    Abstract Evaluar la sostenibilidad de los sistemas agricolas campesinos representa un reto a nivel global ya que la agricultura familiar produce mas del 70% de los alimentos del planeta. En Nicaragua, el 49% de la poblacion rural practica agricultura familiar, siendo el 38% de subsistencia. La zona de estudio se situo en 11 comunidades del municipio de San Jose de Cusmapa (departamento de Madriz, Nicaragua) donde se realizaron 80 encuestas semiestructuradas. Se analizaron los indicadores de: rendimiento, eficiencia energetica, tasa beneficio/coste agricola, riesgo de erosion, agrobiodiversidad, acceso a innovaciones tecnologicas y participacion en las organizaciones. Esta agricultura campesina esta muy lejos de las objetivos de sostenibilidad marcados y se manifiesta en carencias alimenticias y nutricionales. Entre los indicadores analizados cabe destacar los bajos valores alcanzados en el indicador rendimiento y en la agrobiodiversidad. Por ello, es nevesario que las comunidades campesinas fortalezcan su capacidad de adaptacion con el fin de mejorar su nivel de vida y la salud de su ecosistema. Sustainability evaluation of peasant agricultural systems is becoming a global challenge due to the fact that family farms produce over 70% of food supply. In rural areas of Nicaragua family farm involve about 49% population and the subsistence agriculture represents 38 percent. The study area was located in 11 communities in the municipality of San Jose de Cusmapa (county of Madriz, Nicaragua) where 80 semi-structured interviews were conducted. The sustainability indicators: crop yield, energy efficiency, agricultural benefit/cost ratio, erosion risk, agrobiodiversity, technology availability and community participation were analyzed. It shows an unsustainable handling of both natural resources and farming systems, levels of extreme poverty manifested in food shortages and nutritional deficiencies. Among the analyzed indicators, it should be noted the low values of crop yield and agrobiodiversity. It will, therefore, be necessary to uplift the adaptation capacity of peasant communities, in order to improve their standard of living and the ecosystem's health.

    Abstract Se realizó un estudio comparativo para determinar el comportamiento de algunos aspectos epidemiológicos de la enfermedad cerebrovascular, durante los períodos enero de 2002-agosto de 2003, y enero de 2010-agosto de 2011, en el Policlínico Docente José Antonio Echeverría, de Cárdenas. El universo quedó constituido por 153 pacientes pertenecientes al área de salud e ingresados en el Hospital Julio M. Arístegui Villamil, de la misma ciudad, con dicho diagnóstico en los períodos referidos, de los cuales 66 correspondieron al primer período y 87 al segundo, respectivamente. Mediante la revisión de las historias clínicas del hospital y de las historias clínicas individual y familiar del consultorio, se obtuvieron las variables estudiadas. El sexo masculino y los mayores de 60 años fueron los más afectados en ambos períodos. La hipertensión arterial estuvo como factor de riesgo en el 50 % de los casos durante el 2002-2003, y en un 79,3 % en el período 2010-2011. El tipo de enfermedad cerebrovascular más frecuente fue el isquémico, con un 91 % en el primer período, y de estos pacientes, el 81,8 % ingresó en salas abiertas, mientras que en el segundo período, la enfermedad cerebrovascular isquémica ocupó el 84,0 % y el 77,1 % de los pacientes fue hospitalizado en dichas salas. Durante el primer período analizado, el 61,3 % de los pacientes no estaba dispensarizado en el área de salud, y en el segundo período el 83,1 %. Se concluye que hay insuficiencias en el manejo de estas entidades, tanto en la atención primaria como en la secundaria. We developed a comparative study to determine the behavior of some epidemiologic aspects of the cerebrovascular disease, during the period from January 2002 to August 2003, and from January 2010 to August 2011, in the Teaching Policlinic José Antonio Echeverria, of Cardenas. The universe was formed by 153 patients of the health area, who entered the Hospital Julio M. Arostegui Villamil with that diagnosis in the mentioned periods, 66 of them in the first period, and 87 in the second respectively. Reviewing the clinical records of the hospital and the familiar and individual clinical records of the family doctor offices, we obtained the studied variables. The male patients aged more than 60 years were the most affected in both periods. Arterial hypertension was a risk factor in 50 % of the cases during the period 2002-2003, and in 79,3 % of the cases in the period 2010-2011. The most frequent kind of cerebrovascular disease was the ischemic one, with 91 % in the first period (81,8 % of these patients entered open wards, while in the second period the ischemic cerebrovascular disease represented 84,0 % and 77,1 % of the patients were in-patients of those wards. During the first period, 61,3 % of the patients were not treated in the health area, and during the second period 83,1 %. We concluded that there are deficiencies in the management of these diseases, as much in the primary health care as in the secondary one.

