    Abstract La atribución de significado a los segmentos de información propia del análisis de textos abiertos en investigación cualitativa implica una considerable inversión de tiempo. Las herramientas de Procesamiento de Lenguaje Natural pueden convertirse en un recurso para el investigador cualitativo, en cuanto que sus algoritmos permiten interpretar los textos y codificar cualitativamente de forma mucho más rápida. Esta tarea, sin embargo, requiere probar previamente el nivel de comprensión verbal de estas herramientas. La aparición del modelo GPT-3 de OpenAI ha supuesto un salto cualitativo respecto a modelos de Procesamiento de Lenguaje Natural anteriores. El objetivo fue analizar la capacidad de comprensión verbal de esta herramienta. Se aplicaron las pruebas del índice de comprensión verbal de la batería para la medición del cociente intelectual WAIS-IV. Los resultados de las pruebas de fiabilidad fueron satisfactorios. Respecto al nivel de comprensión verbal, las respuestas situaron a GPT-3 en un percentil superior al 99, comparado con los estándares humanos. Estos resultados demuestran que es posible utilizar este modelo como herramienta para el análisis de textos abiertos, abriendo enormes posibilidades para la investigación cualitativa, aunque su uso debe basarse en una utilización precisa y diseñada ad hoc para cada proceso de análisis.

    Abstract An electrochemical biosensor for creatinine determination in a drop of whole human blood was developed and applied to the determination of creatinine in real clinical samples. It is based on the modification of a dual carbon working electrode with a combination of three enzymes: creatinine amidohydrolase (CNN), creatine amidinohydrolase (CRN) and sarcosine oxidase (SOX). Electrochemical transduction is performed using horseradish peroxidase (HRP) and potassium hexacyanoferrate(II) as mediator. A drop of human blood is enough to carry out the measurements by differential chronoamperometry where one carbon electrode detects creatine and the other both creatine and creatinine. The integrated differential signal obtained in the biosensor is linear with the concentration of creatinine in blood in the range 0.5-15 mg/dL and the enzyme-modified electrodes are stable for at least 3 months at 4 °C. The biosensor was lined to a reference method based on Isotope Dilution Mass Spectrometry (IDMS) with 50 real human blood samples and the results compared with those obtained by alternative routine techniques based on Jaffé method and an enzymatic method (Cobas 8000 Roche®, Crep2 Roche®). There were no significant differences between the creatinine concentrations found by the routine techniques and the developed biosensor.

    Abstract Background: Obsessions and compulsions are heterogenous but can be classified into obsessive-compulsive disorder (OCD), body dysmorphic disorder (BDD), hoarding disorder (HD), hair-pulling disorder (HPD), and skin-picking disorder (SPD). OCD is in itself heterogenous, with symptoms clustering around four major symptom dimensions: contamination/cleaning, symmetry/ordering, taboo obsessions, and harm/checking. No single self-report scale captures the full heterogeneity of OCD and related disorders, limiting assessment in clinical practice and research on nosological relations among the disorders.
    Methods: To provide a single self-report scale of OCD and related disorders that respects the heterogeneity of OCD, we expanded the DSM-5-based Obsessive-Compulsive and Related Disorders-Dimensional Scales (OCRD-D) so that is also includes the four major symptom dimensions of OCD. A psychometric evaluation and an exploration of the overarching relations among the dimensions were conducted using an online survey which was completed by 1,454 Spanish adolescents and adults (age span = 15-74 years). Approximately 8 months after the initial survey, 416 participants completed the scale again.
    Results: The expanded scale showed excellent internal psychometric properties, adequate test-retest correlations, known groups validity, and correlations in the expected directions with well-being, depression/anxiety symptoms, and satisfaction with life. The higher-order structure of the measure indicated that harm/checking and taboo obsessions formed a common disturbing thoughts factor and that HPD and SPD formed a common body-focused repetitive behaviors factor.
    Conclusion: The expanded OCRD-D (OCRD-D-E) shows promise as a unified way to assess symptoms across the major symptom dimensions of OCD and related disorders. The measure may be useful in clinical practice (e.g., screening) and research, but more research on construct validity, incremental validity, and clinical utility is needed.

