26; 95% CI, 1.09–1.47), respiratory (aHR, 3.49; 95% CI, 2.21–5.51), and lung cancer-related (aHR, 3.48; 95% CI, 2.33–5.22) mortality risks than the control group. In subgroup analysis, patients with airflow limitation and ever smokers showed higher all-cause mortality risk among bronchiectasis patients. Therefore, we observed significant interrelation between bronchiectasis and smoking, concerning the risks of all-cause mortality (P for multiplicative interaction, 0.030, RERI, 0.432; 95% CI, 0.097–0.769) and lung cancer-related mortality (RERI, 8.68; 95% CI, 1.631–15.736). CONCLUSION: Individuals with bronchiectasis had a higher risk of all-cause, respiratory, and lung cancer-related mortality compared to control group. The risk of all-cause mortality was more prominent in those with airflow limitation and in ever smokers.

    Abstract To investigate changes in the physical and chemical properties of high-density polyethylene (HDPE) upon the rapid release of hydrogen gas at a pressure of 90 MPa, several characterization techniques have been employed, including optical microscopy, scanning electron microscopy, X-ray diffraction, differential scanning thermal analysis, and attenuated total reflectance Fourier-transform infrared spectroscopy. The results showed that both physical and chemical changes occurred in HDPE upon a rapid release of hydrogen gas. Physically, a partial hexagonal phase was formed within the amorphous region, and the overall crystallinity of HDPE decreased. Chemically, hydrogenation occurred, leading to the addition of hydrogen atoms to the polymer chains. Oxidation also occurred, for example, the formation of ester -C=O groups.     Abstract This study investigates the impact of high-pressure hydrogen gas exposure on the structural and morphological characteristics of O-ring materials. O-ring specimens undergo two cycles of sealing under 70 MPa hydrogen gas, and their resulting variations are examined using advanced characterization techniques, including powder X-ray diffraction (PXRD), small-angle X-ray scattering (SAXS), scanning electron microscopy (SEM) and atomic force microscopy (AFM). Our findings reveal that the lattice parameters of the O-ring material show no significant changes when exposed to 70 MPa hydrogen gas. However, in the micrometre range, the formation of a hierarchical channel morphology becomes evident. This morphology is accompanied by the separation of carbon black filler from the rubber matrix, contributing to mechanical weakening of the O-ring. These observations can be attributed to the pressure gradient that develops between the inner and outer radii of the O-ring, resulting from compression forces acting perpendicularly to the radial direction due to clamp locking.

    Abstract Unexpected viral infections outbreaks, significantly affect human health, leading to increased mortality and life disruption. Among them is the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) which emerged as a deadly pandemic, calling for intense research efforts on its pathogenicity mechanism and development of therapeutic strategies. In the SARS-CoV-2 cytokine storm, systemic inflammation has been associated with severe illness and mortality. Recent studies have demonstrated special pro-resolving lipids mediators (SPMs) lipoxins, resolvins, maresins, and protectins as potential therapeutic options for abnormal viral-triggered inflammation. Pro-resolving lipids mediators have shown great promise for the treatment of Herpes simplex virus, respiratory syncytial virus, human immunodeficiency virus, and hepatitis C virus. Based on this, studies are being conducted on their therapeutic effects in SARS-CoV-2 infection. In this review, we discussed SPMs and reviewed evidence from recent studies on SPMs as therapeutic options for viral infections, including SARS-CoV2. Based on our analysis of the previous study, we argue that SPMs are a potential treatment for SARS-CoV-2 infection and other viral infections. We expect further research on how SPMs modulate viral-triggered inflammation through G-protein-coupled receptors (GPCRs), and chemical stability and druggability of SPMs.

    Abstract Hypoglossal nerve stimulation (HNS) is indicated for obstructive sleep apnea (OSA) but is ineffective in treating central sleep apnea (CSA). We describe two patients implanted with HNS after being diagnosed with OSA at outside sleep labs and failing a trial of continuous positive airway pressure (CPAP) therapy. Despite successful HNS implantation, the patients continued to have persistent symptoms with residual apnea-hypopnea indices above 25 events/h. Although OSA was the presenting diagnosis, we discovered a significant CSA component in the original diagnostic sleep data upon careful review. One patient was confirmed to have a CSA-predominant sleep disorder and improved with adaptive servo ventilation therapy. The other was diagnosed with central sleep apnea and severe periodic limb movement disorder, and improved with medication. Based on these sleep apnea cases, we propose guidelines emphasizing the importance of reviewing basic clinical information upon treatment failure and initiating multi-disciplinary collaboration early in the treatment course.