    Abstract Tall wheatgrass (Thinopyrum ponticum, cv. 'Jose') (TWG) has been identified as a salt-tolerant forage that has acceptable nutritional value and shows considerable promise for reducing saline drainage volumes in California's San Joaquin Valley (SJV). The SJV drainage water also contains high concentrations of boron (B), selenium (Se) and molybdenum (Mo), which may affect the production potential and mineral nutritional value of the forage. A greenhouse study was conducted with soil-filled pots using irrigation waters that varied in B (0.7-20mgL⁻¹) and sodium-sulfate dominated salinity (0.5-20dSm⁻¹), a quality characteristic of the SJV, with a constant background of 0.5mgL⁻¹ Se and Mo. Our experiment confirms that TWG is very tolerant to salinity. For example forage production in treatments with irrigation water with an ECw of 10dSm⁻¹ (ECe of 16.8dSm⁻¹) was 74% of those irrigated with non-saline water. We found that the stable carbon isotope fractionation in shoot tissue was a good cumulative stress indicator of the crop as the discrimination value (Δ) decreased with increased salinity and reduced shoot biomass. Moreover, this forage crop is extremely tolerant to B, tolerating up to 20mgL⁻¹ in the irrigation water without a significant reduction in cumulative biomass. Tissue B concentrations increased with increased B to values above 2000mgkg⁻¹ dry matter (DM). However as salinity increased within a particular B treatment, tissue B decreased. Forage quality, from a ruminant mineral nutrition perspective, raised concerns. Forage samples contained high levels of B, Se, Mo and sulfur near or above the recommended maximum tolerable level (MTL). Although ruminant grazing of pastures containing forage of this quality is not recommended, it has considerable potential as a forage supplement if rations can be controlled. Livestock consuming forage of this quality should be monitored for signs of B, Se and sulfur toxicity and molybdenum-sulfur induced copper deficiency.

    Abstract Se presenta la caracterización forístico-estructural de un sector del bosque ribereño del río San José, Reserva Forestal Imataca, estado Bolívar, Venezuela. Utilizando transectos, colecciones botánicas y observaciones de campo, se describen la estructura y composición de tres comunidades en la planicie de inundación del río: la primera, un bosque medio con dominancia de Croton megalodendron, Carapa guianensis, Pentaclethra macroloba, Eschweilera decolorans, Protium sp., Licania densifora, Guarea sp. y Neea bernardii; la segunda, también un bosque medio, presenta Carapa guianensis, Eschweilera decolorans, Aspidosperma marcgravianum, Licania densifora, Pentaclethra macroloba y Guarea sp.; la tercera corresponde a un bosque bajo con Croton megalodendron, Spondias mombin, Cordia sp., Guazuma ulmifolia, Gustavia augusta y Protium sp. Se incluye una lista con las 77 especies registradas. The floristic composition and structure of a section of the San Jose river’s riparian forest, Imataca’s Forest Reserve, Bolívar State is presented. Through transects, botanical collections and observations, the composition and structure of three forest types along the river food plain is described: the first belongs to a medium sized forest dominated by Croton megalodendron, Carapa guianensis, Pentaclethra macroloba, Eschweilera decolorans, Protium sp., Licania densifora, Guarea sp. and Neea bernardii; the second, a medium sized forest, presents Carapa guianensis, Eschweilera decolorans, Aspidosperma marcgravianum, Licania densifora, Pentaclethra macroloba and Guarea sp.; the third corresponds a low forest with dominance of Croton megalodendron, Spondias mombin, Cordia sp., Guazuma ulmifolia, Gustavia augusta and Protium sp. A list of 77 species found is presented.

    Abstract Family and Community Medicine is the most offered and chosen specialty in the MIR (Spanish medical residency examination), however, every year its attractiveness is questioned due to not all offered positions being filled and a certain number of resident doctors deciding not to continue in this specialty once started. In this context, some of the proposals to address the problem focus on increasing the supply when the facts show that the challenge lies in addressing the demand by making the specialty and its professional scope more attractive. The problem and its determinants are analyzed in this context by focusing on four elements that may be influencing it: the vocational aspects of medical graduates who pursue specialization, the characteristics of the specialty program and the teaching units where training is carried out, the presence of family medicine in the university as a key element for knowledge and affinity to this specialty from undergraduate studies, and finally, the situation of primary care as the space where training is materialized and the priority setting for the professional practice of future specialists.