    Abstract El objetivo de este trabajo es presentar los fundamentos conceptuales de la asignatura optativa Cine, Literatura y Medicina que se imparte en segundo año del grado en Medicina de la Universidad de Oviedo desde el curso académico 2011-2012. Los contenidos que se comentan suponen una introducción general a las formas en las que el cine y la literatura pueden contribuir a la enseñanza de la medicina y de cómo pueden contribuir a la exposición de los estudiantes a aspectos transversales de la medicina dispersos a lo largo de los cursos del Grado. El lenguaje cinematográfico facilita el abordaje de temas complejos en los que las cuestiones éticas y sociales de las enfermedades pueden tratarse mediante la interpretación por actores de forma clara, concisa y dirigida a los elementos nucleares de problemas densos que de otra forma sería dificultoso y lento comprender. La literatura, tanto mediante la escritura como de la lectura, permite aliviar la tensión emocional que produce la enfermedad, interiorizar de forma paciente y reflexiva la experiencia de enfermar, y reorientar la actividad vital en consonancia con la imposición de limitaciones que son propias y características de cada proceso nosológico.

    Abstract La Primera Vuelta al Mundo, la mayor gesta marinera de la Historia, de la que ahora se cumplen 500 años, tuvo una gestación que se remonta al proyecto de Colón de llegar al Extremo Oriente navegando en dirección contraria y aprovechando la ya prevista de antiguo esfericidad de la Tierra, aunque fallara en establecer sus dimensiones correctas. Insistiendo en esa idea tras el descubrimiento accidental de América, especialmente tras nuevos descubrimientos como el de Vasco Núñez de Balboa, acometieron la tarea de cruzar el nuevo y más extenso océano. El proyecto de Magallanes era aún más preciso y limitado: llegar a las islas de las especias por otro camino del iniciado por Portugal, lo que provocó la animosidad del vecino reino que había comenzado sus exploraciones por una vía más tradicional, contorneando África y llegando al Índico y al Extremo Oriente. Muerto Magallanes antes de llegar a su objetivo, la decisión de la circunnavegación del planeta fue decisión de Elcano. Y el crucial “tornaviaje” fruto de la aportación de Urdaneta.

    Abstract The purpose of this paper is to describe the characteristics of the dual-purpose water buffalo production based on the Mexican production system as a model in tropical wetlands. It includes a broad literature review emphasizing the most recent and specialized publications examining key findings to improve our understanding in the performance of the buffalo species (Bubalus bubalis). The complementary topics addressed include reproductive management, parturition, the dam–calf bond, milking routines, and models of confinement and management, in addition to aspects related to milk commercialization. This article summarizes the advances made to date in this production system and its current margins for improvement. The development of dual-purpose water buffalo production systems in Mexico’s tropical wetlands is a relatively recent phenomenon that has progressed and improved due to herd management. Buffaloes are an interesting alternative for dual purpose systems that offer several advantages. The lower milk production of this species compared to cattle is its main limitation. However, the properties of their milk allow one to obtain an added value and make this type of farms competitive. In synthesis, consolidating buffalo production in Mexico’s tropical wetlands will require broadening our knowledge of this species, and perfecting the most appropriate handling procedures. The activities of government agencies and processing enterprises will play vital roles in achieving the integral modernization of this potentially important economic activity.

    Abstract Despite the negative repercussions of a chronic disease, multiple sclerosis (MS) might also lead to positive consequences. This longitudinal study explored post-traumatic growth in MS patients and attempted to identify possible determinants. Post-traumatic growth of 260 patients and their caregivers was compared. A subset of 209 patients and caregivers were evaluated at baseline. Patients filled in the Posttraumatic Growth Inventory and General Health Questionnaire at three different times over a 36-month follow-up period.     Abstract (1) Background: Patients' behavioral attempts in dealing with Multiple sclerosis (MS) play an important role in post-traumatic growth (PTG). In a longitudinal study, we aimed to identify coping strategies predicting PTG. (2) Methods: 260 MS patients answered the Post-traumatic Growth Inventory and the Brief COPE Questionnaire at three time points during a 36-month follow-up period. (3) Results: an interaction effect between PTG level and assessment time was found for emotional support, positive reframing, active coping, and planning coping strategies. Positive reframing, emotional support, instrumental support, religion, planning, and self-distraction positively predicted PTG. (4) Conclusions: to encourage PTG development, early interventions in MS patients are recommended to promote adaptive coping, particularly positive reframing, social support, active coping, planning, religion, and self-distraction.
